Introduction
“Case” refers to a project or a transaction which is either “opened” or “closed” over a
passage of time. It can help in achieving resolution in relation to a problem or a claim. Case
Management deals with the operation of a bundle of content instead of the individual
documents (Vourlekis, 2017). Case Management helps in improving performance of an
organization that helps in putting the case information at the center. This report discusses
about the role played by collaborative practice in relation to case management. It also makes
use of examples for showing features pertaining to successful collaboration. It also talks
about the challenges that has to be overcome in the case of case management.
Role played by collaborative practice in case management
The role of collaborative practice is to facilitate with the various stakeholders in
relation to diagnosis and the treatment plan. Collaborative practice helps in achieving goals
that can help in the aspect of health recovery of a patient. Collaborative practice helps in
reaching the identical objective by working along with other people that can help in achieving
shared goals (Reilly et al., 2015). Collaboration helps the case managers in advocating for
patient, healthcare team along with payer. The practices of collaboration can help in
facilitating positive outcome for patient and healthcare team. The humanistic nursing theory
looks after the emotional along with physical health of the patients at the time of case
management. The humanistic nursing theory can help in looking after the different
stakeholders that can help in ensuring emotional along with physical health of the patient.
Collaboration can be facilitated in relation to nursing by taking recourse to humanistic
nursing theory. Positive patient outcome can be reached with the help of collaborative
practice that can help in ensuring better quality for the patients. Collaboration can help in
sharing knowledge and in building consensus that can help in the arena of case management

the informal care givers. It can help in bridging the area of health and the social service. The
interdisciplinary care team can work with the patients for catering the needs of the society.
Collaboration can help in employing comprehensive assessment that can serve the needs of
the patient within the ambit of health care organization (Van Bergen et al., 2018).
Examples of successful collaboration
Nurse-led memory clinic ( NLMC) is run under the auspices of a nursing practitioner
that helps in providing the students with an alternative to that of seeing a consultant. The
Nurse Practitioner helped in making the experience of assessment in a positive light. There
were many patients who felt that they would visit Nurse Practitioner instead of a health
professional. The nurses in this clinics have time of communicating that helps in the area of
case management. The consultation times are more in the case of these clinics that can
facilitate the aspect of collaboration. The communication among the nursing practitioner in
the NLMC helped the patients in relation to their memory issues (Dodgson, 2018). This
example shows that the features pertaining to successful collaboration is communication that
can help in stating things clear in front of the nurses and the nursing practitioners. The wish
of the client was not overlooked in the event of the agencies working together. There were
the consultation team, nursing practitioner and the patient but each of these agents helped in
ensuring that more importance was given on the clients that proved to be of great help in the
area of case management (Wang, Persson & Huemer, 2016)
In the event of working for Australian Nursing Home Foundation, I saw that there
was a patient who was suffering from cardiovascular disease along with mental illness. The
health care experts were trying to treat the mental condition of the patients but they were

failing. In the end, the help of Mental Health Australia along with the services of Mental
Health Foundation in Australia was asked for resolving the case of the patient. The Mental
Health Foundation wanted a kind of counselling treatment to be given to employees that can
help the patient in recovering from the fragile mental condition. These agencies prioritised
the health of the patient that helped in solving the case.
Challenges to be overcome
The challenges that has to be overcome in case management relates to the fragmented
process that creates challenges in the case of care management. Health care data should be
seen in a holistic manner that can address in a proper manner the needs of the patients. There
is limited data access that prevents in the aspect of that of collaboration. The health care
organization should be able to negotiate and collaborate with the insurers that can help in the
aspect of getting the patient data (Phillips, 2015). The challenges for collaboration lie owing
to the fact that there is poor data quality that can help in identification of the patients. The
organizations are incapable of implementing the Enterprise Master Patient Index (EMPI) that
proves to be a barrier in relation to collaboration. The other challenges that has to be
overcome is the limited involvement in Information Technology. The data governance team
are not properly employed that becomes a hindrance in communication of the needs of the
patient. The barrier that is created in the aspect of collaboration is owing to the lack of the
standardization. For the aspect of standardization, resources are required who would have
both clinical along with operational context.
Conclusion
Collaborative process helps in facilitating with that of the different stakeholders
pertaining to diagnosis along with treatment plan. Collaborative practice can help in
achieving the goals that can recover the health of the patient. The sharing of knowledge can

be facilitated with the help of collaboration that can prove to be useful for building consensus
pertaining to that of case management. Collaborative practice can prove to be of great help in
supporting families and informal care givers. Challenges to be overcome pertains to
fragmented process that acts like a hurdle in the path of case management. The data of health
care should be seen in holistic manner that can help in addressing needs of patient. Limited
data access would prevent collaboration in a health care setting. Negotiation should be carried
out with insurers that can be useful in getting data about the patient.
