INTRODUCTION
Community profile is a way of collecting as well as organising information in order to
understand health and health relation issues that defined in the specific community. In this
regards the information is gather needs to provide the accurate aspect of picture of what is occurs
within the specified community. Along with this, community assessment is also consider as a
process that is effective in evaluating as well as determining all the specific needs and challenges
that specified community faced (Alzarea, 2016). Mainly, it also define the skills, resources and
many more aspect that directly contributed by local community of institutions. For developing
the same, it required identification of issues and attitude that includes in the community and at
the same time also assess the both social and economic conditions. Current report is going to
discussed about Merthyr Tydfil that is a specific country brought and particular community is
also determine as a Treharris which is a small town and community in the Taff Bargoed Valle in
the south of Merthyr Tydfil. This project is cover various aspects that is related with
identification of demographic health needs of community. Along with this 2 key issues and
interest by evaluating the social determinants of health model. Furthermore, potential impact of
climate and Brexit on health condition towards the specific community is also mention in this
project. At last, various effective ways are also determined that help in reducing the impact of
Brexit change within the specified community.

a) Identification of the demographic health needs and the community and critical analysis of the
data in context of what this means for health
There are various demographic healthy needs that is refers to the heath of community
issues through comprehensive, systematic analysis. Mainly, demographic is refers to the study of
human population on the basis of their growth, density, distribution and statistics about the
marriage, disease and death. In addition of this, demographers often suffered from identity crises
at the time of trying to locate themselves within the particular discipline. Needs in health care is
normally define the capacity to benefits that evaluated them an effective intervention that should
be available to meet these needs and improve health as well (Rosa and et. al., 2016).
Furthermore, committee proposes a community that is related with the improvement process as it
is related for accountable community in evaluating as well as monitoring the overall health

matters and also at the same time also addressing specified health issues. In context of
community of Treharris, there are certain health issues which faced by specific community:
Diabetes: It is one of the major health issues that faced by Treharris people as around
198333 people who leaves in Merthyr Tydfil. As it is approx 7.6% of the population in Wales
aged 17 and over. Mainly, it is highest prevalence in the UK. This is the major health concern for
Treharris people that put negative impact over their health. Thus, it is crucial for health care
services to determine as well as evaluate at risk group and at the same time also provide them
evidence based support that support to prevent progression to diabetes (Smee And et. al., 2016).
In addition of this, it also needs to be reciprocated by people through taking personal
responsibility towards their lifestyle. For preventing this, there is some health needs like
encourages healthy environment with the help of promotion of healthy and sustainable pre
school scheme. Promote healthy lifestyle choices through the public awareness campaign.
Dementia: This is also refers to the biggest healthy issues for Treharris as people who are
living along with as well as caring for people who effected through dementia. Due to this, large
number of get affected. This may create issues and negative over the people and their health as
well. In context of this, there are certain health needs to prevent the issues that help in
overcoming from the issue of dementia. As there are various older people who are largely
affected because of their short term memories. For overcoming this, everyone is responsible to
understand the person and also includes the personal information that is crucial for caregivers to
understand and use when they are working with the resident. Along with this, people who are
suffering from dementia be prepared, know how to interact, the right time and many more.
Furthermore, social rehabilitate person also find out the area in which dementia patients and
psychological support is required. In addition of this, it also consider various themes such as
emotional support, meaningful activities, dignified attitude and many more. All these are
required by people who leaves in Treharris to maintain their health issues.
Mental health: In Merthyr Tydfil, large number of Treharris community people have
mental health issues that put negative impact over their life style and day to day activities. In
this, people who are feel stress due to mental health (Ogola and et. al., 2015). This type if issues
are increasing as per regular basis as it has been evaluated that in this specific area various
people are consuming higher rate of tobacco that create issues in increasing the possibilities of

human body that put negative impact over the community and people as well.

Health of people starts with the homes, workplace, communities and many more. In this
taking care of ourselves by eating good and staying active, not smoking and getting the
recommended immunization. In addition of this, screening tests, seeing a doctor when individual
sick as all these are directly influence health of Treharris. Mainly, our health is also evaluated in
the part that access to social and economic opportunities by improving the quality of out
schooling, keep safety of workplace, cleanliness of our water, air, food. Healthy people 2020
determine the importance of identifying the social determinants of health by considering creating
social and physical environment that directly promote good health for all the individual of
Treharris community (Bauld and et. al., 2018). One of the main effectiveness of social
determinants of health area within the healthy people 2020 that is framed with the aim of
creating as well as developing social and physical environment. As all the people deserve an
equal opportunities to make the effective choice that directly lead to good health. Furthermore, it
is crucial to needed not only in health care but at the same in the field such as eduction,
childcare, housing, business, law, media, planning about community, transportation and many
more. Mainly, it required better understanding about interest of other area in order to evaluate the
arising issues and attitude of Treharris community. As it provide solid understanding about the
determination of issues and attitude as well. In this regards, there are 2 key issues of area of
interest by using a social determinants of healthy model:
Diabetes: It is related as a major problem in Treharris community people as it arise due
to lack of insulin within the human body. Mainly, it occur when blood glucose and blood sugar is
too high. As it is main source of energy and also comes from the food that people eat. In this
regards, insulin is a hormone that is made by the pancreas. Mainly, there are different symptoms
that have diabetes like excessive thirst, Fatigue, Losing weight without trying to, blurred vision,
tingling in your hands of feet and many more (Varghese and et. al., 2015). This type of
symptoms are show the issue of diabetes in human body of Treharris community. As social
determinants of health is play a crucial role in determining the issue of access, that directly
incorporates difference in vulnerability and exposure by inter-sectoral action that directly led

from the health sector. As this type of system is important in mediating the differential
consequences of illness in people lives.
Dementia: It is also related with the overall term for diseases and conditions that is
characteristics by a reducing in memory, language. This type of health issue majorly occurs in
Treharris community people due to lack of thinking ability, critical thinking ability and memory
loss as well. As it is a groups of condition that is characterised on the basis of impairment of at
least two brain function of human like memory loss and judgement (Skellern, Hutchinson and
Dunn, 2016). In addition of this, dementia is major concern of disability and dependency among
older people in Merthyr Tydfil. As it is also affects older people because it is not a normal part of
ageing. Especially in over the world approx 50 million people have dementia that has a physical,
social and economic impact on people health and their carers, families and society at large level.
This is arose due to lack of awareness and proper knowledge about dementia that resulting in
stigmatization and barriers to diagnosis and care. In this, individual requires proper education,
income so that they can take proper treatment to improve the health and well-being.

Apart from this, the concept of CSDH conceptual frameworks is also play a significant
role in analysing as well as managing the issue of health in Treharris community. As it also
required proper attention towards the health needs so that people get easily aware about the
different health issues. Mainly this frameworks includes different factors and stages that can be
understood by following points:
Structural determinants/social determinants of health inequities: Socio-economic Position: The main role of social position in developing as well as
generating health inequalities that is crucial for human for keeping their health secure
(Shen, 2015). In this regards, it also supports to the human rights substantiate a demand
over the part of oppressed and marginalized communities in order to effectively
expression their collective social power. Furthermore, the role of power in context of
better understanding of social path way that may support in tackling the entire social
determinants of health inequalities.
Intermediary determinants/Social determinants of health: Material circumstances: This type of aspect includes housing and neighbourhood quality
and consumption foe example, financial is refers to purchase healthy food and warm
clothing that protect people from any kind of healthy issues. Behaviours and Biological Factors: Under this factors, it consider physical activity,
tobacco consumption and alcohol consumption which are distributed in different social
groups that affect their health. In addition of this, biological factors are also based on the
genetic factors that also affect their life style and health as well.
 Psychosocial factors: It may consider stressful living standards and relationship as well.
It is also related with people psychological stressors due to hectic life style and income
level as well.

implications for health or inequalities in the community
In Merthyr Tydfil, there are number of older people who are facing healthy issues due to
climate change and Brexit as well. In this regards, dementia is related with the collection of
symptoms that is categories on the basis of individual cognitive ability and their emergence of
behaviour disturbances and loss of independence that is inability to undertake the day to day
activities and task as well (Barakat and Ucheonye, 2016). In addition of this, one of the common

and general presenting symptom is related to the short term memory that is cause of progressive
damage of brain wherein in older people reducing person's mental and physical functions slowly.
In addition of this, this type of health issue is account for around for 80% and which is over 75
years of age people in Merthyr Tydfil. The impact of the diseases on our health services is
enormous. Furthermore, there are adverse effects of climate change that are experience by low
income communities in over the world (Mmuoegbulam, Antai and Asikong, 2018). In this who
have higher level of vulnerability that is related to environment determinants of health, wealth
and various another factors. As climate change also poses a wide range of risk in context to
population health risk that may increase the ratio of health issues. If global climate change
continues on the specific as well as current trajectory. In this regards there are three main
categories of health issues such as direct acting effects, mediated through climate related changes
in ecological system and other relationship and last is more diffuse consequence that is based on
impoverishment, displacement, resource conflicts or mental health problem. Apart from this,
current variations within the climate condition that is already have adverse impact over the health
of people who are not much stable in their financial conditions. As this type of impact may
maximizing the rates and ranges of transmission of any kind of infections diseases that is caused
by unclean water and contaminated food. Change and variation in temperature, seasonality
includes the agricultural production in different regions that consist some of developed countries
through which health and functional capacity of older people get affected.
In addition of this, Brexit also huge their impact over the health issue as people who
belongs to Treharris community as it is comes in Merthyr Tydfil. It is developed country that
have effective income scale and education level as well. Due to high pressure of working life
large number of people get affected as they take huge tension. This is the reason that people
suffer from dementia as they loose their memory and critical thinking that directly affect over the
human body and mind set as well. As it is one of the major concern for Treharris community in
Merthyr Tydfil.
d) What the profited community could do to help reduce the impact the Brixit or climate change
on the 2 key issues
Treharris is a given community that plays important role in reducing the issue of Brixit or
climate change on major two key issues (Castaner and et. al., 2018). These issue are dementia
and diabetes. In Merthyr Tydfil, most of the population are faced such type of issue that have

negative impact on their entire life. Brexit has main implications for health and social care in
Treharris. Governance mainly concerns relate to public health of Treharris community,
competition and trade law, as well as research also. However, climate chance and Brexit have
identified a several potential chances for improvement in some areas such as competition law,
flexibility of training, should the Treharris Government take them. As it will help those people
of society who have effected through diabetes and dementia. Both are major health issue that
have negative impact on life of an individual. Diabetes is considered a hazard factor for vascular
dementia. Such kind of dementia happen due to brain damage that is turn to impact on people by
reduced or blocked their blood flow to brain (Gómez-Olivé And et. al., 2018). Many people with
dementia and diabetes have brain occurrence that are hallmarks of both vascular dementia and
Alzheimer's disease. In order to overcome this issue, government of Treharris taken an effective
action treating patients safely, providing better facility etc. Community also played important
role by increasing awareness among the people who have effected from diabetes and dementia.

From the above mentioned report, it has been concluded that developing a community
profile refers to the determining community issues and attitudes that is directly locating notable
features in the specified area. Along with this, assessment of potential social and economic
condition and trends in the particular. Furthermore, some information can be gathers through
primary sources like face to face conversation with people. Being with the evaluation of the
population of Merthyr Tydfil economic characteristics and trends of the broader community and
the particular area. In context of this, health conditions has also affect the overall performance as
well as condition of Treharris. In this demographic health needs also fulfilled by including
various aspects of evidence based practices. It may support in managing the health issues and at
the same time also improve the economic condition of Treharris in Merthyr Tydfil. Apart from
this, Brexit is also affect the whole community that raised issues in Merthyr Tydfil.
