Introduction
The essay is generally based on the case study that
is related to John who was having the treatment of
colorectal cancer. The case study generally portrays
about colorectal cancer and its different kinds of
symptoms and care plan that are highlighted in the
case study, in order to prevent John from getting
affected by the diseases, John and his wife Carol
met the doctor about the issue and the doctor
prescribed the ways of treatment. Now, John has
gone off from the operation and is currently
receiving his last cycle of chemotherapy. I am the
Cancer Care Coordinator at the hospital where John
is receiving the chemotherapy. I will meet him to
provide detail information about the discharge and
self-management planning with a follow up of the
active treatment.
Discharge Planning
The diagnosis and the identification of the disease
are generally done through a bowel cancer test.
John was actually ignorant about the issue and his
wife Carol pushed him to visit the doctor and the
doctor assured him about the existence of colorectal
cancer in him. There are different discussions done
on the basis of surgery, postoperative plans and

postoperative plans that can make John sustain the
disease (Malik et al. 2018).
PTFU is generally acceptable to the patient with
colorectal cancer. This can be considered as a
reality with the alternative to the OPFU for the
patients that are clinically in a sustainable state.
This bears to some higher initial cost with a
provision of self-management program (SM) with
proper remote surveillance. There should be an
intensive follow up after the curative resection of
colorectal cancer which is associated with a specific
approach to survival (Geer, 2016). This generally
benefits the regimens which employ cross-sectional
imaging and also with the carcinoembryonic antigen
determination. There are risks adapted in the
approach of the follow up which are intensively
making the patients with a higher risk of the
recurrence (Worrall-Carter et al. 2016).
It also increases the overall efficacy and the cost-
effectiveness which are related to the disease. It can
be said that ongoing improvements in the
stratification of risks, detection of diseases and the
overall treatment will generally increase the
surveillance of the postoperative state to John.
There is generally a need for large and randomized
control trials for the determining of the optimal
surveillance regimen. This will include with it a
quality of survival analysis, life with quality and a
proper and cost-effectiveness towards the

assessment of the efficacy (Moodie, Tolhurst &
Martin, 2016).
The evaluation, diagnosis, plan of care and
prognosis should be documented with a date which
is authenticated by the therapist who is associated
with the performing of the service. There is an
intervention provided by the doctor on the basis of
the treatment and physical condition. This can also
be given by the assistant doctor, but all should be
dated and documented and authenticated by the
permissible law. There are separate flow charts
which are also considered as the component of
documents or record for self-evaluation as well as to
carry on the ongoing treatment process. The
symptoms related to the recurrence of colorectal
cancer are the same as being faced in the first
instance. This generally includes weight loss,
diarrhea, and constipation. This also comes with
back or pelvic pain and at times trouble in breathing
(Sahle et al. 2016).
Collaborative approaches to education and
planning for self-management
The number of long term survivors of cancer is
generally 5 years. The cancer survivors generally
return in the normal functioning after the
completion of the treatment. This results in a wide
range of physical and psychological problems
which recede with the time of association. These

problems are related to the emergence of the
treatment of cancer and other chorionic diseases
that are long term in nature. There are psychological
long term effects and often late effects which are
generally associated with the deficit in the overall
physical and emotional functioning. There are
emergencies of public health initiatives which are
on the basis of large scale. There should be an
increased awareness of the follow-up plans of care
which are allowed in order to allow a proper and an
adequate surveillance rate. There is prevention in
the management of the long term effects of cancer
that is needed to be taken up by John in order to
stop the recurrence of the disease further (Maiorino,
Bellastella & Esposito, 2015).
John needs to stay under active checkups in order
to survive. This is because active checkups will
keep the doctors updated about the disease. They
will understand that there is a chance of recurrence
or patient is totally out of the danger. In case of
recurrence, then there should be radiation therapy
applied to John as per requirements (Billiet et al.
2018).This will provide a proper care plan and
active identification process. This is because such
diseases take a lot of time to cure and it never gets
fully cured up. Therefore follow up treatment is a
must in case of John. There are several ignorance’s
which have been shown by John at the first
instance. This ignorance has led to such a situation.

The symptoms which took place like back pain or
stomach pain or indigestion was not at all looked
carefully. Therefore after the active treatment which
is going on, John needs to be more careful about the
diet he is in taking (Lawrence et al. 2016).
He should abide by all the rules related to health
that are being prescribed to him during the
discharge time. The physiological impact and the
uncontrolled anxiety leads to a reduction in the
quality of life, the rate of suicide attempts increases,
reduction in the adherence of the recommended
surveillance and also in the reduction of the health
engagement that is related to health-promoting
activities (Tamagawa et al. 2015). There is a need
for development in the therapeutic activities,
elicitation of the unmet needs, proving of proper
information, and in the expressing of the empathy
that is active towards the listening. There are some
strategies which can lead to overcoming such
diseases; they are like psychotherapy, an
antidepressant medication which contains with or
without anxiolytics. There should be referral given
to social work that is considered as counseling and
chaplaincy (Hafekost et al. 2016).
These actions can reduce stress and make the
patients feel stress less. These kinds of medicine
can also reduce the chances of having suicidal
attempts. Therefore it can be said that a patient
should continuously stay in touch with the doctors

so that slight problems that arises can be cured.
There are different mental health care plans in
Australian Cancer society which deals with such
problems effectively. Cancer treatment causes hair
loss, nausea, vomiting which is a social and
emotional problem and often can make John feel
low. Spirituality can also be considered in the broad
sense with the lives of the patient during stress,
illness and dying (Dunn et al. 2017).
There are different existential issues which trigger
at such point of time when a patient is suffering
form cancer. Therefore there are spiritual diagnosis
done on the basis of the mental condition and other
clinical symptoms ascertained with (Carlson et al.
2016). These strengthen and also make a positive
impact on the recovery of the patient. Therefore it
can be said that John can go through such problems
when he will take rest in his home. As a Cancer
Care Coordinator at this hospital, I would solely
suggest Carol to look after her husband carefully. If
the mental condition becomes unbalanced, they
should positively visit for a checkup, otherwise, it
can lead to different other problems (Dobson et al.
2015).
These are some of the essential symptoms and
identification according to the situation which
should never be ignored. The patient should be kept
under proper follow-up checkups. This will also
increase the rate of survival and the issues regarding

the survival strategies will also be solved (Carlson
et al. 2015). This is the only intention of the
identification and discussion of the issues prior to
its happening. So that there are no problems faced
further regarding the mental or physical imbalance
of the patients. This is because at times such
imbalances can lead to a serious injury to the
survival of the patient (Anderson et al. 2016).
Conclusion
It can be said that there should be a proper follow
up of treatments required, in order to make John
survive for at least 5 years. Because, after treatment
of such cancers the sustainability rate is hardly 5
years. There is a need of proper checkups so that the
disease does not get spread to the other parts of the
body and disturbs any other associated organ.
Adjuvant Chemotherapy generally helps in the
prevention of the recurrence to the disease. This is
particularly addressed with distance recurrence.
Looking to the current situation, these are discharge
planning and collaborative approaches to education
and planning for better self-management.
