INTRODUCTION
Lip augmentation is defined as a clinical procedure that can provide plumper and fuller
lips (Laurino and et. al., 2018). In recent times, injectable dermal fillers are the most common
methodology which are being used by individuals to improvise their appearance (Lip
Augmentation, 2021). The advancement in surgeries introduced varieties of dermal fillers, in
which most common fillers which are being used today consist chemical called Hyaluronic acid.
Studies depicts that hyaluronic acid is a natural substance, present in the body (The Impact of
Dermal Fillers on Perceived Personality Traits and Attractiveness, 2021) It is beneficial to
expand volume of lips (Chi, 2020). This report includes discussion of self-determined experience
in of a patient's consultation, delivery of treatment and also follow ups.
Task 1
Patient record
Miss X visited to the hospice in concern of dermal filler lips. She was excited to take into
consideration this major treatment as it was totally a new experience. I immediately recorded
patient's name and DOB, also her personal details such as her address, telephone number gender,
her skin type before starting consultation process. According to me, it is important for a person to
maintain systematic record and details of a patient for future records.
Consent Form
I have gave her consent form. The consent form is document which is to provide written
information in regards with associated risk, benefits as well as options of the procedures of
surgery. This material served as a record of discussion at the time consultation process
(Tamiolakis and et. al., 2018).
Initial Enquiry
I explained her a better understanding of surgery and core knowledge of procedures of
dermal filler lip surgery. I told her there are several types of dermal surgeries that can injected
into individual's lips. In which most common fillers are HA and collagen (Lip Augmentation,
2021). They both increase volume of lips (Singh, 2020). I asked her if she was pregnant or
breastfeeding as these surgeries are not recommended in this case. I also listened her desired look
from surgery and discussed what we can do, in order to fulfil patient's demand.

Consultation
At the time of consultation, we discussed about best options for treatment which can be
predicted to have best results for her. I explained her that it is soft tissue filler injected into the
skin on variety of depth along with particular angles. Also, we further discussed about the
procedure which was going to take to even out her both lips. I told her that fillers are temporary
and fillers are used to volume the area and volume for last up-to 612 months (The Impact of

fillers are to get absorbed into the skin. Whereas, lasting period also depends on brand, quality
and density of chemical which are used to make filler.
Imagery Pre and Post Treatment
The considered imagery techniques which are used in the case of Miss X to hypnotherapy
at first, in order to ease her anxiousness. Whereas, the post treatment which is considered as use
of HA fillers. These are considered natural fillers which can be used in the individual's case. It
can provide metabolism to skin naturally (Lip Augmentation, 2021). Longevity majorly depends
on the area of filler which are used. HA fillers are temporary (Sykes and et. al., 2018).
Perceived Final Outcome
The carefulness in the selection of individual, observing medical history and a detailed
consultation outlining the advantages, disadvantages, limitations and adverse are the key
component of perceived final outcomes. Discussion of aftermath in regards with surgery which is
swelling as well as bruising, is also termed to be key component to counselling.
Initial Treatment Plan with Ongoing Amendments
Initial treatment plan determined that can be considered by analysation of medical history
of individual. It is important for the medical institute to consider medical history of an individual

knowledge of risk and benefits of surgery. The initial plan consist the use of local anaesthesia
and injecting hyaluronic fillers to volume the lips.
Test Results
The test result which recorded of Miss X are termed to normal. Her skin type is soft and
delicate in nature. The psychological test reports reviewed normal on scale that led to patient's
eligibility of cosmetic surgery. As per my observation, I have analysed that sensitive skin type
individual may undergo to experience risk factors. Although these risk factors can be managed

by multiple acute care plans and person-to-person care. I observed that her medical history was
fit for the dermal lift surgery.
Contemporaneous Notes
As per my self observed notes, Miss X seems pretty much comfortable while interacting
and as she fully understand the pros and cons of the dermal lip surgery. I told her the fillers
which we are going to use are made up of natural components and have ability to get absorbed in
the skin which made her to ease her anxiousness which she was feeling since she entered room.
Issues Elucidated at Consultation
There were no issues which were reported by Miss X. I had effective conversation with
an individual. We had discussion which option is best and benefits associated with procdeure.
Also she seems to be collaborating with me.
Preferred Treatment
The preferred treatment that are being used in this case are use of HA (hyaluronic fillers)
fillers (Lip Augmentation, 2021). It can provide natural effects to the skin of patients. The post
surgery effects of HA fillers are easy to mange (The Impact of Dermal Fillers on Perceived
Personality Traits and Attractiveness, 2021). It contains a local anaesthetic properties that can
numb area within seconds that allow maximum amount of numbness to an individual (Wollina,
and Goldman, 2020). Also it can help in minimising discomfort topical which can be applicable

Traits and Attractiveness, 2021). Addition to this, HA is natural component of the skin which is
naturally metabolised skin. Studies concluded that it can provide shape, structure and volume to

The benefits which are associated with HA fillers are it control over lip volume, gradual pace of
treatment, bumps and have high dissolving properties, leads to less bruising, less allergic than
others (Lip Augmentation, 2021).
The Outcomes Plus Any Post Treatment Advice
The post treatment advices that can be suggested to Miss X that she should drink plenty
of water in order to keep her lips hydrated (Skrzypek and Mlosek, 2021). It can enhance the HA
fillers affects and increase their longevity. I also asked Miss X to avoid lipsticks, gloss and lip

balms for several days. These can promote lip infections after the surgery that is also associated
with further risk factors such as leaking of skin and raptures new cells of lip region.
Side Effects
The considered side effects of facial surgery which are considered to be bruising,
swelling and sometimes bleeding are common side effects of dermal filler surgery. Moreover, it
is also associated with side effects such as itching, redness, tenderness as well as allergic
chemical reaction reactivation of cold sores(The Impact of Dermal Fillers on Perceived

It can also prevailed blisters lumps under the skin, which studies considered as
concerning side effects of dermal filler surgery (The Impact of Dermal Fillers on Perceived
Personality Traits and Attractiveness, 2021). Hence, this side effect is not common that only
occurs in one in two patients. Addition to this, another harmful adverse effect is vascular
occlusion blanching of the individual's skin and sometimes it can also produce white patches on

Reflective account
Miss X was 28 years old, female and her mental health was good and she was
completely eligible for cosmetic surgery. Administration of mental health is required, before
starting any cosmetic surgery on patients (Lip Augmentation, 2021). I observed that texture of
her skin is soft and in order to achieve natural look for her lips, we decided to use HA filler on
upper layer of lip, to even both two layers of lips. It could give her nice look and desired look. I
analysed that she seems to be optimistic, meanwhile little bit anxious as it was her first cosmetic
surgery. I counselled her and eased her nervousness by communicating with her while
consultation. I told her importance of drinking water after getting dermal fillers surgery done. It
has been observed that water can help individuals to hydrate skin and it can enhance the effects
of hyaluronic acid fillers (The Impact of Dermal Fillers on Perceived Personality Traits and
Attractiveness, 2021).
As per the discussion with patient, the considered treatment was application of HA fillers
and its desired outcome to provide volume by pumping upper lip of Miss X, as it can help to
even both part of lips. Before we started surgery, I asked her to remove lipstick and make-up.

Cosmetic product can promote infection to individual's face. We asked her to apply numbing
cream on her lips to numb the surgical area. As a numbing reagent, the most preferred drug is
lidocaine 4, which was used in the case of Miss X. It can also work as a local anaesthesia for
patients (Lip Augmentation, 2021). Numbing of area allows individual to suffer less pain while
undergoing in surgery. It was applied for 10 minutes prior to injecting as it works by stopping
nerves from sending pain signals to brain of an individual which decrease the pain of individual
when we inject fillers into lips.
Also, before I started injecting, I always make sure my hands are neat and clean. I always
wear surgical gloves and apron before starting any surgery. There were the essential part of
standard practice, which could also help to avoid cross infection. I also ensure that if my patient
was aware about the treatment and its associated risk factors which I was going to proceed and
reassuring the whole procedure. I kept confirming with Miss X, if she was feeling okay and
comfortable before injecting. In order to be on safer side, I always prefer to use safe technique
which was aspiration. It provided me, assurance when I inject fillers, that if I was not injecting
into the blood vessels of individual. Along with this, it is beneficial to prevent vascular occlusion
which is common risk factor of dermal lip surgery. The vascular occlusion results to diminishing
of blood supply. I also checked the depth of needles while injecting fillers into the lips of Miss
X. The tissues at higher depth of lips have sensitive tissues, it requires constant observation.
Moreover, I always make sure angles and depth of needles should be correct before I release the
filler into lips. After injecting in separate regions, I asked Miss X to sit straight, in order to check
the volume and balance the filler out so both sides can have equal volume.
I gave Miss X to have a look in mirror to see ending results of surgery and to make sure
whether she was happy with results or not. She was happy and satisfied with results. By the end
of surgery, there were no circumstances which we experienced in the case of Miss X. Moreover,
the procedures went successfully well. Further, I demonstrated her, possible side effects such as
redness and temporary bruises which can arise, and that were common. I asked her to take
healthy diet and drink plenty of water. It can help her to enhance lips and effects of fillers. I
asked her to avoid lipstick for short time period. The tendencies of HA fillers are to absorb into
the skin which can to infections which causes leakages, damage of blood vessels etc. At last I
advised her, about to take care of her lips as the product takes two to three weeks to settle down
into the lips.

I have achieved understanding that the quality of care is provided by surgeons, which
involves dignity and respect towards patient. In which consultation skills plays a effective role. I
have analysed that it is helpful in establishing trust with individuals and can form positive
relation within patients and surgeons. The effective consultation skills allows establish
transparency. This transparency can help in determining individual's desire from surgeries. It can
help to satisfy needs and to attain positive outcomes for service users. Consultation skills also
involves observing skills and communication skills of practitioner. It allows to identify the state
of mind of patients, whether they are comfortable or uneasy through their body language while
making interaction with them. In my opinion, consultation skills also help to ease individual's
anxieties and distress by making effective conservation with individuals. Surgeons should be
broad minded, in some cases collaboration of good mentality and good communication skills
might work in administration of an individual and can accomplish the task of easing their
nervousness and anxieties in regards with any cosmetic surgery. Another, area which I have
explored is Professionalism. I analysed that the professional behaviour is directly linked to the
safety and security of an individual. I felt most successful as a learner when I started being
professional toward my work and associated ethical principles. Professionalism in medical filed
plays an essential role. It can promote productivity and systematic in work which can help to
attain effective results. It is also an essential part of standard practice of health care
professionals. As a surgeon, I always make sure that I am wearing gloves and apron while
assisting patients.
CONCLUSION
Overall it has been concluded that Dermal Lip Surgeries are safe for individuals. Most
common fillers which are used to provide volume, shape and structure to lips are hyaluronic acid
fillers and collagen. These are made up of components which are present in human skins. These
fillers are also skin friendly and get easily absorbed by the skin. In the case of Miss X, the HA
fillers are used to achieve a positive outcome. These surgeries are also associated with side
effects in which some are common adverse effects such as tenderness, swelling, bruises,
whereas, blisters lumps are concerning side effects which are termed as negative impact of
surgery.
