Patient 1: Alex Abraham
Patient’s details
The reason I choose the patient
Nurses have to maintain different values and the beliefs during the treatment of
patients. Attitudes, assumptions and mindset of patient can influence nurses’
behaviours. Decision making in relation and impact of judgement are important for
evaluating patients’ conditions. After observing different case studies the professional
career of a nurse will develop in a better way. Transformational learning is one of the
process of constructive and deep meaningful learning that will help a person not only
theoretically but also by acquisition and support them in practical field. The
assignment will show introspection and the proper analysis of case studies by
improving their own beliefs and promotion of transformational learning (Melrose,
Park, and Perry, 2013).
Alex Abraham is a 42-year-old man who was
diagnosed with type 2 diabetes recently. He
works out of the town for 12 hours shift and has
to stay away from home for two weeks. He is
married and has two children. He has difficulties
to accept the diagnosis.

Mr Alex is chosen as I have experiences in treating
diabetic patients. The present condition of Mr Alex
can be managed with the support of patient’s family
and my knowledge. The self-management of
diabetes can avoid long-term health difficulties but is
often complex and hard to accomplish. Patient’s
autonomy for managing their diabetes is more
effective for self-care and glycaemic control.

Ask him for meal plan and proper
ratios of nutrients will be informed.
Physical exercises will be suggested
for better physical condition and
outcomes.
I will take help of genogram and
ecomap to find the risk factors and
support factors.
I will give him some handouts, which
promote diabetes awareness. I will
educate him about self-management of
diabetes.
The medication regimen and lifestyle
modification routine should be
introduced to the patient (Makuc,
2016).
I will also emphasize on appointments
to review the progress of his health.

Patient 2: Hailey Berry
Patient’s details
The reason I choose the patient

Hailey Berry is a 66-year-old woman,
diagnosed with cervical cancer. She and
her partner of 30 years met oncol ogist.
They want review of pre-operative
checklist with nurse.

I have experiences in oncology unit. I can give support and meet their expectation, as I am able
to understand their physical and psychological condition. I have also experience to treat
homosexual patient. I do not possess any bias towards same sex couples
The patient and her partner are stressed about the diagnosis and
surgery. They might be anxious that they cannot get better care
due to their LGBTQ status and ethical differences.

al.,2015).
