Introduction: As per the AHRQ national health disparity report
2013, the rate of harm with respect to the stays at
the hospitals is around 25.2 % within 100 admissions.
The major contributors to these hospital acquired
conditions include the adverse drug events, medical
errors, or sudden accidental patient falls (Hopkin,
2018).
It has been observed that these kind of conditions
arises due to the lack of communication while facing
any kind of problem.
The organizational leaders are required to pose as
system thinkers and analyze the workplace to check
in for the safety concerns and address any risks with
punitive reactions.
This in turn will help building a safer healthcare
organization (Ginter, Duncan & Swayne, 2018) and
prevent the occurrence of any kind of healthcare
hazard.
