Student Directed Functional Assessment Interview(Centre personnel to conduct interview with student– with parent present)Student: Age:Sex:Date of Interview:Interviewer:Parent Guardian:“We are meeting today to find ways to change the centre activities to find out if you will like itmore. This interview will take about 30 minutes. We can help you best if you answer honestly. You will not be asked anything that might get you into trouble.” **If the student finds it hard to describe the problem(s), ask them to recall a recent example or simply to describe what happened yesterday.1. Using child’s program, have the child point out activity areas/times he/she is having success. I have asked them to recall the activity they have conducted in identifying pictures and colours while reading time from books and other props used in the session. Theyare able to remember the objects they have learnt with the sound that alphabets give through use of puppet. 2. What is it about these subjects/times you like? The children have been able to remember previous activities they have done in learning session. They have developed social skills through classroom and group activates. They also have developed their communication skills both using their home language and English thus inclusion has been an effective approach in a learning session and empowering children. 3. Are there any times/subject areas in your schedule you don’t enjoy?All three children A, B and C have enjoyed the learning session. However child A did not likethe story. 4. What is it about these subject/times you don’t enjoy?The first child ‘A’ has not enjoyed the study that was included in the reading session. However, he has enjoyed the group’s activities and the puppet that was used in the classroom. Child B and C have enjoyed both playing within the group with the objects that were provided. 5. Tell me about times when the staff(educators) speak to you about your behaviour. (If necessary, provide some of the following prompts: paying attention, completing work, talkingin class, coming late, etc.) Child A: Yes my teacher asks me to pay attention in the class, especially when they are teaching me objects names. They do not provide any home work. Child B: Yes she asked me not to talk .Child C:She did not comment on anything. 6. Tell me how things could be done differently to help you enjoy the activities/experiences more? Child A:I would love play with the puppet that tells story.
