INTRODUCTION
Community assessment can be explained as the process where it is needed to determine
the requirements of the public within the specific area or community. It is necessary to provide
all of the requirements as it will directly allow to uplift the living standards of a public. In any of
the community, where there is the requirement of uplifting the standard, it is necessary to
identify all of the issues which they are facing so that suitable decision can be taken for obtaining
the positive result (Alzarea, 2016). To collect any of the information, it will be necessary to take
the help of different resources of collecting the information i.e., Primary source & the secondary
sources. Both of these sources can easily collect all of the information through which data and
goals can be achieved.
In the context of the assignment, the chosen brought is Cardiff which is the capital city of
Wales. In this respective city there are different communities of people are living and among
them the selected community profile is plasnewydd. Different topics will be discussed in the
essay. Firstly, there will be the identification of the health needs of the community with its
critical analysis. Based on health needs, it will be necessary to determine two key issues where it
will be discussed using a social determinants of a health model. Once, key issues will be
identified, it will be necessary to focus on the potential impact of Brexit or climate change. In the
last question, there will be the identification that how potential impact can be reduced in the best
possible manner.
MAIN BODY

data in the context of what this means for health.
Demographic health needs can be explained as the process where different community
faces different types of problem-related to health. It is also termed as the study of the public at
large based on their growth, density, distribution, medical issues, and many more. It has been
found that demographers usually face some of the problems at the time of identifying crises
(Barakat and Ucheonye, 2016).
In any of the health care, there is the requirement of different types of health care
facilities as it will directly allow them to identify the problem through which improvement can
be brought within the community. In the present scenario, there are different community which

has been performing with the help of the health committee to improve the overall health
condition. The main purpose of the community is to identify the problem by the process of
continuous monitoring because it will help to fix out the problem of some of the critical health
issues. In the context of Wales, there are different cities where different community stays among
whom some of the community has been facing the problem which is not allowing to accomplish
the goals. Below, there is a discussion about some of the health problem which is being faced by
plasnewydd who is living within the part of Cardiff (Campbell, et. al., 2019).

Firstly, the people who are living within Cardiff are facing the problem related to
Tobacco, Mental Health, Immunization. All of this problem is increasing regularly because it has
been found that people are consuming a higher percentage of tobacco. Due to the higher
percentage of workload, they are unable to take some of the important decision which creates
legal issues form them (Lee, Park and Lee, 2019). Also, other main diseases that are being found
within the community of Plasnewydd are related to diabetes, overweight and obesity, Substance
abuse, and many more. The above-mentioned diagram shows that although people are healthy
but still there are a number of them who have been facing the issue related to their health. It is
necessary to focus on their health level else it might create issues for the public at large.
All of these diseases directly or indirectly create issues for the person because they are
unable to perform the task which they must perform within a specific period. Although, the
