Introduction
Detailed information is required to diagnose the problem, which the patient is facing is
provided by the clinical reasoning cycle (CRC). It is a process applied by the nurses to collect
information, indications and to process the gathered information for comprehending the patient's
problem. Suitable nursing intervention is then arranged following the diagnosis of the patient's
problem. Pharmacological intervention is observed in Ted's care plan routine, and necessary
signs are provided in case analysis. The impression of Ted's surgery upon himself, and his family
and the link to pathophysiology from literature evidence is comprehended in the current
discussion. The strategies for the care planning of Ted and the purpose of nursing interventions
are subsequently included. Finally, pharmacological intervention for organizing Ted's port
operative condition is added.
CRC analysis using RLT
The initial stage of the Clinical Reasoning Cycle or CRC comprises the comprehension of
Ted's condition in detail. It is because the patient's biopsychosocial as well as the cultural and

significantly seen in his case. The discussed patient is a widower and lives by himself. The only
family that he has is his daughter who resides in a distant city. Gwen, his partner is considerably
aged and lives in the same apartment but a bit away from. Thus, Ted necessitates sustained
pharmacological interventions as well as nursing care to deal with his present health condition.
His folks will be bothered for his biopsychosocial impressions and the considering methods of
treating his condition.
The impact of Ted's surgery on both the cultural and spiritual aspects is also to be considered
in his nature. The attributes even become worse due to his old age and health complexities. To
overcome such a situation, he will need to obtain cultural and spiritual support.
Problem Identification from cues collected
Necessary and relevant ideas from stage 2 of CRC will be gathered. Along with that, the signs
or symptoms will also be accumulated from the case study in order to take action on the third
stage of CRC. Then, the collected information will be executed in the processing information to
detect the pathophysiological issues concerning the condition of the patient. In this conferred
case, Edward Ted is leading his 80s and has undergone a surgical formation of temporary
colostomy just after 4 days of his bowel resection surgery. The comorbidities that he is involved
with include obesity, heart failure, and identification of malignant mass through coloscopy, type
II diabetes melilites and gout. He has vomited twice due to his nauseous feeling. His overall
critical health problems include high blood pressure at 135/85, fever with T 38.1, detection of
