QUESTIONNAIRECase management is the health care organization that provides health services to theservice users. For conducting survey on the Case management we have prepared questionnaireand has taken sample size of 4. 1) Do you think that case management is providing quality services to the clients?YesNo31When the questions were asked by the clients that what is your opinion about the servicesof the Case management then three users have said that they are satisfied with the services. Theysaid that whenever they go to the Case management for getting the treatment medicalprofessionals treat them well and behave with them politely. Whereas, one respondent hasviewed that services provided by the cited firm are very poor, as they are not satisfied with theservices of health care organization. Whenever they need immediate treatment practitioners donot examine them and they have to wait for longer period for getting the services. 2) Does working practices of case management are appropriate in identifying the needs ofservice users?Yes2No2When researcher has asked about the working practices of the Case management andwhether cited firm is proving you service as per their needs. Then two respondents have said thatyes medical professionals treat them well and ask first about the daily routine and actual healthproblems. According to the issue their prescribe them medicines and treatment. But two clientshave said that no Case management is not paying attention on the needs of patients.
