Importance of Dental Records and Charts: It is essential practice of dentistry to keep
the records of patients in proper manner. It is fundamental to delivery of quality patient care.
Patients records and charts are official source of treatment and patient related communication,
clinical notes, diagnostic information. Charts and records provide invaluable data which includes
the past and future information. That is helpful to assess the quality of care. With that it assist
plan for treatment going forward.
Dental charts: it is a graphical tool for maintaining and organizing all the essential
information and records of patients gums and teeth. It keeps the record regarding to issues and
conditions which is described in dental chart. It includes records such as area of cavities, missing
teeth.
Radiographs: it is equipment with help of it images of teeth can be analysed.
Radiographs has law level of radiation to identify the malignant or benign masses, to find hidden
dental structure, cavities, bone loss. It assists dentist in order to analyse problems that impact
teeth and gums.

Adults: Sign of distress can be monitor in adults by noticing and reporting their daily
routine such as setting alone, avoid conversation with friend and families, addicted to drugs are

consulting them psychicist.
Older people: in older people it can be identified by the monitoring circumstances which
they suffer such as extreme stress, bereavement, trauma, physical cause like medical conditions,
Alzheimer disease and other dementias. It can be manage by mediation, nutritional counselling,
acupuncture etc.
Children: Sign of distress in children can be monitored by analysing their behaviour
which includes restlessness, inattention, poor focus, avoidance etc. it is managed and supported
by focusing on the positivity, scheduling relaxation activities, rewarding them for their brave
behaviour and so on.
People with special needs: These kinds of people are very sensitive. It is important to
monitor distress via analysing their changing behaviour which can include agitation, somatic
symptoms such as stomachache, headache, tantrum, crying, restlessness etc. it can be manage
and supported through traditional medicines, relaxation technique, yoga, naturopath and so on.

Why patients refer to other team members and how is this done or communicated.
If team member are not able to understand and analyse the problems of their patients so
in this case patients refers to another team member. Such as many patients come with different
dental problem such as teeth ache, gum disease, tooth erosion, cosmetic needs, bad breath, oral
cancer, mouth sore and so on. Considering this, patients are referred to another team based ion
their requirement. For this team members can communicate with orally which includes face to
face communication, written communication such as emails, telephonic communication etc.
