INTRODUCTIONHealth and social care is a team of workers who work together in a coordinated manner toincrease the effect and achieve higher results and outcomes for patients. They focus onmaintaining and protecting the interest of the individuals. It focus on maintaining health andsafety provisions to safeguard and protect the interest of the individual. This study will highlight, role of different health care professional who are involved inthe delivering care to the individuals. This study will also highlight mechanisms to report poorpractice in Health and Social care. This study will also include, roles and responsibilities of theinter-professional team in meeting care needs of individuals. It will also highlight, potentialbarriers to inter-professional working and effective measures to overcome such barriers forhigher results and outcomes.National Health Services (NHS) is a publicly funded health care system which focuses ondelivering health care services to individuals for higher results and outcomes. It was founded in1948 and is headquartered in Richmond House, London, England.TASK 1A.C. 1.1. Assessment of how health and social care professionals protect individuals in theircare.Health and social care is the services provided by the health care practitioners of NationalHealth Services (NHS). Health and social care is important as it helps in providing physical,emotional and social support top each individuals. NHS provides effective support to theindividuals for early recovery and effective results and outcomes. They focus on delivering care tindividuals with effective treatment for disease, illness, disability p0or injury. It also focus oneffective recommendation with specific measures and accommodation like care home for theirconvenience (Role of Health and Social Care Workers, 2019.). Health and social care focus on providing effective care to people by providing emotionaland physical support with effective treatment and necessary measures which leads to higherresults and outcomes. Health and social care provide effective treatment and care for familymembers and individuals. It helps in people who are facing trouble in old age or individuals whoare addicted to drugs and alcohol. It also provides care to physically disabled individuals andpeople who are facing financial issues. NHS focus on giving personalized care to individuals and

also involve family members of patients for effective decision making and higher results. NHSfocus in improving health and well being of individuals with enhanced care and quality.Various healthcare practitioners like nurses, doctors, dentist, clinical psychologist,chiropractor, family members come together to give effective quality and care services to theindividuals. Health and social care focus on giving treatment by considering all effectivemeasures to reduce the impact (Curtis and Burns, 2015). It focus on higher standard of living byconducting self management programmes and health coaching and peer support to lower theimpact and give proper support for higher outcomes of the individual. They take necessarymeasures to safeguard the interest and maintain hygiene for higher operational efficiency,performance and productivity by giving effective care services and support to the patients.NHS focus on complying with various laws and legislation, hygiene to protect andsafeguard each individual for better recovery and maintain healthy and positive lifestyle. NHSfocus on improving the health of the individual by giving effective treatment by completelyanalysing their health. NHS focus on better health and social care practices by improving patientsatisfaction and experience by providing effective services by the high professional staff orhealth care practitioners. They focus on primary care, integrated care, urgent and emergencycare, mental and physical health care, etc. they also tackle challenges like high alcoholconsumption, smoking, drug addiction and obesity.A.C. 1.2. Evaluation of the mechanisms for reporting poor practice in Health and Social care.Poor practice means when health care practitioners fails to deliver good quality care andsupport to individuals. In case of negligence and effective maintenance of standards leads tolower operational standards. Poor practice in health and social care occurs when staff membersignore the rights and do not maintain specific standards into the NHS may lead to increase inabuse and cause harm to the patients and individual's family members. Supportingworkers,personal; assistants, care workers, front line managers, senior organizational managersand other health care practitioners must perform while maintaining specific standards whichgives higher results and effective recovery of patients as quickly as possible (Costa-FontandGreer, 2016).Increase in poor practice in health and social care is due to increase in workload, complexorganizational structure or system, lower staff capacity, rice in sickness and illness, advance intechnology and higher targets.
