Identification of Focus for Community Teaching: The self-management of diabetes has long
been acknowledged as an important component of care for the people with diabetes. As per the
current statistics the prevalence of diabetes continues to be one of the greatest threats for the
ethnic minorities. The community care workers play an important role in the diabetic
management. Hence, a primary care clinic has been chosen for the community teaching plan for
the management of type 2 diabetes.
Epidemiological Rationale for Topic
The rationale for choosing diabetes for developing a community teaching plan is based on the
fact that, in the year 2018, about 34.2 millions of Americans or 10.5 % of the population had
diabetes. About 26.8 millions are undiagnosed with diabetes. As per the reports, in the year 2015,
88 millions of Americans aged 18 years had pre-diabetes (American Diabetic Association, 2018).
Furthermore, diabetes has been considered as one of the seventh leading cause of death in the
United States. Among the various ethnic minorities found, the prevalence of type 2 diabetes
among the American Indians were found to be the highest.

knowledge deficiency and the risk for ineffective therapeutic management.

are emotionally prepared. Furthermore, while disseminating information about, innovative or
interesting methods needs to be adopted, such as distribution of pamphlets or some colorful
souvenirs.
Experimental readiness: this will be displayed, once the participants’ starts asking questions
about diabetes, or will be able to answer the questions with readiness.
Learning Theory to Be Utilized:
Social learning theory can be applied to the health promotion program that considers the
dynamics of individual behavior and it provides directives for designing the intervention
strategies for influencing the behavior change in the target audience.

To reduce the disease burden of type 2 diabetes and improve the quality of life.
To reduce the annual number of new cases that has been diagnosed with diabetes
