INTRODUCTION
This assessment report is considering the metal health assessment of Mr. Tom, involving
the analysis and gathering of patient present condition and problems along with the assessment
of his social and psychological symptoms and behaviour related to mental and physical health
well-being with the help of health improvement tools and techniques (Carroll, and et. al., 2021).
Then this case will concluded with the formulation of symptoms, acute care need and treatment.
Case study
History- Tom is the 60 year old man, he has been providing with mental health care from
last two years. He stay in his house with his wife and daughter and take home nursing care. Tom
has mental health is serious it has been diagnosed that, he is having paranoid illness. This is the
mental illness at which person start thinking odd and eccentric, in this situation person start
unrelenting himself from others, do not trust on others possess suspicious behaviour for others.
Situation is can be normal and acute in this case of illness. From last 2 years tom behaviour was
abnormal he was get afraid from others and do not trust on other, feel uncomfortable at public
place with misunderstanding of getting harm by others, he has the history of family mental
illness.
But now his situation is serious and need acute care because recently he was found to
harming himself. We have did some metal health assessment and physical health along with the
study of psychosocial health in order to identify seriousness of the situation along with his
vulnerabilities and strengths. So GP doctor have analysed the seriousness of tom situation and
referred him to the psychiatrist for further diagnoses.
Physical examination
Some conmen medical test have been done such as urine, blood and brain scan. It has
been analysed that synaptic marker level of the tom brain is low in comparison to the healthy
brain, so there is the further requirement of mental health diagnosis such as actigraphy
assessment (Freeman, Freeman and Garety, 2016). Body mas index of the patient is calculated
and it has been analysed that BMI is healthy and dental, physical hygiene is good. So this
healthy body mass index and hygiene is the strength of patient physical conditions From blood
test level of hormonal changes have been found, such as changes in the level of Oestrogen.
Mental health examination

Mental evaluation is done in order to identify the metal activities and thinking of the
patient, this assessment is done by asking some question to the patient and result is concluded
from the answer of patient (Harpin, 2018). So in this case basic questions about thoughts,
behaviour and feeling is asked to the tom and then it has been found that his behaviour is not
normal, he feel some hallucination that some one is try to hart him and in order protect himself
he do some abnormal activities which hart him by his own. Some time he feel insecurity from
other and misunderstanding that people are trying to heart him so that's why he perform some
self-defence action which can hart others too. His physical health is not too bad but there is the
wounds caused by himself to his body. Vulnerabilities of tom psychosocial health is that his dad
habit's such as alcohol consumption and introvert nature which contribute to his mental illness.
His inappropriate belief that he can not be safe between the peoples. Tom strength is that he
wants to improve his condition and trust that he can get well and he want to be social from
enjoying his life. Tom conditions was manage with regular injection of anti-psychotic
medication but know he needs the acute care as it is the situation where he can harm himself and
other.
Formulation-From the mental and physical health assessment various abnormal
symptoms and problem is analysed and these factors are directly dedicates the requirement of
acute care for Tom. So that major problem in this case which indicates the requirement of acute
mental care is Tom fragile and stress full behaviour which can make major harm to himself and
his family. His self and other harming behaviour is the symptoms of acute mental illness. Where
he is not mentally able to identify right and wrong action so that it can be worst for tom and
others too. His acute abnormal thinking for others that they can hart him is the serious symptom
of acute mental illness which can result the sever condition for tom other peoples (Horwitz,
2020) . Symptoms Self harming and others harming behaviour, Hypertensive to the criticism,
Social isolation, Thinking and believe that other can harm all these factors are the symptom of
cute paranoid illness, which show the need of acute care.
Clinical features- So from the above analysis it has been formulated that patient is
having the various complains of the acute mental illness, which has to be cure with the acute
inpatient care, in order to reduce and control the mental health issues. From the mental and
physical health assessment various problems is analysed that relate to the acute care practices,
complaints of the violent attack, dominant symptoms during the time of admission was delusion
