PART 1 INTRODUCTION
There are some health issues which affects growth and development of people
residing in society. These days there are different health issues such as obesity,
physical inactivity, smoking, diabetes Type 1, diabetes Type 2, alcohol misuse,
depression, stress, sexual health, teenage pregnancy, etc. are happening. This report is
based on health and social care organisation National institute for health and Care
Excellence (NICE). In this report there is discussion about diabetes Type 2 which is a
diabetic health issue which affects individual's body severely. Culture and environment
are two factors which discussed which affects this disesae.
In UK around 2.7 million people suffering with diabetes (Diabetes UK, 2018).
There are possibilities that this data may reach to 4.9 million by 2035 (PHE, 2016). This
might cause death of person. Diabetes is lifelong health issue which gives kind of
glucose in person's blood. In this condition, individual is not able to utilise blood sugar
from body. This disease affects health of person in adverse manner. There are some
server issues which arise due to diabetes Type 2 such as Hyperglycaemia,
Hypoglycaemia, Microvascular complication on brain, eyes, blood vessels, neuropathy,
hearing impairment, skin problems, etc. Out of such complication, in this report there is
discussion about Neuropathy foot ulceration. This is the disease due to which wound
occur on foot of patient.

PART 2 FACTORS INFLUENCE THE HEALTH OF THE INDIVIDUAL AND
THE WIDER SOCIETY
There are different factors which gets affected because of diabetes Type 2.
Environment and culture are two factors which are used in respect of diabetes Type 2.
These factors give positive as well as negative impact on individual as well as society.
Out of total diabetes patients approximately 15% suffer from this complication.
Diabetic Neuropathy is damage of nerve fibres in people due to low or high level of
insulin (Connell, 2012). Due to Neuropathy foot ulceration, there are possibilities of loss
of sensations in feet, excessive pain. There are possibilities of painful wound at foot of
person. This is because wound gets serious and possibilities that individual has to be
hospitalised for treatment. This is combination of lower limb arterial insufficiency, lower
limb diabetic and local trauma. There are possibilities of inadequate flow of blood and
hence surgical treatments are done in order to improve circulation of blood. There are
possibilities that person has to use wheel chair, special footwear, crutches, etc. NICE
Guidance: Type 2 diabetes in adults: management is discussed in order to deal with
Diabetes.
CULTURE
Culture is one of the important factor which affects health of person. Culture
means trends, rituals followed in society. There is difference in culture of various
different countries as per change in geographical region, human behaviour, etc. There
is positive as well as negative impact on society as well as individual.

Diabetes Type 2 is the disease which is much prevailed in UK society (Fromm,
2013). Diabetes has negative impact on health of individual. Due to Neuropathy foot
ulceration, there are wound at feet of patients. These become adverse due to increment
in quantity of glucose in body of patient, this may lead to hospitalised of patient and
sometimes lead to death.
Positive influence on individual
Literacy rate of UK is good which makers people aware of actions such as
regular exercise to be taken to overcome diabetes.
Life style of UK is fast and forward, this makes individuals to people more
physical activities. This helps to reduce possibilities of diabetes. In UK there is less trend of sweets, beverages, etc. on various occasion, so this
reduce possibilities of health issues to people.
Negative influence on individual
In UK there is more consumption of alcoholic drinks, so this affects health of
people. As diabetes Type 2 is caused because of alteration in level of insulin, so
if people in take more alcoholic drinks then this affects health of patients. NICE

There is lack of emotional touch among people of UK. This affects mindset of
people residing there because they feel lonely and they have to handle pressure
on their own. This increases chances of Diabetic among people.
IMPACT ON SOCIETY
In UK there is more use of packed food, this creates health issues among people
residing in society.

Diabetes Type 2 may lead to death of patient. This lead in increment of morality
rate in society.
There are different conferences, seminars, workshops which provides knowledge
about neuropathy. This helps to creates awareness among people and hence
they perform operations to overcome negative impact of diabetes Type 2 (Garner
and et. al., 2012).
ENVIRONMENT
Environment means circumstances which occur in external environment of
society. In environmental factor vegetation, natural resources, etc. are considered. In
UK, there are many issues related to environment such as air pollution, climatic
changes, etc. are occurring. These also give negative impact on health of people
residing in society. Due to environmental issues, there is problem to patient suffering
from Neuropathy foot ulceration. In rainy season, wound become more critical.
Environmental factors may lead to intake of chemicals, microbiome, etc. which leads to
diabetes Type 2.
IMPACT ON INDIVIDUAL
Pollution such as noise, air or water, lack of cleanliness, etc. has adverse impact
on health of people residing in society. As there are many factory having many workers,
there are some harmful radiations, etc. which affects health of people. There are many
issue arise from such factories such as dysfunction of genes, energy expenditure,
problem in breathing, etc. which affects diabetic patient's health.

People of UK use many ways such as exercise through they can keep
themselves fit and healthy. People of UK believing in vehicle pulling. This helps to create good and safe
environment for health of people and possibilities of Diabetes reduces (House
and et. al. 2013).
Negative influence on individual
Due to Flaura and fauna in environment. There are possibilities of negative
impact on performance on health of public.
Due to excessive industrialisation, there is lack of natural vegetation which
affects living style of individuals.
IMPACT ON SOCIETY
Agricultural waste, dumping of waste material, etc. affects environment of
society.
Over- crowding at occasions of festivals, vacations, etc., also hampers
environment cycle, so there are possibilities of health issues to people.
UK is good name in tourism sector also, in case of imbalance of environment,
this affects image in industry.
IMPACT ON WIDER SOCIETY
Due to Diabetes, there are various different types of cost incurred in order to
manage and handle them. These includes loss of job, mal-nutrition, early age death,
sick leaves, etc. There is requirement of medications for proper treatment of patients.
As per Diabetes UK, 2016, costing of drugs was around £956.7million in last year.

FROM THE UK
Government of UK is taking corrective actions through which they can reduce
possibilities of performing operations in effective and efficient manner. As per this
concept, there is requirement of change in plans and policies which are effective in
maintaining good environment in society (Marmot and Bell, 2012). Government of UK is
taking actions such as opening of health and social care sectors which helps to take
care of people. They have some policies through which negative outcomes of diabetes
can be reduced. There is a policy launched by NICE Guidance: Type 2 diabetes in
adults: management in 2015.
As per this guidance take action in order to take care adult having age more than
18 suffering from diabetes Type 2. They talks about diet chart, creating awareness,
Cardiovascular management, glucose level, etc. There are some advises under Mental
Capacity act 2016 which helps patient to deal with Diabetes. This act helps to make
policies through which health carers can make patient to make changes in working
style, so appropriate actions can be taken. As per this act, people who are suffering
from diabetes Types 2 are assisted to deal with external changes in effective way.
There are some programmes, such as conferences, seminars, etc. which is effective to
provide knowledge about changes in working style. This policy provides knowledge
about diet chart of patients and activities which has to performed to maintain physical
fitness of individual such as reduction in obesity, encourage to weight loss, etc. There

are various dietitian under NICE act 2015 which provides knowledge according to
culture, health belief of patients as per their trends. There are possibilities that people
do not accept diet chart of provided by dietician, in this case dietitian has opinion in
regarding such diet chart. This helps to set glucose level which helps to maintain level
of glucose in patient's body. In order to get positive results out from such plans, there is
requirement of some awareness plans such as health check-up camps which helps to
overcome long term health issues (Regitz‐Zagrosek, 2012). NICE helps to improve
condition of patients suffering from Foot Neuropathy for educating them about dos and
don't guidelines. They can create quality standards and performance metrics for guiding
patients about health. NICE provide information about commissioners, practitioner in
order to improve health of patients.
EVALUATION OF POLICY
With the help of change in perception, education, awareness about diabetes
adverse effect on health of patient can be reduced. As per Independence mental Health
Taskforce there is negative impact on mental health of individual because of diabetes
Type 2. Apart from this, there is less awareness about people, hence possibilities of
negative results are more due to Diabetes Type 2. conferences, seminars are
expensive for people who attended it. There are some nurses with aim to maintain
physical and emotional health of patient, but they are not so effective.
PART4 DIFFERENCE IN ROLE OF NURSE
MENTAL HEALTH NURSE
Nursing is the stream in which professional personnel takes care of sick or ill
person. Mental health nurses are individuals who take care of patient who are mentally

retarded or mentally imbalance. They are not able to take decisions on their own in
different situations. It is important that mental nurse is able to make good relations with
person. This helps to make person comfortable and hence possibilities of positive
results are more. There is requirement of some skills which are effective and significant
for maintaining overall growth of patient.
Nurse must have good listening skills which helps to create good and effective
relations with creating interest and empathy with patients. These nurse helps patients to
be social with other people (White and et. al. 2012). Nurses provide guidance to
mentally imbalance patient to deal with situations through practical guidance and
motivating. In this professionals such as social health, psychologist, etc. are involved.
Nurse can educate patient through interviewing, behavioural theories, etc. This helps to
improve condition of individual. In case of mental health nurse, it is important to follow
ethical methods which helps to raise living of standard of patients. When they are not
able to express themselves, then nurse has to understand and help in expressing.
LEARNING HEALTH NURSE
There are many individual, who are not able to learn new things happening in
environment. Hence learning health nurse motivate them to learn new and effective
approaches. There are some physical issues to individuals such as in case of health
issue so they are not able to learn changes which are effective and significant for
market. These individual are able to deal with changes happening n society. There is
requirement of verbal and non- verbal communication in order to make this group learn
few things. Individual suffering from this diseases can frame and read short questions

but are not able to express them. This group is isolated from society and they need
learning through which they can learn new approaches of society.
As per Mental Capacity Act 2017, nurses plays vital role in making them aware of
social alteration and motivate them to learn new behavioural changes. Nurses use good
and impressive images, pictures, etc. in order to give positive vibes to client through
which patient can gain knowledge and actions can be taken to get knowledge of
external changes. Learning health nurse had good relations with peer members and
family, family of patient, this helps in improvement of individual' health. In order to
create awareness among these individual clubs, pubs, events, shows, etc. are used.
This helps them to make aware about environmental changes. But at the same, they
have to take care of these patients and safeguard their interest (Zoeller and et. al.
2012). Social workers, carers, etc. help to maintain health of learn. They paln events,
seminars, etc. through which they can educated about social environment.
CHILD NURSING
Child nurse works to uplift position of children and young generation. These
nurses are closely linked with family also. These days’ diabetes are common disease,
so there is requirement of nurse in order to overcome such issues. Children nurse
makes good relations with children and then it is easy for children for share pains and
grievances with nurse. This helps to maintain good health and make them learn to
protect health. Children nurse plays important role in making changes which are
effective and significant for maintaining good relations among them. These nurses give
advice to parents related to lifestyle, treatment, communication, spiritual beliefs, etc. It is
responsibility of nurse to use language, tone, voice, etc. according to mindset of child.

this helps to maintain good relations among them and sharing f information is easy.
Story telling, pictures, comics, etc. are some effective ways through which condition of
children or young person can be improved. Children nurse must be in contact of every
related party to children such as dietitian, school, etc. this helps to safeguard interest of
children.
CONCLUSION
From the above report it is clear that Diabetes Type 2 is common disease in UK.
There are many complications due to this, where neuropathy foot ulceration is one of
the adverse issue arise from it. In this report culture, environment are discussed which
hampers Diabetes Type 2. There is discussion about NICE Guidance: Type 2 diabetes
in adults. These guidance are effective for maintaining good condition of patients
suffering from diabetes. At last there is discussion about, three types of nurses i.e.
mental health nurse, learning health nurse, child nursing. These nurse helps to improve
condition of diabetes suffering patient and they take actions in order to make them
social aware.
