INTRODUCTION
Nursing clinical reasoning refers to a procedure through which nursing staff can collect
cues, process the information, come to an understanding of a patient problem or situation. It
include analyse the actual medical condition of an ill individual to plan & implement
interventions, evaluate outcomes and reflect on the same (Rice and et. al., 2014). The given
report is based on selected case study about Jack having problem of cancer of colon. It will also
include immediate and short term problems after having partial colectomy and formation of
colostomy.
MAIN BODY
Gathering further information about Jack's condition
According to the given case scenario, Jack was suffering from cancer of colon and has
partial colectomy and formation of colostomy. It is necessary to monitor actual condition of
patients by evaluating respiration rate, pulse rate, temperature, blood pressure, SpO2, urine
output and blood glucose level (BGL). However, these tests are helpful to analyse actual medical
situation of Jack along with monitoring after effects of surgical process to take appropriate
actions to overcome with issues (Cummings and Connelly, 2016). Moreover, surgical process of
colectomy and colostomy may create severe complications which are required to be determined
earlier to take desired steps to solve particular issue.
Immediate and short term problems of Jack
Jack has partial colectomy and formation of colostomy which has certain after effects on
body in that few of them can be considered as immediate problems and rest of them are shot
terms issues. Initially, process of colectomy has number of immediate and short term problems
such as pain, fatigue, bleeding, blood clots, diarrhoea, constipation, a blockage in intestine,
inactive intestine, damage to nearby organs, infection,inability to control urination, need to
urinate often and intense need to urinate (Cazzell and Anderson, 2016). In addition to this,
immediate and short term impacts of colostomy can be consider several issues including
excessing bleeding, surgical wound infection, thrombophlebitis, pneumonia and pulmonary
embolism. It also include increased pain, swelling, redness, drainage, headaches, muscle aches,
dizziness, fever, nausea and tarry stools.

Nursing clinical reasoning can be defined as process of collect information and process it
to understand particular condition of an individual. It include the criteria of planning,
implementing interventions, evaluating outcomes and then provide reflection (Duane and Satre,
2014). However, nursing reasoning cycle include several stages including consider patient,
collect information, process data, identity problems, establish goals, take action, evaluate
outcomes and reflect on process & new learning.
In given case of Jack, few of stages can be used as he required care after surgical process
i.e. colectomy and colostomy. The few of stages can be used to provide after care of surgery to
patient including collect information, process data, determine problems and take action
(Moorhead and et. al., 2018). Moreover, it is necessary to gather information about after
symptoms through monitoring temperature, pulse rate, respiration rate, blood pressure and SpO2.
It will facilitate to analyse collected data and determine actual problem of Jack in order to take
appropriate actions for well-being of him.
CONCLUSION
The above report has been concluded that nursing clinical reasoning can be described as
to collect data and analyse that to determine health problem of patient. It include few tests to
gather further information like pulse rate, temperature, respiration rate, blood pressure etc. to
monitor health of patient.
