Part 1:
The leading cause of death for men, women and people of most racial and ethnic
backgrounds in the United States is heart disease. In the United States, one person dies of
cardiovascular disease every 37 seconds. Every year nearly 647,000 people die of heart
illness, that's one out of four deaths. Between 2014 and 2015, heart disease costs the US around
€ 219 billion annually, covering expenses for treatment, medicinal drugs and lack in survival
efficiency (cdc.gov, 2020). Cardiovascular Diseases (CVDs) are a category of diseases involving
heart and blood vessels which include, acute coronary heart syndrome (ACS), peripheral arterial
disease, congenital heart disease, cerebrovascular disease, rheumatic heart disease and deep vein
thrombosis and pulmonary embolism. Strokes and heart attacks are acute complications caused
as a result of blockage preventing the blood from flowing to the brain or heart. Thus, there is an
urgent need for developing a health promotion plan that aims at promoting and improving health
quality of the individuals in the community by implementing appropriate strategies to reduce the
prevalence of heart diseases and other comorbidities. Adults have now been well-established to
build cardiovascular (CVD) risk factors. Yet new research emphasizes the importance of CVD
developments in the course of life, starting as early as the childbirth. As a consequence,
initiatives in early years can be a vital part of the global effort to reduce the CVD burden.
It has been reported that heart diseases is highly prevalent among adults aged 65 years
and above in addition to other co morbidities due to various reasons which will further increase
in coming years (Bell & Saraf, 2016). Thus, the target population of this health promotion plan is
older adults who are above 65 of years.

Prior to commencement of the health promotion program it is important to identify a
specific health issue in order to develop intervention strategies addressing those health concern
targeting a specific population. Thus, considering the incidence and likelihood of the chosen
target population, health promotion plan must be developed and implemented in order to get an
improved outcome. Older people are at an increasing risk of heart related conditions and other
age related comorbidities. The increasing rate of hospitalization is seen among this population
due to an increased disease burden including both chronic and acute conditions, which calls for
an urgent need to develop and implement health promotion strategies for them addressing those
concerns and increase the overall quality of life.
The strategies and approaches for health promotion in a particular population in the
community:
Healthy population:
Promoting healthy lifestyle.
Preventing risk factors.
Prevention of other comorbidities.
Minimizing the risk of other health complications.
Population with risk factors and/or experiencing symptoms:
Health promotion and screening.
Encouraging them in maintaining a healthy lifestyle.
Creating awareness among them about the importance of implementing self-
management strategies.
Limiting disability (Kumar & Preetha, 2012).
