INTRODUCTION
Care plan is a process of analysing the individual health, social risk and needs to
determine the support that is needed to meet those objectives and to achieve the potential
outcome. It plans to describe in easy and accessible the person's need and requirement. In this
report it is include the care plan of social worker that include the whole plan of social worker
and it some wide variety of roles and this reflect the roles of social workers in other community.
The other care plan is of physiotherapist that will include all the roles and responsibilities of
them in the context of their patients is being discussed. The other care plan is of support workers
and the way used to work in different health care departments is being analysed. Apart from the
care plan of GP is being discussed and what are there roles and responsibilities is being
determined in this report.
Care plan from the perspective of Social worker
From the perspective of Social worker the Care plan for taking the best care of Morris
and Jenny Brown will be as follows-
Need- Jenny is suffering from the problem of severe Arthritis and thus this is affecting
here a lot. She is also suffering from some other health problems like Alzheimer's Disease. Also
Morris is facing a bipolar problem (Blahous, 2018).
Intervention/Action- The following actions will be required from the point of view of
Social worker-
Ensuring that the Doctors of both Morris and Jenny Brown are providing them with the
best care by making the use of best healthcare facilities.
Ensuring that Nathan manages the finances of the old couple in a right way and provides
them with the necessary financial assistance whenever desired.
Ensuring that the other social needs of the old couple are met out in the right manner
which will help them in meeting their social goals and objectives.
Professionals- The following professionals will be required to complete this particular
intervention/action-
Social Workers- The Social Workers will assess that whether the social needs and
requirements of the the workers are being met out.

Social Volunteers- The Social Volunteers will assess the need as well as requirement of
both Morris and Jenny to find out whether they are able to meet out their social needs and
requirements in a proper way or not.
Timescale- The timescale which is required for the purpose of completion of the project is 30
days.
Outcomes- The monitoring of Jenny and Morris's social needs and requirements will be
done after taking a detailed feedback from them in which they will tell more about their
experience. This will be quite helpful for them in ensuring that they are able to report any kind of
abuse which is being faced by them (Fekadu and et.al., 2016).
Care plan from the perspective of Physiotherapist
From the perspective of Physiotherapist the Care plan for taking the best care of Morris
and Jenny Brown will be as follows-
Need- Jenny has a need for best care to be provided to be her because she is suffering
from the problem of severe Arthritis (Finch, Griffin and Pacala, 2017). Thus she needs that
regular exercises must be provided to her which will help here a lot in coping up in an effective
manner with this problem.
Intervention/Action- The actions which are required from the point of view of
Physiotherapist will be as follows-
Providing regular exercises to Jenny which will be quite helpful for her in ensuring that
the movement of her body takes place which will help her in dealing with her severe
problem of Arthritis.
Jenny will be provided with muscle exercises so that more movement of her muscles can
take place which will help her a lot in dealing with her problem of Arthritis.
Jenny will be provided with regular training which will be helpful for her in dealing with
the problem of Arthritis.

Physiotherapists- The requirement of Physiotherapists will be due to the reason that

Muscle Training Providers- Muscle Training Providers will be required because of the
reason that Jenny needs to be trained in her muscle training. Thus in this way Jenny will
be provided muscle training which will help her a lot by helping her exercise more.
Timescale- The timescale which has been set for the completion of this particular action is 15

of Support Worker will be as follows-
The weekly shopping needs of both Jenny and Morris will be fulfilled by ensuring that
their shopping lists are framed and the necessary items which are required by them are
delivered to them by the Support Worker. This will help in ensuring that the needs and
requirements of both of them which are related with grocery can be fulfilled.
The hospital visits of Jenny and Morris will be arranged by Support Worker by ensuring
that regular appointments to the Doctor are arranged for them.

Support Workers- The Support Workers will be required to look after the needs and

et.al., 2016).
Intervention/Action-
Regular health check-ups of Jenny needs to be carried out (Liew, 2018). These check-
ups should be regarding the problem of Arthritis. Thus this will help Jenny a lot in
ensuring that her health issues related with Arthritis can be managed properly.
Regular medicines are to be given to Jenny which are related with Arthritis and
Alzheimer's Disease. This will help her a lot in ensuring that she can deal with this
memory loss problem. Also Morris should be given proper treatment related with his
Bipolar problem. This can include the different type of counselling sessions which will

provided to them so that they are able to deal in a right manner with these problems (Pouessel
and et.al., 2017). Also the mental health issues which are being faced by Morris can be handled
properly by the Nurse through providing proper counselling to him which will help in dealing
with these issues in the right manner.
Intervention/Action-
To monitor the medicines which are required to be taken by Jenny for her Arthritis
Condition and Alzheimer's Disease. This is needed to make regular medical reports
which will help in regulating the medicines if required.

Reflection
I used to deal with many patients who used to suffer from various problems and situation
and that could be related to mental health or any other factor. Recently I have come across with
the situation of Morris and Jenny Brown they were married couple and they have one son named
Nathan, they were financial dependent on him. Jenny was suffering from the arthritides and other
health issues which become very painful for her to bear this pain. When I contacted last week to
social care and report their concern. It is become very important to deal with the situation for
both couple has their son has stop giving money to them and that makes their situation to worse.
I was really tensed about the situation of them as they are in very poor state and hardly has any
money to survive. As it is quite difficult for me handle the situation because Nathan has shouted
on them so loud and make them so weak from inside and out. During dealing with this situation I
found the importance of financial help and emotional help to patient as this will help them to
fight with in worse situation. I feel very bad for both couple and it is necessary to care about the
their financial needs and requirement if they need in future. Jenny was at her very bad situation
and I was quite tensed about her health issues and I learned the importance of family support and
financial help.

CONCLUSION
From the above report, it can be concluded that Care Plan is required in healthcare
services so that the procedure for providing the best healthcare to the patients who require it can
be initiated. Framing of such a plan ensures that the healthcare professionals are able to deliver
the best services to them. The Care Plans can be prepared from different types of perspectives.
These perspectives are quite important from the point of view of the healthcare workers because
this helps them a lot in providing the right services to the patients who desire these services.
