INTRODUCTION Integrated care planning can be defined as more responsive care towards patients. In the recent time many people are suffering from mental, physical many issues. Medical professional need to acknowledged their problems soon and have to provide them care properly. Present study is based on case study of Colin who is having challenging behaviour. Current assignment will outline various provision and resources that are helpful in integrating care working. Furthermore, report will describe unmet needs and support of patients. LO 1 P1 local community resources and provision that can help in integrating care working A patient with mental illness needs to get proper care in hospitals so that health condition of person can be improved soon(Glasby, 2017.). Here is the case patient is suffering from challenging behaviour. Currently it is refusing to take medication and his condition ae getting affected badly day by day. It is very essential for the health care providers that to provide them proper care so that his condition can be improved soon. Local community resources that can help the person in getting integrating care are explained as below: General medical professionals: Doctors are responsible for providing their proper care to patients so that health condition of that person can be recovered soon.They need to provide medication to Colin so that he can start controlling over his behaviour. Doctors need to involve them in outdoor activities so that he can divert his mind which will make them feel refresh and his behaviour will be changed. Family care giver: they are another important community resource, as Colin is very close to his mother. Hence his mother must give attention to him and have to spend some time with him so that he feels good. Sometimes mental illness arises die to aloneness, if person has nobody to talk and share their feelings then it turns into irritation which many times make them mental ill person and person starts reacting loud on small things that is what happing with Colin as well(Costa-Font and Greer, 2016.). If his mother gives her care to him then it would be better and his condition will be improved soon. He has two siblings as well but they do not see him often. if his family pays attention on him then it may help in improving his mental health soon. There is various provision which are made by professions to improve condition of person by offering them proper care. 1
Collaborative working provision must be followed in integrated care planning for the patients. There is essential that all the medical professionals, nurses coordinate with each other’s and provide best care to the person who is suffering from disease. In the case Colin diet plan is very poor which has increased his weight, if all the staff members in hospitals share the information and take care patients collaboratively then it would be better in improving his condition soon. Person centred care provision must be followed in hospitals as nobody is paying personal attention on Colin which has affected his mental health(Moss, 2017.). If he would get proper personal attention, then his mental health will be improved soon. P2 Local unmet need Outline local resources and provision that supports integrating care working Health care needs Colin a patient who is suffering from autistic spectrum and face problems while communicating with staff and other people. Due to some health needs which are not fulfilled and affect Colin overall health are Mental health Suffering from autistic spectrum Colin is unable to cope up with changes and fulfil all his needs. Rude behaviour of local caretakers affect overall mental health of Colin(Dickinson and O'Flynn, 2016.). He is to close to her mother and at that situation he is unable to meet her and siblings who are also see him often Colin feels lonely and unable to communicate with any of his family member make him rude and affect his overall mental health. Due to mental illness Colin need to be reminded to use bathroom and assist to wash clothes and dress up. Dietary need In proper diet of Colin affect his overall health. Due to unbalanced diet he increases his weight because at meal time Colin only requests for chips. Loneliness affect overall diet routine of Colin no one is their for care like his mother. Colin need proper and healthy diet to overcome problem of mental health. Care takers need to make him familiarize like a family member. Medication need Person suffering from autistic spectrum can have very high pain. Patient also show unusual response with their pain like laughter, singing. Colin need proper medicines to reduce his pain and maintain health. 2

Let him know that you respect his feelings. CONCLUSION In this report Colin is a patient suffering from Autistic spectrum. Care takers need to communicate with the patient to make them feel important. Keep regular update of their health maintain diet for them for their good health. Proper medication is required to maintain patients health. 3
