Outline the Planned Support model for Case Management, giving a brief summary of each of the
seven (7) steps.
In this case management the focus is on various type of programs funded by
Commonwealth department of social services. Knowledge on funded programs by government is
provided. Information on community service organization is provided with their referral forms.
A brief overview of generational poverty is given and knowledge about Welfare Dependency in
Australia is provided that what are the trends and where they need to focus in order to improve
things. Lastly on the bases of the situation certain questions are answer which help in
understanding that uncertain things can take place at any point of time and one need to be ready
to deal with them in effective and efficient manner. One should focus on the things going around
so that they would able to analysis situation and take correct decisions.
Question 1.2
Research the types of programs funded by the Commonwealth Department of Social Services
(DSS)
There are various type of programs which are funded by Commonwealth department of
social services (DSS) which area as follow-
 Certificate provided by Australian government for volunteering- Volunteers are the one
who play important role in shaping country where they focus on building stronger and
resilient communities (Commonwealth Department of Social Services. 2020).
 Bali Lifetime Rehabilitation Assistance package- In this package government provide
lifetime assistance for extra expenses for the rehabilitation purpose.
 Fair pay for workers working in social and community services- The people working for
social and community services are given fair pay as per government instruction.
 Seniors connected program- It’s a key part of government’s election policy 2019 where
focus is on providing support to old people that is about 55 age.
Write a brief summary of the funded programs including the relevant guidelines and processes
for this type of funding.

The funded programs of Australian government are research training programme (RTP)
for scholarship for international students in which tuition fees are for free and stipend is provided
for general living. Commonwealth government university provide scholarships for Masters &
Phd students in which annual living allowance of $27,082 is provided (funded programs of
Australian. 2020).
Question 1.3
Identify a community services organization and provide an example of their referral form
Avon community services is an organization which is providing services in two streams
which are housing services and Youth services (community services organization. 2019). By
providing housing services they are helping individuals to maintain tenancy. With the help of
case management the company help in budget and maintaining of houses according to standards.
In youth services they organize six week programs with the objective to educate young people
regarding various social issues. Example of their referral form is as follow-

In your own words, define generational poverty.
Generational poverty is termed as system in which families are facing difficulties due to
lack of money and are living in poverty from minimum of two generation. People belonging to
generational poverty focus on survival and deal with daily issues or challenges and do not focus
on planning (Xiu, Guo and Li, 2020).

Review the article Welfare Dependency in Australia which is uploaded in the course resources.
Welfare dependency main aim is to focus on individuals belong to working age and on
cash payment receipt. This can be calculated by comparing administrative data and taking
information from ABS data on population of working age. Trends in the welfare dependency are

according to the type of households it has been identified that recipiency rate vary from the graph
of year 2009-10. It has been identified that there is currently rise in proportion of working age
people in year 2009. Around 65.7% of people has received welfare in between the time period of
2001 and 2009. It has been identified while comparing the benefit of recipiency in international
marketplace, Australia has able to take good benefit. Australia is the country who is highly
relaying on income testing in comparison to other countries. Furthermore, it’s current system is
most progressive which is providing benefit in the OECD. The explanation for rise and fall in
benefit recipiency depends on three components which are labor market trends, demographic and
family changes and policy changes. Australia stand on sixth number for lowest unemployment
rate in OECD (Morin, P. and Rigazio, 2012).

translating services (Jia and et.al., 2013).

b) The client requests financial assistance. After speaking with the client, you realise she has
a number of debts and a referral to a financial counsellor is appropriate. The financial
counsellor at your service is a male. Through the translator, the client explains it would
not be appropriate for her to see this worker.
(i) Describe the importance of respecting this cultural difference.
It is important to have knowledge of different culture as it helps in dealing with uncertain
situations and most importantly helps in establishing effective communication. The people would
able to understand the situation and also provide other person accurate information who is belong
to different culture. This helps in taking effective decisions within less time span and effective
results can be accomplished (Aronson and et.al., 2017).
(ii) Explain how you would handle this situation.
In this situation where client is not able to communicate effectively with the help of translator so
in this case we will look for another translator. The one who is able to understand the situation
properly and provide accurate information to the client so that she is able to have effective

information and take the advice of financial counselor. Moreover, with the use of video calling
the statistics and other essential information is explained to her (Wu and et.al., 2015).
c) One day the client’s husband comes into the centre and says that his wife is sick but he is
here to pick up documents she needs for the bank on her behalf. He is very demanding
and insistent.
(i) Outline the correct process to follow in this situation.
In this case the process which need to be followed is that they should ask for the
proof that he is her husband. After that cross verification should be done that is
approve from client side should be taken. If client agrees and give the permission
to give the document to her husband, signature of husband must be taken and after
than they should hand over the documents.
(ii) Explain how this process fits in with handling family structure and dynamics of
clients.
This process fits in with handling family and dynamics after consulting with the client
and understand the exact situation. Steps must be taken by the authority after informing
the client and taking her approval before every step.

5 young children. Her partner is currently in gaol, serving a lengthy sentence. Sarah recently lost
custody of her 5 children, who are currently in Foster Care due to Sarah’s mental health and drug
issues. Sarah wants to have custody of her children. She mentions she is having trouble

order to ensure that she is taking her medicines on time and would able to recover fast. Secondly

c) After one meeting with the housing case worker, Julie tells you that the case worker can
assist her with the rental arrears however Julie will need to find new accommodation more
suitable to her current income (Centrelink). Julie seems very anxious and depressed about
moving and her whole situation and says she would rather just kill herself than deal with it
anymore. You are very concerned about her well-being and anxiety.
(i) Explain the importance of following your duty of care in this situation.
In this situation one should focus on doing yoga and should go for outing rather
then thinking about the situation. If she goes on thinking about the problem all the
time it will lead into many other problems such as health issues, depression, etc.
She should focus on getting new job opportunity in order to find solution to the
problem.
(ii) Outline the process you would take to ensure you have abided by your legal
reporting requirements.
The process which Julie must follow in order to abide by legal reporting
requirements is that she should hire a lawyer. The process and the actions guided
by lawyer should be followed by her as lawyer is the only person who has the
proper information on legal requirements. Hence steps and document work
formulated by lawyer must be followed by her.
(iii) Write an appropriate case note documenting this session

currently unemployed. She has shifted to new place according to her current income. But
the problem is she is facing difficulty in adjusting in new place which has lead into
depression and anxious. Here mind is not stable and client is thinking just to kill herself

In the case of Jem, he is suffering from mental health problem, have lost his job and is in need of
place for accommodation. The role I am playing is of Mental health case manager. I have major
contribution in the case as piece of mind is essential to carry any activity in effective and
efficient manner. Client has get benefit from all the services he is getting as all the services are
important as he is facing mental problems and do have job and place to live. By following
effective structure and outline the activities which need to be performed in order to obtain desire
results has helped in providing benefit to the client and has helped in preventing service
duplication problem. The case conference is complex in nature where at a time, there are many
problems which need to be handled. By understanding the problems and according to problems
and difficulties an blueprint must be prepared which cover all the issues so that effective actions
can be taken, hence in this manner improvements can be done.
