For a service to be documented successfully on the same day, some key elements have to
exist. This will make the service be more sufficient and effective, it all starts from the quality of
the service delivery to the desired outcome. Time would be a very important element so that the
documentation can be done over the same day. This is explained by the fact that documentation
is timely, and rapid documentation leads to a successful service rendered in such cases where
time is of essence.
Client participation is also an element that cannot be assumed as it aids in the whole
process especially with the immediate feedback. During clients’ participation the clinician should
ensure that he or she position the desk properly and the seat which the client will be sitting on,
this will help maintain eye-contact which is an evidence that the client is concentrating and
hence the service is sufficient (Asan et al.,2014). The other factor would be the environmental
setting which should be conducive for both clinician and the client.
Collaborative Documentation standards is a procedure in which clinicians and their
clients cooperate in documentation of a current and reviewed valuations, fresh facilities plans,
assessment and advancement of notes-office created in the society with people and special
groups (Schmelter, 2013).
The community mental wellness centers have the role of providing support, education
and creating awareness of mental illness in the community. This is to eradicate stigmatization of

a challenge for providers if immediate creation of documents is needed. The other limitation is

to handle documentation.
The advantages of utilizing this method in a mental health Centre in any community
manifest itself in many ways. The Clinical Engagement provides an opportunity for the clients in
the community to feel more involved in the process of treatment. Most clients in a community
are made aware of their treatment plan and the outcomes as most clients do not know their
specific treatment plan in relation to the goals set owing to the lack of communication from the
clinicians on the progress issues (Stanhope, 2013). There is also increase in the clinical capacity
as this capacity is able to serve the increasing needs of the people in the community and
supporting access of the services by clients.
Safety of care has improved tremendously and this is evidenced by the outcome of the
collaborative documentation process. Problems relate to health issues such as skin illness and the
infections acquired during stay in hospital have been taken care of effectively giving a positive
outcome of the care programme measures. Effectiveness of care following quality practice care
and achieved outcomes. Timeliness of care outcome measures determine patient access to care
especially when people are many in an emergency department.
