INTRODUCTION
Oral health assessment can be described as to move from a restorative approach to patient
care in order to make them disease free. It involves to provide support to an individual to
determine actual oral problem which facilitate to reduce risks along with meeting needs of them
appropriately (Poklepovic and et. al., 2013).

By conducting the assessment of individual to ascertain any medical discrepancies,

(Outpatients).

A referral is basically communicated to other team members by using the method of fax
as this easily and quickly communicates the information to other end.
4.5 How communication can support individuals who present signs of distress
The patients in whom doctors or other health care centre members ascertain a matter or
sign of medical distress can be supported by way of communication as doctors can interact with
them to tell them about how can the treatment be proceeded and individual can be cured
(Chapple and et. al., 2015).
