INTRODUCTIONHealth assessment is a plan that outlines the specific needs of the individual and howthose needs could be addressed by health care system. According to the constitution of Indiahealth care is the responsibility of the state governments to raise the level of nutrition and thestandard of living of individual by improving public health services (Zeanah, 2018). The presentassignment focuses on the primary, secondary and tertiary health care services that caters theneeds of the individual. It also elaborates Health watch and Clinical Commissioning Group.1.Definition and an example of the followingPrimary Care It is the day to day health care facilities provided to the individuals according to theirneeds. These are the providers who act as a first contact for the continuous care of patients andcoordinates with other specialist according to the patients needs. Primary care providers are thedoctors, nurse practitioners and the physician assistants. Primary Care Support England is theprimary care service provider in UK which fulfils needs of the individual. Secondary CareIt is the medical care facility that is provided by the specialist by the referral of theprimary care physician. It requires more specialised knowledge, skills and equipments than theprimary care health providers. It is the specialist focus on the patients specific system of the bodyor a specific disease or condition (Estes, 2013). This is the condition when primary health careproviders are unable to find solution to the problems and end up referring to the secondary healthcare providers. Clinical Assessment and treatment services provides excellent secondary careservices all over UK. Tertiary Care These are the specialized consultative care service providers which provides specialisedhealth care facilities by working in a centre that has a personnel and facilities for specialinvestigation and treatment. These are the physicians which are specialised in neurology,cardiology and other specialised fields. Therefore NHS Provides appropriate health care services in to meet the needs of theindividual. It is a publicly funded national health care services throughout UK. 1
