Identification of Focus for Community Teaching
• The chosen focused community are diabetic people who
are prone to many metabolic disorders
• This focused community forms the most significant
community in the USA
• It Can be controlled with the help of awareness and
changes in lifestyle
• The awareness through the teaching plan should be raised
to persuade people for living healthy life

Epidemiological Rationale for Topic
• This focused community has been selected as more than
100 million people in the USA are diabetic people which is
about 9.4% of the population
• 30.3 million people have diabetes in 2015 in the USA
• 84.1 million people have three diabetes conditions that can
lead to the type 2 state if it is no treated in 5 years

Nursing Diagnosis
Diabetes could be diagnosed through
a simple blood test and caused when
the pancreas fails to generate
sufficient insulin
Type 1 and Type 2 are two types of
diabetes
Type 1 diabetes is an autoimmune
disorder
In type 1 the body attacks pancreases,
which do not allows to produce insulin

Readiness for Learning
• Very few people are diagnosed with
diabetes and take action to cure it
• The community does not identify
diabetes and health problem and
continues to ignore the concerns
• The differences between the
community prevents community
programs to offer uniform
understanding with awareness

Learning Theory to Be Utilized
• Social learning theory is a suitable
learning theory for the health
promotion programs
• This theory is ideal for both children
and adults
• This theory reflects that all learning
arises from experiences and
assimilation

Goal
• The goal is to reduce the people who have diabetes and
also reduce the deaths caused by diabetes
• The awareness program with a focus to make aware and
help the diabetic people
• The goal will be to control the blood glucose level via
dietary along with life changes and medication

HP2020 Objective Relate to Alma Ata’s Health for All
Global Initiatives
• Alma Ata’s Health for All Global Initiatives has been
reinstated by the WHO
• Its initiative for diabetes is well aligned with Alma Ata’s
programs
• It aims to assess the impact of diabetes on national and
community health and use the information to educate
people about the severances of the disease
