INTRODUCTION
Referral in healthcare refers to the transfer of care of patient for their better cause and to
provide better and effective healthcare service which can enable patient to get better health
improvement. This can also relate with transfer of care of patient from the hospital to the nursing
staff. Here, according to scenario, Mr. Fred Brown has been referred to the nursing staff to get
better and appropriate care of their health which should be delivered for better health
improvement. Here, it is essential to ensure about the delivery of appropriate healthcare services
which are associated with improving patient health through appropriate nursing services. In this,
there is discussion about the mindful and therapeutic communication strategy which can be
effective to delivery better care and support to Mr. Brown at their home. There is also discussion
about the inter-professional capabilities which includes the optimise care through using the
nursing practices. It also includes the inter-professional and intra-professional collaboration to
provide better healthcare services to improve patient health (Hammond et. al., (2021)).

According to the case scenario, it is essential to ensure that there are some health issues
create difficulty for Mr. Brown where it is essential to get proper healthcare services which are
associated with taking care of effective response for better and improved health. Here, Mr.
Brown has diagnosed through the 6cm stage III anterior tibial ulcer right leg with PAD right
lower extremity along with absence of claudication. Mr. Brown has discharged from the Charles
Sturt Base Hospital. There are also some healthcare issues which are associated with pain in right
leg which is similar to the left leg which has been treated previous through using the surgery
(Boehnke et. al., (2021)).
Here, Mr. Brown need for the wound management which need to take care about the
effective response towards the delivery of better and effective response to manage the healthcare
issues which should be taken care of better improvement in the wound to get filled. In this, there
is need to get different services which includes physiotherapy, nursing management, hospital for
further treatment. Here, different agency should work collaboratively which includes the inter-
professional and intra-professional service delivery which can be effective for better

improvement in health (Klein et. al., (2022)). In this, there is need for better communication that
can be effective and allow to get better interaction for better delivery of healthcare professionals
which are associated with taking care of effective response. Here, as a nursing staff, there is need
to use the effective communication strategy which should be effective to provide better response
towards the delivery of better healthcare services and ensure about the proper care and treatment
of Mr. Brown. Here, nursing staff should be mindful which are associated with taking care of
patient wound management where communication should play an effective role in taking care of
patient for their better health improvement (Rajbanshi et. al., (2021)).
Here, as a nursing staff, there is need to take care about the good health through using
mindful and therapeutic communication. It can allow nursing staff to establish better
communication which can be helpful in getting better care related with delivery of effective
response. Here, nursing staff should ensure about sharing all the information which are highly
needed in taking care of wound and helpful in healing of it with better effect. Nursing staff
should share the information about the activities which should be performed or not and which
type of activity should be avoided that can have negative impact on the Mr. Brown wound. Here,
nursing staff should provide instruction to Mrs. Brown about takin care of Mr. Brown health and
should not perform activities that are not effective that can allow to get better care about the
effective response on delivery of proper healthcare services (Connolly et. al., (2021)). Through
using the communication strategy which are associated with delivery of better care environment
for Mr. Brown. Here, Mr. Brown health should be taken care about the good health which are
associated with other healthcare needs. Here, nursing staff should also inform Mr. Brown about
reduce the other physical activity which can resist the wound healing.
Discussing inter-professional capabilities for optimum care
There is need for better and effective response towards taking care of Mr. Brown in
which it is essential to ensure the collaboration of using different healthcare specialist who
should work collaboratively in dealing with case of Mr. Brown (Chua et. al., (2022)). Here, it is
needed to take care of Mr. Brown healthcare needs where different healthcare agencies and
healthcare professional should collaboratively to deliver better and effective response towards
the delivery of better care. Here, inter-professional capabilities can be effective which can be
helpful in getting involve in better healthcare service delivery. There is also need to use the
healthcare service which are needed by the Mr. Brown to improve their health. Here, it is

essential to ensure about getting healthcare support from the nursing staff for the wound
management, doctors for the proper health check-up and medication. Physiotherapist who are
essential to take care about the physical health. These are highly effective that can allow to get
better learning through using of collaboration which enable healthcare professional to deliver
better and effective response towards the service delivery (Merati et. al., (2021)).
Here, through using the inter-professional skill which is effective method to work
collaboratively with the healthcare professional which are effective and allow different specialist
to work on single case which can allow to get better health improvement in patient health. This
can have related with taking care of effective healthcare services which are associated with
delivery of better care support in response to get effective health improvement. Here, it is
essential to get better delivery of healthcare service through using the collaboration which can
help in getting delivery of particular services that can allow to work together for the better
decision making for the health issues of Mr. Brown. Here, nursing staff member should take care
of collaboration to ensure about proper management of healthcare services which are associated
with taking care of better service delivery. Through using the effective response of
collaboratively work that include different healthcare professional as the specialist to use their
speciality in providing better health improvement of patient (Patel et. al., (2021)).
Intra-professional working can be also useful in the healthcare service delivery where
healthcare professional team with the same type of professional get involved in making better
decision for the patient better health improvement. Intra-professional collaboration can be
helpful in getting better and effective response towards the delivery of better care and support in
improving Mr. Brown health. It also helps in preventing the medication errors, deliver better
patient outcomes and help in improving better experience. It also helps in reduce the healthcare
cost which can help Mr. Brown to get better healthcare services with less cost. Here, patient may
get better healthcare services which are associated with taking care of healthcare service delivery
that can help in getting better improvement to patient health (Preis et. al., (2021)).

From the above discussion, it can have concluded that there is need to ensure about the role
of nursing staff in managing better healthcare service delivery. There is also need to proper

nursing staff should use the appropriate communication strategy which can be effective to

provide better sharing of information that can be helpful in wound management for the patient.
There is also discussion about the effective response towards the healthcare service which are

physiotherapist, nursing staff to work effectively in dealing with better healthcare services for
