Task 1
Consideration of the patient
In Peter’s scenario it can be seen that he is suffering from COPD, which is a very crucial
disease and in most people this disease becomes chronic. This disease possess obstruction on
flow of air from the lungs. The most important symptoms of this disease include suffering from
difficulties in breathing, cough and production of sputum. While doing the considerations of the
patient, the nurse need to focus on the nursing assessments, the diagnostic reports that the man
had till now and also his medical reports. The nurse must consider the psychological conditions
of the patient along with his sexual, functional, emotional conditions and the age. The medical
background history of the patient need to be considered (Vaismoradi et al., 2014). In this case the
patient is suffering from chronic obstructive pulmonary diseases. The main causes has already
been described that the man is a heavy smoker and also drink socially. At present, the patient has
been found to be suffering from chronic obstructive pulmonary disease. The treatment of this
disease a much time taking process. The two common conditions that lead to the cause of this
disease are chronic bronchitis and emphysema (Köberich & Farin, 2015). The background
medical history is evidenced from the breathlessness that the man is suffering. The disease has
effected the lining of the bronchial tubes which has got inflamed. The alveoli at the end of the
smallest air passages are also destroyed. The nurse must at first consider all the present
conditions and the background medical histories of the patients (oskenniemi, LeinoKilpi &
Suhonen, 2015). At first The nurse must make sure whether the patient will be able to recover
within a short period of time or he will take a long time to be cured. All the nursing documents
including the details of the vital signs of the patients and also the progression report of the
patient are needed to be prepared. In the present case, the nurse should check chest size of Peter
as he is suffering from COPD, should check the oxygen level of the patient along with the pulse
rate. Finally, analysis of the changes in the vital signs should be done from before. The nurse
need to note down the outcomes supposed to receive after the treatment period is over and then
do the analysis how much the patient has recovered from before (Kelly et al., 2016).

Task 2
Nursing assessments.
For doing the nursing assessment at first the nurse need to completely go through the
clinical reasoning cycle. The nursing assessment includes all the information about the patient’s
physiological and psychological status of the patient. The first important assessment of nursing is
the admission assessment. The nurse must know carefully that whether this person has allergies
against some medicines or not. The nurse must know in details that the man has already been
immunized before or not, any injections have been given regarding the pulmonary diseases or
not (Arakelian et al., 2017). In the present case scenario, the man has been found to be suffering
from COPD so he might have taken medicines of bronchitis. She must do the thorough check up
about the medicines that Peter has taken till now. The physical and emotional needs differs from
person to person and also vary a lot according to different ages. Children, adolescents and old
persons have different needs and requirements. In the given case, the patient is a man of 44 years
of age. So he does not need much emotional supports, he just need the treatment of the diseases,
the medicines and also the diagnoses from time to time. The vital signs of the patients like the
blood pressure, body temperature, pulse rate are also needed to be checked and recorded from
time to till (Peplonska, Bukowska & Sobala, 2015).In this case, the person is suffering from
pulmonary diseases, so the oxygen carrying capacity must also be checked from time to time.
Peter must do the spirometry test and this test can be done after the inhalation of a medicine like
albuterol (Lamprecht et al., 2015). It increases the path of the airways so that air can pass
thorough the way. Shift assessment is another important assessment for a nurse and she should
do this assessment for each and every patient seeking care from her (Cornell et al., 2013). For the
given patient, the nurse definitely not going to spent the whole day in the hospital so she should
regularly check the vital signs of the patient and also note down the improvements that the
patient has been making because of this treatment. She should note down all the changes in the
vital signs and analyse how the patient has changed from the diseased conditions to normal
conditions. The last important assessment criteria is the focussed assessment. In this assessment
the nurse need to carefully assess the specific systems that are found to be effected by the person.
In the given case study, the patient is suffering from chronic obstructive pulmonary diseases. So
the effected systems of the patient’s body are the respiratory systems and the circulatory system
