Introduction
Adoption of electronic health records offers a crucial avenue in which improvements on the
delivery, quality and continual clinical care are necessitated. Electronic health record system is a
digital version of medical information offering real-time, patient-centered access to records and
making information available easily and securely.EHR are crucial avenues for containing patient
medical history, accessing evidenced-based tools and streamlining workflow. This review offers an
EHR reflection based on Gibbs cycle (1988). Reflection is an essential step in reviewing, analyzing and
evaluating experiences in the care process. In this avenue, reflecting the usage of EHR and the
adoption of e-learning in the health care setting is being focused. Fundamentally, the underlying
responsibilities as towards being a registered nurse and evaluation of care outcomes in the digital
era are being reflected upon in this task.
Personal and professional reflection
Electronic health records are computerized systems that collect stores and display patient’s
details. They are essential tools in creating organized clinical information on individualized patients.
EHR systems can advance health care output of patients and improve the quality of patients in the
care services. It offers an avenue for timely access and tracking of patient’s progress (Black et al.,
2016). In this personal reflection focus shall be on the critical incident analysis as an avenue that
needs essential focus is transiting as a registered nurse. This is an essential avenue in dealing with
challenges occurring in the health care practice.
The critical analysis approach is a fundamental process that is essential in managing changes
in the medical services; it is a comprehensive approach that entails focusing on incident assessment
on likely causes, when and why it can happen. Entrenching aspects of EHR mechanisms in health
care practice is an essential aspect of managing the occurrence of incidence and medical errors in
the care practice (Ajami & Bagheri-Tadi, 2013). Focusing on this aspect as a transition in the health
care practice is a key component in evaluating care outcomes for patients through the application of
digital systems to minimize patients' harm and injury.
In my practice reflection, I encountered the emergence of various medical errors among
clinical and nursing practitioners despite the application of EHR systems. Some of the reported
errors emanated from negligence on part of the health care team in reviewing patient case analysis.
Before the occurrence of the incidence, the normal process in the health care system was continuing
as usual. While in my normal hospital activities, I engaged myself on a patient who was scheduled for

surgical procedures. However, before being taken to the surgical ward, it was discovered that the
wrong surgery procedure was to be undertaken for this patient. This signified a great potential
medial error commission as a near miss. The nurses preparing the patient were so dejected, sad and
scared on the repercussion of this activity had it happened was it not for a second thought. The
whole scenario emerged from the health care support team who took the blame and admitted on
pointing wrong information tag on the patient, hence the EHR signifying details of another patient.
The whole incidence brought a huge burden on my shoulder as a graduate nurse since I was
part and parcel of the team evaluating and preparing the patient to the surgical ward. Such actions
or omissions are fundamental and are a cause for alarm in the health care settings due to increasing
harm and danger to the patients which were not intended for. In focusing on this incident, there is a
need for shaping my skills on root because of analyses as a key framework for future practice in
investigating the root cause of problems and medication errors in the care settings. Root Cause
Analysis is essential aspects that are vital as a registered nurse as it allows for a systematic
investigation towards ascertaining reasoning behind the occurrence of incidence in the care setting
and preventing further occurrences in the future. Further, root cause analysis tools in the EHR
framework are essential in providing safe and quality patient care, hence understating these
perspectives during my transition is essential in ensuring positive health outcomes for patients in
future practices (Saunders et al., 2018).
Hence through a critical assessment based on root cause analysis of health care issues in this
context, patient misidentification regarding first point registration processes and accurate patient
identification process played a vital role in this reflection process. Enhancing the appropriate patient
identification process through EHR and E-health solutions are vital for future mitigation aspects
(Alsadi & Saleh, 2019).
Evaluation
According to Jooste and Japser (2011), critical incident assessment and analysis offer
detailed scrutiny on the root cause of incidence and how it can affect the overall future practice in
care settings. It is viewed critically in the sense it can lead to fatalities and harm the patients, the
health care staff and impact the system in general. Assessment of critical incidences is fundamental
in real-life situations as it leads to the promotion of professional engagement. Critical incidence
assessment is used for reflective actions and available tool for the health care practitioners to
develop preventive features which monitor and prevent the occurrence of these issues.
