INTRODUCTION Care coordinator is person that responsible for taking care of various patients so that it can enjoy health and better lifestyles in future circumstances. The report is based on case study of Liam that is 15 year old male patient which have recently joined healthcare homes program. I have been appointed as RN to review patent notes and shared care plan before surgery but the information given in shared care plan are incomplete. Therefore report is to find relevant information that will helps in better treatment of Liam. PART 1 1. Patient care Team There are various facilities, professional Liam and his parents are allowed to access for effective treatments of goals. Liam is a patient that is suffering from JIA (Polyarticular) since age of 8 year. It has been seemed form report that level of pain has been increased in joint and wrist that was fractured in an accident. There should be open communication between care team or use of electronic health records so that patient can be delivered qualitative services. Elements of primary health care is firstly all care taker need to be provided education and training on several health problem that are faced by patients and the way they can be treated or provide facilities for better health. Liam and its family member need to provide more nutrition and better food so that it can gain strength to fight with diseases (Chung, Juang and Li (2019)). At the same time it should provide healthy environment where patient and its family member can live that have safe water supply. Essential medication knowledge needs to be given by professional to patients and its family member so that they can make use of them in treatment of problem faced by Liam. There should be proper treatment of diseases that is availability of technology and medical facilities so that Liam can have healthy lifestyles (Health Management, Ethics and Research, 2019). On the other hand secondary health care is more focused, specialised and usually support individual that are suffering from complex health condition. So, Liam is one such patient that is suffering from sever diseases such as JIA and Asthma which require specialist to treat its problem in better manner. Therefore experienced, highly skilled, qualified and knowledgeable healthcare is required for treatment of Liam. It also requires innovative or more advanced technologies that can used to treat Liam from several injuries or fracture in right hand, asthma and JIA (Verma & Sood (2018)). Specialised monitoring need to be provided to patient in secondary care which helps in understanding reason behind problem or sever condition of patient 3
has been created so that effective solution can be found. Therefore both secondary and primary healthcare facilities, technologies and professionals can contributed in get better outcome or health of Liam (FAQ: What is the difference between primary nursing care and acute nursing care?, 2020). 2 Patient barriers to care goals It can be illustrated from case study that patient care goal of Liam is that he had been irritated to feel different from other due to various health problem so one of the major goal of patient is to be look, feel like other normal individual in society. Another major goal of patient is that it want to play sport at weekend but it could not play due to odd one out so it need to be provided proper care and support by its family. On the other hand Erica goals is that she is very concerned about psychological healthy of Liams so want to find better strategy that can issued problem quickly so that Liam can live better lifestyles (Duggal, Brindle, & Bagenal(2018)). Secondly she want some specialist to get deep information about reason that have caused Liam problem worst form few months such as Asthma and JIA. Thus, it want a better care plan or strategies that can contributed Liam to live health and better lifestyles. Therefore are few barriers in effective implementation and achievements of patient barrier such as parents of Liam that is Michael and Erica are not able to balance their respective responsibilities that have adversely impacted on health and well-being of patients. They are unable to transport Liam to various therapy session and medical appointments such as Paedtrician appointment quarterly, local fortnightly physiotherapy and monthly medical appointment in Sydney that have created most significant barrier in effective achievements of patient care goals. Another point is that his father and mother both does not have enough time to have spend or play with him which created a feeling of being isolated or differentiated from other people. For examples: When every time he visit his father it feel that he is burden on them as Michael does not have enough time because of two main responsibilities that is care of children and farm (Wang, & Hajli (2017)). So, lack of time devoted by her father and mother has lead to create a feeling of isolation among patient. Liam since at age of 8 has diagnosed of Juvenile Idiopathic Arthritis and again it was found that it had asthma at age of 10 year. Since last two years Westtown region was suffering from drought that have lead to dust storm which have made condition of Liam more serious. Recently 3 years ago Liam have fractured its right hand due to fell of his bike so all such circumstances have made situationof Liam worst and badly affected on health and well-being. Nor it was 4

provide environment where it can feel like other normal people is society thus all such situation have created in achievements of patient care goals. Lack of self management tool such as regular consultancy of paediatric rheumatologist and paediatrician ASAP nor Laim was provided independent environment at home and school for medication and trigger identification so that appropriate action can be taken. Therefore it can be stated that lack of all system, medical treatment and care of patient have act an effective barrier in patient care goals. PART 2 1. Nursing practices There are various care coordinator skills that are required by registered nurse to provide better treatment and care to patient so that its problem can be resolved. Four quality needs to be presented in every care coordinator to accomplished its particular task and render best services to customers.It is not an easy job to be nurse so the care coordinators need to be specified knowledge, capabilities and qualities or experienced to manage better health and well-being of individual (Improta & et.al., (2019)). Therefore there need to be few basic qualities present in nurse as care coordinators to avoid consequence ongoing stress. Such as: Empathy:It can be stated that all health care provider needs to be empathetic which means it should be able to understand several problem, feeling that are faced by patients such as Liam. Therefore it is one of the most crucial traits that need to be presented in care coordinator and need to have full detailed about medical history of patient.All such knowledge about patient helps care coordinator to find appropriate action or strategies that can used for better treatment of individual (Farahani & et.al., (2018)). It also helps care coordinator to understand feeling of family member that they face while taking care or delivering high quality services to patients. Thus, empathy skills help care coordinator to remove various obstacle that act as barrier in effective treatments of patient. Communication skills: It is another skill that needs to be presented in care coordinators as merely understanding feeling of patient is not enough which means care taken needs to have betterknowledgeaboutthewayimportantinformationarecommunicatedtopatient. Communication skills is also required to give various instruction or exchange information to caregivers or health care practitioners in order to ensure smooth delivery of care to patients.It should also know that effective listening is one of the great ways of communication so it needs to take feedback from patient and subordinate to know about their respective opinions (Hughes & 5
et.al.,(2016)).Therefore,communicationskillsareoneofthegreateststrengthofcare coordinator that helps it to easily complete its task and get better outcome or results. Resilience:Nurse are the front line car taker in terms of psychologically, emotionally and physically so that there is no secret that nurse need to be resilient while taking care of patient. There are lot of stress that have to be faced by care coordinators while performing its task so they need to have capacity of resistance to avoid stress and lot of tension for delivering better quality of services to patients (O’Brien & Mattison (2016)).In order to manage different types of patient care coordinator need to be more patient or resilience so that harm can caused to its health. Thus, as care coordinator individual must have balance of empathy and big picture thinking that can contribute in getting quality of life and better health outcome. Time management skills:It is another skill that needs to be necessary presented in care coordinator which stated that is should have ability to manage time so that services can be render to patient within limited time frame. The first step that is effective to reduce amount of stress is prioritising activities and task that need to be performed by each individuals.Care coordinators by performing various activities on time is able to get better results and treat patients such as Liam in effective manner (Freeman & et.al.,(2016)). Time management contributed in delivering best services to patient as each task has been performed smoothly and effective. Thus, effective management of time lead to save of life of individual specially in health care sectors so this skills must be present in care coordinator for treatment of patients. 2. Identify strategies In order to effectively implement health care plan there are various strategies that can be used by care coordinators which can lead to be better health and well being of Liam. Such as care coordinators need to organised a program where patient needs to provide opportunities to play with other similar individual as its ages so that it goal to feel like other people can be fulfilled.Healthy and supportive environment that encourages its to play sports can reduce feeling of depression and contribute towards effective achievements of his mother goals. Regular therapy and medical treatment needed to be provided to Liam so that it can become healthy within limited time. New technologies should be used by care coordinator to treat patients as it will contribute in getting better outcomes (Cady & Belew(2017)). Health care program organised by firm will lead in reduction in amount of physical and mental stress level faced by patient. Liam is socially withdrawn that is a major concerned of her mother Erica so need to 6
provide proper meditation for resolving health issues. Another strategy which can be used for better treatment of Liam is used of innovative technology to monitor health condition of patients so that various steps can be taken to control or prevent diseases for effective health care. Specialist professional needs to guide proper medical treatment that needs to be provided to Liam so that it can recover from complex diseases such as asthma and JIA. Cloud centric lot based techniques can be used for treatments of diagnoses such as Asthma and JIA. Proper plan, nutrition and facilities provided to patients can contributed in effective management of diseases. It should be counselled so that it can share its feeling or issue with it so that appropriate strategies can be find for better treatment (Duggal, Brindle & Bagenal, (2018)). Thus, all such health care program, technology and innovation or organisation will lead to improve health of Liam or assist in effective achievement of particular goals. CONCLUSION It can be concluded from above report that there are various elements in primary and secondary health care which can enhance health of person.Liam and its family by providing such healthcare facilities can live better, healthy lifestyles. It can also be stated from above analysis that there are numerous skills which must be present in care coordinators so that it can delivered best services to number of patients.
