INTRODUCTION
In the health and social care sector, professional and health workers responsibility is to
meet the needs of on the basis of their health conditions. Social worker role is to restore balance
in an individuals, family and social life (Mengoni and Redman, 2019). They work to help the
individual to maintain health and recover form complicated health condition. Social workers also
helps in the development if individual abilities to make then independent. Down’s syndrome is
the genetic disorder, this effects the physical appearance as well as the ability to learn and
develop mentally. This genetic disorder is caused by the extra inherit chromosome in infant or
baby. In this report care needs of the 30 year old Down’s syndrome service user is assessed for
the development of care plan. Here I am working as the social care worker to build the care
plan for servcie user. This care plan is based on the achievement of service user goal, to become
independent and achieve improvement in life. Reflective analysis of self professional skills are
done in this report in context to the care and achievement of service user goal.
MAIN BODY
From the given case study of the service user, it has been identified that, service user Ola
Adeyemi is the 30 year old man with Down's syndrome. service user has lived his life with his
parents with proper support and care, service user is dependent on his parents for some common
daily life activities, like dressing. service user is not able to perform the social activities, such as
cooking, travailing, job etc. but now due to the frail parents, he has to become independent.
Down’s syndrome
This is the genetic disorder due to this, service user face the several physical, mental
health complication and disabilities. When the child born with the extra copy of 21st
chromosome, that health condition of the child is consider as the down syndrome, which further
lead to the physical and mental disabilities or delays in the development. There are the multiple
physical and mental health complication face by the adults in that situation due to which they
need, support of other in their daily life get dependent on others. Depression, obsessive
compulsive disorder, abuse, hearing loss, spinal cord compression, Periodontal disease are some
common complication associated with that genetic disorder. Social challenges face by the service
user are social withdrawal, loss of interest, regression in cognitive and social skills (McKibben,
2019).

In order to assess the Ola's needs I will take the help of APIE model. This is the
appropriate model of the problem assessment, problem solving and care planing. This model is
communally use in the nursing care practices. Below is the identification of the service user need
with the aid of APIE component.
Assessment
This is the initial step of the APIE, according to this step systematic integration of the
quantitative and qualitative data should be done. Under the quantitative data physiological status
of the servcie user, such as the routine observation of the temperature, weight, blood pressure,
pulse is done. Under the qualitative aspects of the service user ideas, exceptions and concerns is
develop.
For the evaluation Ola's care needs systematic integration of quantitative and qualitative
data required to done. There is the need of proper physical health assessment of the Ola's, such
as measurement of the weight, temperature and pulses. service user strengths, preferences,
aspiration and personal goals assessment is required in order to identify the final care needs of
the service user. For the Ola's needs assessment it has been identified that weight is at normal
category, pulse are normal, his strength is education until the age of 18, another strength of the
Ola's is his interest and self concern about the improvement of health condition to become
independent. He require the physical support for the daily life activities. There is the need of
service user education regarding self care , so that he can achieve his goal to move from home to
more independent living arrangements. There is the need to assist the Ola's with the use of public
transportation and healthy diet, in order to avoid some harmful food component. Ola's need is
appropriate accommodation, daytime activities, employment. There is also the need of speech
and motor skills improvement (Hill, 2018).
Planning
Immediate after the systematic assessment of the service user health condition and care
needs, net step is effective care planing. On the basis of this step formulation of the suitable
interventions and strategies should be done in order to address the presenting problem. Plan
should be focused on service user preference, biological elements, that is also important to assure
