Introduction
While treating patients suffering from chronic and terminal disorders, nursing
professionals should implement effective healthcare interventions inclusive of both physical
and mental healthcare interventions (Kearney et al., 2014). To implement such holistic care in
the care process of patients, it is important to assess the priorities of the patient depending
upon their healthcare requirement so that with application of such interventions, their health
improvement could be obtained (Beemster et al., 2019). Further, in such condition,
application of trauma informed care process (TICP) helps the nursing professionals to
provide the patients with empowerment, safety security and sense of awareness so that with
physical and mental awareness patient could be provided with confidence, self-esteemed and
self-determination (Cleary & Hungerford, 2015). In this aspect the case study of Mark would
be analysed so that the TICP principles and healthcare interventions could be provided to the
patient with identification of three life events related to his depression, stigma due to his
mental instability, and trauma due to the death of his father.
Identification of the issues
Upon detailed analysis of the provided case situation, it was observed that due to
multiple physical, emotional and environmental stress factors, mark developed the critical
level of mental and physical healthcare situation, affected his cognitive abilities, physical
strength and coping skills. It was seen that he was associated with games and after primary
success and fame, he developed mental health concerns from the age of 18. However, it was
seen that despite having mental illness and concerns from the age of 18, he was diagnosed
with sever bipolar disorder and depression at the age of 42, that indicates towards the lack of
awareness of the patient regarding the mental illness that lead to prognosis of prolonged
disease condition. As per Kalil et al. (2016), if any disease is not treated for a longer period of

time, it could affect the patient negatively and lead to critical healthcare conditions in future.
The close friend of Mark mentioned that in that situation, there was a lack of awareness
among the communities and society regarding mental illness and hence people were not able
to achieve timely healthcare interventions (World Health Organization, 2015). Similar
situation was observed in the mental healthcare condition of Mark that indicated towards his
inability to understand his mental condition and hence suffered from the adverse mental
healthcare condition for 24 years. Due to the prolonged depression, he was unable to perform
in his game and his performance deteriorated continuously afterwards. As per Kolodny et al.
(2015), patients who are suffering from mental conditions and are unable to detect or seek
intervention immediately are at higher risks for substance abuse that could lead to severe
mental health condition by increasing the urge of self-harm. I the case study of Mark, similar
situation was observed as due to extreme depression he became addicted to cocaine that
deteriorated his mental and physical health. Therefore, as per TICP principles prepared by
SAMHSA (2019), he lacked awareness about his mental illness that made him suffer from a
prolonged depression and bipolar disease.
The second issue that could be identified from the case scenario of Mark was related
to increased stigma about mental health. Corrigan, Druss and Perlick (2014) mentioned that
patients that are affected with mental conditions, are ignorant of this aspect of their health due
to social stigma and despite of understanding the symptoms of the situation, they are unable
to accept the prognosis of the disease and seek for mental healthcare interventions. In the case
situation of Mark it was seen that the initial stage of his mental illness was harsh and he used
to lock himself prior to the warm up session of each game and cry so that he could overcome
his grieves and pan and could perform in the matches. Further, he used to consider his
inability as a result of severe flu due to which he was unable to perform properly (Seligman
& Csikszentmihalyi, 2014). Therefore, his negligent behaviour and inability to accept the

mental disorder was observed in this condition. Further, it was seen that he started consuming
cocaine over the weekends so that he could overcome his inabilities for few time and could
forget all the inability and humility he has faced. Therefore, in this situation, it could be said
that he started his cocaine related addiction so that he could overcome his mental illness and
diverts his thinking from mental illness. Billieux et al. (2015) mentions in the study that
majority of the drug abuse cases are the result of mental health condition, due to which
substance abuse increases. Hence the drug abuse, ignorance of his health condition was the
effect of his increased social stigma due to which, his mental health condition worsened. As
per TICP by SAMHSA (2019), in this condition, the patient lacked the safety and security
related to his mental health condition. As providing him privacy in such condition, could
have provided him with courage so that he could approach mental healthcare professionals
overcoming his social stigma.
The third aspect which would be discussed in this situation would be presence of
mental trauma that he suffered after the death of his father, in between of his mental health
condition. As per Schoenfeld & Cameron (2015), patients of mental health condition are
tremendously vulnerable to environmental and emotional stress and in such situation has the
risk of breaking down. Similar situation was observed in the case scenario of patient after the
death of his father, he was unable to focus on his game and then started consuming cocaine to
overcome his mental trauma. Further, it was also observed that after the death of father, his
mental health condition worsened and he lost his mental strength, self-confidence, self-
esteemed and hence, critical health condition arise (Corrigan, Druss & Perlick, 2014). The
patient was alone fighting with mental illness as after the death his father, he did not had
anyone to share his pain and grief. Therefore, in this situation, as per TICP and SMAHSA
(2019), the patient lacked support of his peers and family members with whom he could have
involved in the constructive and effective communication.
