As per the assessment data provided by the case study, Abe has
GFR 20 ml/min/1.73m2, which indicates he is at stage 4 of CKD.Rationale: As per standard guidelines, Stage 4 Severe CKD is indicated by
GFR = 15-29 mL/min, and the level of Abe’s GFR count falls in this range,
he can be diagnosed with stage CKD.

Glomerular filtration rate provides a clear and concise method of
determining the stage of chronic kidney disease or CKD, hence GFR test
would be the best indicator (e Silva, Costalonga, Coelho, Caires &
Burdmann, 2018).
Task 2:

Health promotion and education has proved to be one of the most
effecti8ve strategies to combat and manage a progressing disease in the
last couple decades (Sud, Tangri, Pintilie, Levey & Naimark, 2016). Stage
4 CKD being an end stage renal disease, along with medication, proper
disease management and health promotional education can enhance the
quality of life drastically. Based on the assessment findings, the most
pressing issue for Abe is his progressing CKD at stage 4. As per the
leading research findings, more than 20% of the stage 4 CKD progresses
rapidly to stage 5, which is the last and fatal stage. First and foremost, the
patient should be given direct instruction and detailed education
regarding fluid and salt consumption. Due to progressively reducing
nephrological functionality in the body of the patient, it is imperative to
limit the water and salt intake to slow the progression down (Husain et al.,
2018).
Dietary management is also a very important aspect of the same,
taking food items having low sodium and potassium percentage has been
proved to be highly effective in stalling the progression to some extent.
Along with that proper medication management, with proper dosage and
