Question 1:
The patient exhibits common issues associated with aging and along with that her eye
sight seemed to be effected by the aging for which she required glasses and could not
successfully read the health documents and access information. Along with that, she had no
basic health literacy and could not even define her issues using proper medical language
which will require assistance (Manach et al., 2017).
Question 2:
I would provide culturally safe patient education session with evidence data to support
her education and information sharing process.
Question 3:
Healthy aging oriented health promotional programs would be suggested to her to
enhance her health literacy and support her through successful disease management.
Question 4:
The nursing diagnosis for her is activity tolerance, for which she will be given
physiotherapy and medication; self-care deficit, for which she would be given
recommendation for a 24*7 assistive care giver; and lastly, pitting oedema, for which she
would be given compression socks and elevated posture training (Kaeberlein, Rabinovitch &
Martin, 2015).
Question 5:
I would have to take into consideration her cultural and religious values as she has
stated herself to be a devoted churchgoing catholic. For her age and gender, her privacy and
lack of health literacy will be considered.

Question 6:
Although none of the interventions given are exceedingly invasive of privacy and
dignity, additional care to take informed consent and provide female care provider will be
taken to respect her privacy and dignity (Davitt et al., 2016).
Question 7:
Family has a huge role in improving quality of life, especially for aged patients. In
this case her family will be involved in the care planning procedure and would also be
encouraged to participate with Mrs Reynolds in community activities which she used to enjoy
in the past, but has discontinued due to osteoarthritis and other adversities.
Question 8:
I would consult with her and provide her with probable options but provide her with
decision making authority and respect her choices optimally.
Question 9:
Her highest priority health adversity or issue which requires immediate intervention is
her activity intolerance due to pitting oedema and osteoarthritis, which is not just affecting
her mobility, but is also hindering basic self- care activities. Hence, based on this high
priority rating, NSAID pain medication and physiotherapeutic interventions will be given to
her (Manach et al., 2017).
Question 10:
The interdisciplinary members to be included are: a dietician, a fitness expert, a
physiotherapist, a cultural identity and spirituality liaison officer or advisor, head registered
nurse, her doctors, and her care giver.

Community resources that can be provided to her includes community care support,
social care support and aged care inclusion programs, which she can access through her care
advisors and her multidisciplinary team (Davitt et al., 2016).
