Should your vocational workplace have their own safe sleep checklist
or other relevant documentation that safe sleep check has been
completed, submit three copies to document three instances that you
have performed and completed a safe sleep check in the workplace.
Provide the details of the safe sleep check you’ve completed in the
space provided below, and have your workplace supervisor sign the
declaration form to confirm that you have performed and completed
the described safe sleep checks:
Guidance: the safe sleep checks described must be done for the
following children:
- one baby 0 to 12 months old
- one toddler 1+ to 3 years old
- one child 3+ to 5 years old

Once you have performed and completed the safe sleep check for the
three children, submit the completed checklists, complete the
information required above, and have your vocational workplace
supervisor sign the supervisor declaration below:
