INTRODUCTION
The present study is based upon the case study and this inter agency report work mainly
comprise of the multiple factors associated with the Ted and Mary case study and in reference to
the differing cultural needs in communities within UK. This study examines how information
sharing has been perceived as cultural issue and how it has influence on delivery of health and
social care. This study also demonstrates the key agencies associated with dealing of specific
complex case. Furthermore, the present study demonstrates the attributes of multi-disciplinary
team and skills possessed by the leaders. To effectively reflect on the areas which needs to be
developed and evaluate cultural needs of communities.
MAIN BODY
Question 1
Analysing the multiple factors associated with the case study and in reference to the differing
cultural needs in communities within UK, especially when to share information.
As per the case study, Mary is an elderly women who is aged 78 years. Mary’s husband
Ted has been tied to the chair in the fear that in her absence he might walk away, burn the house
down and even hurt himself. The volunteer has also observed that, Mary has lost weight and
states that, Ted is frail and also has difficulty in remembering things. Mary seems to be shaky
and teary when the community centre offered her to provide with the local health services. The
same information in turn has been shared to the manager, Gill who has been working on an aged
care assessment team without even disclosing the identity of Mary. However after the assessment
of the case the Gill has decided to effectively proceed without seeking consent of Mary and Ted.
This is because the Ted is highly vulnerable and is also at the high degree of risk of the serious
harm. However, the Gill has contacted the local health service and social care assessment team
visits Mary and tends to provide support for both Ted and Mary in emotional as well as practical
manner.
As per the (Riordan and et.al., 2015) believe that, UK is a diverse cultural society which
tends to mainly focus on developing the own culturally heritage, beliefs, language and attitudes.
Information sharing is considered to be useful in effectively ensuring that the elderly clients
receives the type of care and help they need in order to live on an independent basis. Information
sharing is considered to be as one of the most significant measure which has to be done within

the health and social care sector in order to gain better care results and outcomes. It is considered
to be important for the health and social care organizations to comply by the law in order to
significantly share information and improves the care of the patient. Sharing of crucial
information with the agency or council health service is to effectively provide the best possible
care for the patient and it is significant in reducing of the pressure on the urgent set of care
services. It helps in providing the care treatment to the party at the earliest in order to live a
healthy and prosperous life. Failure to share the key relevant information in turn results in harm
to the respected party which in turn likely affects the health of the individual person.
Communication is considered to be one of the significant aspect which mainly focuses on
sharing and also transferring the key relevant information among all the cultures which is useful
in identifying and also examining the effective major traits. As per the Coyne and et.al., (2016)
believe that, in this specific case it is considered to be of crucial relevance which is useful in
effectively dealing with the key relevant problems and must also facilitate care for an individual.
Information sharing must be in proper consent with the parties. But, in this specific case
relevance, Ted condition has been continuously deteriorating and he was at the high degree of
risk to harm. It means it is important to take such action to provide them with the best possible
care and outcomes. In case it has been guarded by the law and the rules it is considered to be of
utmost importance and relevance to improve the sharing of the information by effectively
outweighing the individual as well as the public interest. Disclosing and sharing of the
information to the health care organization without the consent of the patient and family
members is done when the patient is at higher risk of getting hurt or cause harm to himself and
his partner.

Information sharing in turn has been significantly influenced by the large number of
factors. It comprises of both inter- professional differences and professional interrelation. The
information has been significantly provided by complying with the health and social act, 2015. It
is of utmost relevance to effectively comply with the aims and the information sharing rules in
order to deliver the best possible care services with high degree of relevance and importance
(Papoutsi and et.al., 2015). The influence of the culture is wide within the health and social
sector. This is mainly because of they belong to the different culture, perspective, belief and

attitude towards living their life. However, the cultural issue within the health and social care
setting will largely influence the perception of the illness, health and death and the beliefs related
with the cause of disease, approach towards health promotion and how the illness and pain has
been expressed or experiences. This is where the patient must seek help and provide them with
the best possible treatment. I have an effective problem solving skills and has the key capability
to take decision with utmost degree of accuracy to understand the needs and want of the patient. I
have the capability to resolve the problems which helps in living a better and healthy life. I focus
on effectively critically analysing the key relevant set of problem and communicate the same
with the care agencies and organizations to take appropriate set of decision. I have the capability
to focus on the team building capability which in turn is considered to be prominent in ensuring
that the team has been working in a significant and reliable manner to improve the results and
outcomes (Kostkova and et.al., 2016). I also take responsibility to improve the health conditions
and provide them with the best possible treatment with greater set of accuracy and relevance. As
per the Gibbs reflective model, I believe that, culture is considered to be as a pattern of idea,
behaviour and customs which has been shared by the particular individuals or society. I have
found out that, both patients as well as the health professionals are largely influenced by the
respective culture or any cultural issues. As a practitioner, I am highly culturally competent.
That usually means that, I have cultural awareness, cultural knowledge, cultural skills and
cultural encounter. This is significant in promoting high degree of trust and also focuses on
improving the treatment adherence. Information sharing is relevant in creation of the new
awareness and carry out better treatment planning (Mansfield-Devine, 2017).
In this relevant case study, Mary was not willing to talk about her problems with the
volunteers because she has a fear that they might put them into a nursing home. Mary has a
belief that she can manage but after assessing by the team it has been found out that the Ted is at
higher risk because of his vulnerable condition. Cultural set of issues has significant degree of
negative influence upon the health and outcomes of the individual. Complying with the best
possible treatment adherence, effective health care interventions and effective set of
recommendation associated with the lifestyle changes is useful in providing the best possible

Inter- agency working is more than one agency who has been working together in a
formal and planned manner which is considered to be significant in developing and also
supporting the person who needs care. This can be done at both operational and strategic level.
The main responsibility of the inter-agency working is to effectively share responsibilities across
all the agencies. They focus on successfully working together to ensure that the needs of the
individual has been effectively met. It is significant in reducing the risk of harm and creates an
environment where inter- agency working can be significantly flourished (Dheensa, Fenwick and
Lucassen, 2017). The government of the UK has significantly increased the inter-agency
collaboration in relation to the service associated with the health and social care. It is significant
in providing the best possible care and also effective care results. Inter- agency working is highly
relevant in the duplication and delay in the health and social care services. Aged care assessment
team must effectively focus on proving assessment and help to the elderly people. It is
considered to be very useful in providing them with the best possible support which is useful in
best meeting their care needs. As per the specific case scenario, dementia care package will be
given which is useful in providing Ted and Mary by meeting their emotional as well as the
practical needs (Ølnes, Ubacht and Janssen, 2017). This way it is useful in helping the elderly
people with the greater set of accuracy and efficiency.
Social care staff is considered to be one of the most effective team which helps in providing
the significant degree of care to the Ted and Mary in order to improve their health conditions
within specific situation. Such health and social care staff helps in providing care because of his
vulnerable actions and thought process like run away from home, hurting himself and causing
fire to property. The staff members are considered to be prominent because it helps in helping
people who are facing relevant degree of challenge associated with the old age. They focus on
providing both emotional and practical level of support to Mary and Ted. Health care
organization plays one of the key relevant role to improve the accessibility and quality of the
health care. They focus on providing the best possible treatment and also focuses on significantly
promoting the health services in case of Mary and Ted. Indulging Ted in the cognitive

key members who are part of multi-disciplinary team within the specific situation of Ted mainly

secretaries, occupational therapists, counsellors and care workers. They all work together as a
team to deliver comprehensive care to the patient. The key attributes which is linked with the
multi-disciplinary team comprises of trust and respect within the team members. The multi-
disciplinary team must also significantly focus on agreed clinical governance structure and must
always make the bets use of the skill mix within multi-disciplinary team . They also comply with
the agreed set of protocols and systems for the better communication and effective set of
interaction between the team members. This team is highly relevant in improving the health
outcomes and focuses on promoting collaboration working. They also focus on effectively
