INTRODUCTION
In the health and social care sector, professional and health workers responsibility is to
meet the needs of on the basis of their health conditions. Social worker role is to restore balance
in an individuals, family and social life (Mengoni and Redman, 2019). They work to help the
individual to maintain health and recover form complicated health condition. Social workers also
helps in the development if individual abilities to make then independent. Down’s syndrome is
the genetic disorder, this effects the physical appearance as well as the ability to learn and
develop mentally. This genetic disorder is caused by the extra inherit chromosome in infant or
baby. In this report care needs of the 30 year old Down’s syndrome service user is assessed for
the development of care plan. Here I am working as the social care worker to build the care
plan for servcie user. This care plan is based on the achievement of service user goal, to become
independent and achieve improvement in life. Reflective analysis of self professional skills are
done in this report in context to the care and achievement of service user goal.
MAIN BODY
From the given case study of the service user, it has been identified that, service user Ola
Adeyemi is the 30 year old man with Down's syndrome. service user has lived his life with his
parents with proper support and care, service user is dependent on his parents for some common
daily life activities, like dressing. service user is not able to perform the social activities, such as
cooking, travailing, job etc. but now due to the frail parents, he has to become independent.
Down’s syndrome
This is the genetic disorder due to this, service user face the several physical, mental
health complication and disabilities. When the child born with the extra copy of 21st
chromosome, that health condition of the child is consider as the down syndrome, which further
lead to the physical and mental disabilities or delays in the development. There are the multiple
physical and mental health complication face by the adults in that situation due to which they
need, support of other in their daily life get dependent on others. Depression, obsessive
compulsive disorder, abuse, hearing loss, spinal cord compression, Periodontal disease are some
common complication associated with that genetic disorder. Social challenges face by the service
user are social withdrawal, loss of interest, regression in cognitive and social skills (McKibben,
2019).

In order to assess the Ola's needs I will take the help of APIE model. This is the
appropriate model of the problem assessment, problem solving and care planing. This model is
communally use in the nursing care practices. Below is the identification of the service user need
with the aid of APIE component.
Assessment
This is the initial step of the APIE, according to this step systematic integration of the
quantitative and qualitative data should be done. Under the quantitative data physiological status
of the servcie user, such as the routine observation of the temperature, weight, blood pressure,
pulse is done. Under the qualitative aspects of the service user ideas, exceptions and concerns is
develop.
For the evaluation Ola's care needs systematic integration of quantitative and qualitative
data required to done. There is the need of proper physical health assessment of the Ola's, such
as measurement of the weight, temperature and pulses. service user strengths, preferences,
aspiration and personal goals assessment is required in order to identify the final care needs of
the service user. For the Ola's needs assessment it has been identified that weight is at normal
category, pulse are normal, his strength is education until the age of 18, another strength of the
Ola's is his interest and self concern about the improvement of health condition to become
independent. He require the physical support for the daily life activities. There is the need of
service user education regarding self care , so that he can achieve his goal to move from home to
more independent living arrangements. There is the need to assist the Ola's with the use of public
transportation and healthy diet, in order to avoid some harmful food component. Ola's need is
appropriate accommodation, daytime activities, employment. There is also the need of speech
and motor skills improvement (Hill, 2018).
Planning
Immediate after the systematic assessment of the service user health condition and care
needs, net step is effective care planing. On the basis of this step formulation of the suitable
interventions and strategies should be done in order to address the presenting problem. Plan
should be focused on service user preference, biological elements, that is also important to assure

the servcie user centred care plan, which can be recordable, observable, directive and
understanding etc.
I have identified that on the basis of APIE theory planing of the service user should be
person centre care, which should consider the individual and the social model of disability.
Models of the disability is involving the to models, such as the individual and social. On
the basis of the individual model, disability view as either person failure or tragedy to be normal.
In other had social model of the disability indicate that, disability is can be caused by negative
impact and outcomes of society. Person centred care is the type of care under which service user
each and every type of needs are managed or fulfilled by the health care staff along with the
medical needs, to help the service user for achievement of their personal goals. Under the person
canter care plan there is the huge participation of service user, they can make their choice of
interventions and seek the help for their personal needs too (Santy-Tomlinson and Mackintosh-
Franklin, 2020). The main care approach for the Ola is holistic to empower him work for
becoming independent, through the improvement and development of physical and cognitive
skills along with social knowledge. For the personal goal achievement of Ola's person centred
care plan individual and social model of the disability is applied. service user care outcomes
goals are illustrated below.
Goals
Improvement of the speech and motor skills of Ola's.
Improve the ability and awareness of Ola's about self care.
Make Ola's independent and eligible to live his life with appropriate job and accommodation.
Below is the brief illustration of the person-centred care plan in context to identified
needs.
In the case of Ola, individual model of the disability indicates that, physical and mental
health disability of the Ola is due to his own genetic problem (Sciaraffa, Zeanah and Zeanah,
2018). But in other hand lack of the understanding and awareness or fear of the social
participation is due to the social factors like, lack of the employment for disable person, lack of
the public facilities on the basis of disable person needs, lack of the accommodation etc. in
context to the all these physical, mental and social issues of the down-syndrome service user key
interventions which can be implement for the service user goal achievement are as follow.

Speech therapy- In the case of down-syndrome individual understand think more
effectively rather then expressing things. There is the need of therapeutic support to increase the
ability to express feeling. This can be done by improving the vocabulary and development of the
language programme, which can help the service user in improvement of his expressive language
and syntax skills. By the help of this intervention Ola empower to live independent, with good
communication ability development. Here as the social care worker my role is to coordinate with
speech therapist to get the treatment or therapy for Ola. Health professional in context to the Ola
will use the strategies for communication and swallowing problems. The key techniques which
will be use for the Ola speech theory are recasting, parallel talk, expansion, praise and modelling
by providing the example of specific language.
Physical therapy- This is another most important intervention for the Ola, as this can
help him out with the improvement of physical movement, with the aid of which he can become
more eligible to live independently. Physical therapists helps the service user to gain the muscles
strengths, balance, coordination and movement skills. Collectively all these factors contribute to
the improved independence with daily activities and quality of the life. Physical theory activities
are fitness games and activities to increase the muscles strength, crawling, pulling, setting and
standing to improve movement. In order to improve the balance and posture control some key
activities should be performed by the service user, such as jumping, skipping and dribbling.
Physical therapy also guide or help for the physical moment improvement by determining
specific exercise and diet. Here my role is to manage the physical therapy session within the
right period of time.
Special education- Due to the lack Ola self care and management skills there is the
higher need of self care education so that he can live independently and take care himself
(VanZant, and McCormick, 2021). Proper hygiene maintenance, healthy diet, behaviour and
public transportation use knowledge will be given to the Ola. With in the UK there are the
numbers of facilities for the disables effective journey, such as passenger assistance schemes,
travel assistance cards, safe place, audio visual information etc. in order to improve the Ola
knowledge and skills for transportation use that is important to educate him about all these
facilities.
Assistive technology- Multidisciplinary staff will make sure that Ola is assisted for the
work activities with the help appropriate supportive tools, such as three sided pencil's,

touchscreen computer, keyboard with Large letters. As a social care worker I will make sure the
availability of each and every facility for the servcie user, I will coordinate to the technical
specialist, regrading the servcie user to help him with appropriate tool, with the aid of which he
can perform the key work in job role. This intervention is can be very effective in context to the
Ola personal independence and employment goal achievement. I have plain to fined out the
appropriate job for him and coordinate to that organisation regarding the service user technical
support needs, and then further send technical specialist to aid the Ola with assistive technologies
for easy working.
Social and emotional support- Individual with the down-syndrome faces multiple mental health
complication like, depression, anxiety and social withdrawal. As an social care worker I will
arrange the appropriate accommodation for Ola and refer the occupation therapist in context to
the Ola transition to the supported accommodation . In context to the prevention of these
complication from the case of Ola that is important to motivate him for the social gatherings,
positive behaviour. I will make sure that I am listening to his needs and problems, by giving the
huge attention so that he can feel good and get influenced to communicate with others in society,
I will coordinate to the occupation therapist regarding the fulfilment of Ola emotional and
physical needs at supporting accommodation (Taub and et. al., 2017). occupation therapeutic
will take care of all the basic needs of Ola.
Implementation- This is another key step or process of the care, involving the
implementation of appropriate care plan, by assuring the clear inputs, which can be practicable.
Under this step service user should also be educate to manage self care. For the implementation
of the person centred care interventions I have to build the therapeutic relationship with Ola, so
that he can communicate with me related to his personal needs and choice of the care. valuing
people plan is involving the some key principles which have to be followed by every health and
social care professionals, those principles are civil rights, independence, choice and inclusion.
The Care act is involving the standards on the basis of which professionals have to make sure the
well bing of service user. According to this legislation service user should be supported with the
appropriate service, before the situation become more serious. On the basis of the valuing
people plan and care Act, in the process of interventions and care plan implementation that is
very Important to manage the care practices, task and therapeutic relationship with the

application of appropriate skills. As the social acre worker I listen service user and other staff
member of the multidisciplinary care group, in order to assure the accomplishment of each
interventions and treatment for the achievement of selected goals. That is very important to give
respect to the service user choice and provide emotional support with appropriate skills, so that
as social care worker I will make sure the application of good communication skill, attention,
calmness to manage the Ola. I have to work effectively for the care of service user and
achievement of the care plan outcome and for this I have to use the critical thinking skills to
manage the each intervention and care process of Ola within the right time. I use the team work
organisation skills to achieve the goal collaboratively with the participation of multidisciplinary
staff. Within the each intervention or treatment session I will make sure that Ola is treated with
respect and dignity, his queries and consider should me answered or manage properly (Bush and
et. al.,2018). I will also make sure that risk associated with interventions is can be prevented with
the appropriate measures in order to assure his safety. For the proper management of the care
plan I have to use the documentation skills, so that impact if the care plan and outcomes is can be
evaluated effectively or need of the improvement in care plan is can be analysed within the right
time.
Evaluation – This is the final stage of the care plan, under this evaluation of
implementation of care plan is done, to ensure that goal have been met . This is the type of
clinical review but also involves the consideration of professional development of the nurse and
key leaning process. The selected evaluation criteria for the care plan is identification or analysis
of the key factors of the improvements, such as Ola care plan evolutional is about the analysis of
improvement in his physical movements, motor skills, speaking skills improvement and level of
the social participation improvement. Improving ability of the Ola related to the transportation
use, social participation, speaking and appropriate body movement is can be consider as the
achievement of care plan goal (Nightengale and et. al., 2017).
Benefits or funding assistance
Within in the interventions section of education on of the key session should about the
funding assistance, under which I will assure the development of Ola awareness about the
various funding sources, which is free or valuable without interest rate. I will make sure that he
should get awareness about the free government services for the disables, so that he can get the

help in right time and save the money for personal use and for his family (Taylor, Vasquez, and
Donehower, 2017). Ola parents are now not able to manage their own needs and requirements,
so with the help of funding assistance he can save the money and help his parents too.
REFLECTION
From the above person care plan assessment various, professional skills and knowledge is
developed, below is the brief explanation in the form of reflection, with the application of Gibbs
model.
Description- I worked for the Down-syndrome adult service user, centre care planing
along with the identification of his care and improvement needs, related to his personal goal of
becoming legible to life independently, with appropriate employment and self management
skills. As a social care worker I deed all these task, such as service user care need identification
(improvement in physical activity, speech, skill of social participation and awareness or skills of
using public transport) and care planning (physical therapy, speech therapy, technological
assistance etc.,). With me for the achievement of care plan goals, other members from the
multidisciplinary staff worked effectively. There my responsibility was to coordinate with all
health care professionals related to the care plan interventions, such as I managed the speech
therapist, physical therapist, technical assistance and other social care worker who manage the
free accommodation and funding for the disables. Through the help of all these practices I
worked to assure the Ola independent living and appropriate accommodation, with appropriate
employment, where he can supported with technical assistance to perform his work role.
Feelings- During the that process of Ola care planing and implementation, I feel that for
the achievement of selected care goal that is very important to manage the participative work
between service user and other workers. That was the very valuable assessment or service user
care practice, through which be help the service user to live independent and quality of life. I am
feeling satisfied that I make the selection of right interventions for the accomplishment of Ola
personal goal.
Evaluation- On of the most effective thing which I have learn from that experience is
the application of appropriate service user care planning framework (APIE), along with the
consideration of several care models (valuing people plan, individual and social model of

disability) in that (Verkuyl and et. al., 2017). Another good thing was the effective participation
with other staff member's and Ola to achieve his care and health improvement goal.
Analysis- From that specify Ola centre care plan assessment experience, I have learn or
understand that every one can live the quality of life no matter there are born disable, because
with proper care and interventions people can improve their quality of life. From that experience
I learn that in most of situation or health condition the key care intervention, should be physical
activities, healthy diet and education.
Action plan- From my experience of Ola holistic care planning, I have learn that,
service user need support in different aspect such as social, emotional, physical and personal. In
order to become more better in that I will use this experience and work in further more practices
like that to add more knowledge and experience in my professional life.
CONCLUSION
From the above study it has been concluded that Down-syndrome is the genetic disorder,
due to which individual face the physical and mental health challenges. In order to make the
Down-syndrome adult service user eligible to live independent, that is the responsibility of the
social care workers to identify service user care needs for the achievement of their personal
goals. For the effective care planing social worker like me should use the appropriate framework
like APIE, along with persona canter care, planing, care act, social model of disability etc. For
the Ola care plan, identified needs are related to the physical activities, speech, motor skills
improvement etc., so for these needs, key interventions are speech therapy, cognitive therapy,
physical therapy etc. from the care plan implementation I have learn the importance of
communication, partnership, documentation, skills, along with the process of care plan
evaluation, through which achievement of the outcomes and identification of the care plan
improvement need is can be done.
