Topic: Prevention and management of Type 2 diabetes
Identification of Focus for Community Teaching: The self-management of diabetes has long
been acknowledged as an important component of care for the people with diabetes. As per the
current statistics the prevalence of diabetes continues to be one of the greatest threats for the
ethnic minorities. The community care workers play an important role in the diabetic
management. Hence, a primary care clinic has been chosen for the community teaching plan for
the management of type 2 diabetes.
Epidemiological Rationale for Topic
The rationale for choosing diabetes for developing a community teaching plan is based on the
fact that, in the year 2018, about 34.2 millions of Americans or 10.5 % of the population had
diabetes. About 26.8 millions are undiagnosed with diabetes. As per the reports, in the year 2015,
88 millions of Americans aged 18 years had pre-diabetes (American Diabetic Association, 2018).
Furthermore, diabetes has been considered as one of the seventh leading cause of death in the
United States. Among the various ethnic minorities found, the prevalence of type 2 diabetes
among the American Indians were found to be the highest.

Nursing Diagnosis
Some of the nursing diagnoses that will be considered is the risk for unstable blood glucose, the
knowledge deficiency and the risk for ineffective therapeutic management.
Readiness for Learning:
Emotional readiness: People should actively show up for the teaching, which will show that they
are emotionally prepared. Furthermore, while disseminating information about, innovative or
interesting methods needs to be adopted, such as distribution of pamphlets or some colorful
souvenirs.
Experimental readiness: this will be displayed, once the participants’ starts asking questions
about diabetes, or will be able to answer the questions with readiness.
Learning Theory to Be Utilized:
Social learning theory can be applied to the health promotion program that considers the
dynamics of individual behavior and it provides directives for designing the intervention
strategies for influencing the behavior change in the target audience.
Goal:
 To reduce the disease burden of type 2 diabetes and improve the quality of life.
 To reduce the annual number of new cases that has been diagnosed with diabetes
 Reduce the rate of death due to diabetes (Powers et al., 2018)
 To increase the proportion of people with diagnosed diabetes who have received formal
diabetes education.

How Does This HP2020 Objective Relate to Alma Ata’s Health for All Global Initiatives
The Alma Ata’s Health for All, global initiatives is associated to the wellbeing, mentally,
physically and socially. This concept has been followed in diabetic care, by the use of the
framework of bio-psychosocial health (ODPHP, 2018). The Alma Ata declaration also
emphasizes on the inequalities existing in the health care system (Kalra, Akanov & Pleshkova,
2018). The current HP2020 Objective also is concordant the philosophy of the diabetic care. The
objectives also recognizes the inequality existing in the health care.

Creativity: How was creativity applied in the teaching methods/strategies?
The creativity will be displayed in the making and the designing of the pamphlets and the
presentation. It should be ensured that the teaching materials were easy to comprehend
containing correct and valid information. Short videos can be shown to the audience regarding
the actual effect of diabetes. The worksheets used will be simple.
Planned Evaluation of Objectives (Outcome Evaluation):
1. I will ask question to the target audience to evaluate the teaching and to ensure that every
body has acquired knowledge regarding the management of diabetes.
2. I will conduct a discussion session, where the audiences will ask their queries and questions.
3. I will call few representatives on the podium randomly, such that they can demonstrate the
use of glucometers at home.
4. I will ask the audience to fill up a worksheet contain basis questions about the diabetes
teaching.
Planned Evaluation of Goal:
After the completion of the teaching session, there will be a teach back session, where the
audiences will be allowed to ask their queries. Feedback can be seeked by asking the audience to
fill up a feedback form.
Planned Evaluation of Lesson and Teacher (Process Evaluation):

Stakeholders will be selected who will be evaluating the proposal and would suggest any
changes that needs to be brought in the teaching plan, or the materials that will be used for the
teaching.
The teachers evaluated whether the questions provided in the feedback form has been
appropriate or the questions that will be asked during the teaching are relevant to the teaching
plan
Barriers: What are potential barriers that may arise during teaching and how will those be
handled?
Some of the identified barriers for the communication teaching plan would be lack of knowledge
or unwillingness to participate in the teaching plan, old health beliefs causing them to follow
certain traditional lifestyles, failing to understand the gravity of the chronic condition or how
diabetes can affect their future generation.
Another major barrier would be funding.
Therapeutic Communication
Before starting of the presentation, I will show short documentary to the target audience.
The video will not be more than 5 minutes. In the meanwhile the information handouts will be
distributed. The initial task would be to make the audience understand the incidence of diabetes
and how is it affecting out daily life. This should be followed by the presentation. During the
question answer session, it should be ensured that each and every audience gets the opportunity

to put forward their queries. The teachers who will be teaching should use appropriate body
gestures while communicating. Demonstrations regarding the use of glucometers at home for
monitoring the glucose level will also be provided. People were given much time to think and
discuss, before feeling up the feedback form. The presentation will ended after the completion of
the feedback round.
