Chapter 1 : Introduction1.1ContextofresidentialcareservicesinEnglandResidentialcareservicesareconsideredas those care services that are meanttoprovidelongtermcareandsupporttothepeoplelivinginresidentialcarehomes.Thereareenormousnumberofresidentialhomesavailableforpeoplewhoneedsalongtermcare.Itmayinvolvechildren,adultsetc.,whobecauseofsomereasonsdonotliveintheirhomeandsotheyliveinresidentialcarehomes.Someverywellknownexamplesofthisincludesthepeoplewhoareadealingwithsomemajordiseasesorillness.ThereportfocusesonunderstandinghowcaremanagersprovidesupporttothesocialcareworkerssothattheycanbeabletomakeimprovementsinthequalityofcareandtreatmentthattheyprovidetothepeopledealingwithDementiaintheresidentialcarehomes.TheCareStandardsAct2000definesacarehomeas“anyhomewhichprovidesanaccommodationandpersonalcareforthemembersofthecommunitywhoisorhasbeenill(includingmentaldisorder),isdisabledorwhohasapastorpresentdependenceonfactors such asdrugsorAlcohol”.Acarehomeprovidessupporttopatientswithaviewtoenablethepatientstoliveanindependentlifeoftheirchoice.Hence,aresidentialcarehomeemployssocialcareworkerswhosupportpatientstomeettheirdailylivingactivitiesthatincludepersonalhygiene,medicationmanagement,mobilisingsafelywithintheirownenvironment,managingincontinenceneeds,accesstolocalcommunities,managingnutritionandhydrationneeds.SomeresidentialcarehomeshavebeenregisteredinEnglandand their forte istomeetaspecificcareneedsuchasdementia.Residentialcarehomessupportingpeoplewithdementiasymptomshavebeenarecentkeyfocusonpolicydevelopment,promptinggovernmenttosetoutavisionforreforminthequalityofcarewiththeaimofprovidingpersoncentredcarethatpromotesandensuresindividualneedsforwellbeing,independenceanddignity.AspertheCareAct2014,itfocusesonthewaysofmakingsomesuitableandrelevantmodificationsinthesocialcarelaw.Itmeansinvestigatingabouttheoneswhoareintheneedofcareandsupport.Also,itmakesasuitableassessmentthatcanhelpinresolvingsomeofthefactorsandafterthatitcantesttheimpactofthesenewaccessedfactorsonthepersonwhetheritishavingapositiveimpactornot.1
ThecurrentfundingforresidentialcareinEnglandiscomplex,hasvariationinwhatpeopleareentitledtoandseemfinanciallyunsustainabletocopewiththedemandsoftheelderlypeopleespeciallythosepresentingwithdementiasymptoms.TherearevariousmeansoffundingthatisprovidedforthedementiapatientsinUK.AspertheNHSfundingcarefornursing,allthepatientswhoaredealingwithdementiaandareresidingintheresidentialhomesaresupposedtogetsomeamountfortheirtreatmentprocedure.ItisconsideredasaveryvaluableinitiativebythegovernmentofUKbecauseitisobviousthatthesepeopleliveatresidentialcarehomes,sotheydonothaveafixedsetofincome.So,itisobviousthattheyarenotcapableenoughofprovidingthemanefficientrangeofcareandtreatment.Thispromptedthegovernment’swhitepaperwhichwaspublishedinMarch2010withtheaimofbuildingtheNationalCareServiceplantoreformtheexistingresidentialcaresystemfortheelderlypeople.Thisplanensuresafairfundingsystem,freeatthepointofuse,ensuresindividualchoice,andsupportsfamily,socialcareworkersandthewidercommunity.TheframeworkforresidentialcareinEnglandprovidesanimportantbackgroundforsocialcarebutisunderfinancialpressureduetorisingexpectationsfromthewidercommunity,increaseinthenumberoftheageingpopulation,newregulatoryapproachestablishedbytheCareQualityCommission(CQC),limitedfundingsupportandanincreaseinmediacoverageaboutpoorperformanceandqualityofcare.FollowingasurveyconductedbyRoyalCollegeofNursinginJanuary2017,asnapshotoftheprevailingsituationaboutqualityofcareinEngland’sresidentialcarehomesectorspecificallyfocusingonstaffingandworkplaceissueswaspublished(BritishMedicalAssociation,2017).Anumberofconcernswereemergedfromthissurveyanditwasrecordedthatanumberpatientswerebeingadmittedinresidentialcarewithhigherlevelofdependencyandcomplexsocialcareneeds.Thiswasaseriouschallengetocaremanagerswhowererequiredtosupportsocialcareworkerstoimprovequalityofcareamidstlimitedfundingandsupportfromrespectivelocalauthoritiesamidstrecruitingcompetentstaff.Theaveragestaffingratioinresidentialcarehomeswasonestaffto4patientsemployedtoprovidecareandsupporttochallengingpatientsdiagnosedwithdementiasymptoms.Thereisneedforurgentreformandthisresearchaimsatidentifyingway2
sinwhichcaremanagerscaneffectivelysupportsocialcareworkerstoimprovethequalityofcareinresidentialcarehomesamidsttheprevailingcircumstances.AccordingtothereportfromthedepartmentofHealth(2017),thesocialcarebudgetsurveyindicatesthattheproposednationallivingwage(NLW)whichcameintoeffectinApril2016ishavingasignificantimpactonthecostofresidentialcareprovidedtotheelderlypeopleincarehomes.MuchofthesocialcareworkforceislowpaidandwillbenefitfromtheNationalLivingWage(NLW)whichissetat£7.20anhourthisyearandisboundtoriseto£9anhourby2020.Itisestimatedthatitwillcostlocalauthoritiesapproximately£151millionplusatleast£227.5millioninimplementationandassociatedcostsin2017/18.Thiswillaffectbothdirectcouncilcostsandincreasedproviderfees.Thenationallivingwagewouldbeabletohelpmorethan7millionlowpaidworkersbutthiswouldincreasethecostsofoperatingresidentialcarehomesinEngland.AimToanalysetheeffectivenessofcaremanagersinprovidingsupporttothesocialcareworkersinordertoimprovethequalityofcaretheyprovideinresidentialcarehomestothepeopledealingwithdementia.1.2TheobjectivesofthedissertationTo identify the challenges faced by staff in residential care homes caring for people withdementia especially those with very high needs.To identify the support required by social care workers to enable them improve their job. To identify how Care home managers can address the needs of staff, improving the performance of the home and individuals.To critically review the policies of NHS England with respect to staff development andtraining of social care workers in residential care homes dealing with dementia patients. 1.3Overviewofthestructureofthisreport3
ChapteronegivesageneralintroductionaboutthecontextofresidentialcareinEngland.Itindicatesthegrowingfinancialpressuresexperiencedbyresidentialcarehomesamidstanincreaseinthenumberofolderpersonswhohavecomplexsocialcareneedsandrequiresocialcarefunding.ItoutlinesthecurrentchallengesfacedbyCaremanagerstoensurecontinueddeliveryofqualitycareinresidentialcarehomeswithlimitedfundingfromrespectivelocalauthorities.Thischapterprovidesthewholeanalysisofwhatisbeingdoneandimplementedinthewholereport.Itinvolvesthefactorsthatthecareworkersleadstoprovidesomesortofsupporttothesocialcareworkerssothattheycanbeabletoprovideabetterqualityandstandardtreatmenttothepatientsdealingwithdementiaandareresidingintheresidentialcarehomes.ChapterTwoconsiderstheliteratureandexaminestheconcepts,keyelements,perspectivesandsupportingtheories.Onthebasisofthebasicaimthatistobeachievedintheresearch,variousobjectiveswillbemadeandafurtherdiscussiononthesamewilltakeplaceinordertoachievetheminanefficientandappropriatemanner.ChapterThreedescribesthedifferentresearchmethodologiesandexplainshowIwentaboutresearchingthetopic.Differentaspectsoftheresearchmethodologyhavebeenachievedaswellsuchastheresearchapproach,designetc.Also,primaryresearchhasbeenusedinthefollowingresearchbecauseitcanhelpinanalysingtheeffectivenessofcareworkersbetteronprovidingsupporttothesocialcareworkersinordertomakeimprovementsinthequalityofcaretheyprovidetothepatientsofdementiawhoareresidingintheresidentialcarehomes.ChapterFourconsidersthevariousfindings.Theseareeachbroughtintocontextwiththeliteraturereview,theoriesandpreviousreadings.Italsoexpandsontheresearchobjectives.ChapterFiveprovidestheconclusionofthewholereport.Itincludesdifferentwaysthatcanbeimplementedbythesocialworkerstoprovideanefficientandqualitytreatmenttothedementiapatientswhoareresidinginresidentialcarehomes.Also,itinvolvestheimpactofdifferentlegislation'sthatthrowsapositiveimpactonthedementiapatientsintheresidentialcarehomes.Achievementofallthefactorandoperationofeverysinglesectionwillbeincludedinthesame.4
