Introduce the patient
The patient about whom the nursing care plan is devised is Mr Christopher Collins.
He is a 54-year-old male who is suffering from osteoarthritis which is significantly affecting
his mobility and causing extreme discomfort. It was decided by the physicians that surgery
will be conducted to relieve the patient of the pressure in the left and help him regain the
mobility efficiently. However, after the surgery has been conducted, he was discharged after
two days with some medication for pain relief with a crutch for assistance for walking and a
supportive knee brace (Strickland et al. 2018). There was no post-operative care carried out
which would ensure proper treatment and recovery from the operation. He has been an avid
sufferer of asthma since his childhood and is on Seretide accuhaler (Wayne et al. 2019).
It is important for nursing professionals to assess the condition of the patient when
they are being re-admitted because of the complications (Seo et al., 2016). The area where the
operation has been conducted, it was observed that there are sutures which have become red
and swollen and can discharge pus any time (Laidlaw, Diduch and Werner 2019). Hence, it
can be understood that a proper assessment regarding the positive health outcome for the
patient should be part of the nursing care plan in accordance to the identification of the health
problems that are probable in the health condition of Mr Collins (Doenges, Moorhouse and
Murr 2019). Thus, the report will be including the identification of the health problems that
are prevalent in the patient considering his medical condition. In addition, the report will also
highlight the interventions that could be devised by the nurse such that a positive health
outcome is ensured for the patient.
Assessment
The assessment of the patient information that is shared is to be analysed by the
subjective and objective data. When the patient is being admitted to the hospital, it is the duty

of the nurse attending to ask certain questions and conduct some tests such that the medical
condition of the patient is understood. The subjective data in the case of Mr Collins is the
complaining of discomfort and inability to mobilize him because of osteoarthritis in the left
knee (Willy et al. 2019). There has been constant knee pain for which the physician had
recommended for left high tibia knee osteotomy. After the operation has been conducted, it is
observed that the condition of the patient deteriorated further and caused the wound to gather
pus and not recover as was expected. In addition, during the admission process, he told the
nurse that he was experiencing nausea and also felt shaky and shivery (Samieirad et al. 2018).
The objective data that are to be retrieved from the patient define the lab results that
are conducted on the patient while he is being admitted to the hospital. The lab data obtained
in the case of Mr Collins is that his blood pressure 98/57, body temperature around 38.3 oC,
white blood count is 18.4 and the platelet count is 276. It is after this that the partial carbon-
dioxide and oxygen concentration is measured. The partial carbon-dioxide concentration is 21
and the partial oxygen concentration is 80. The Glasgow Coma Scale was conducted on the
patient and received a score of 15 (Reith et al. 2016). The left knee of the patient experienced
pain for which the PERL pain assessment was conducted. The score that was obtained by the
patient received 5 out of 10 on their scale. The urine output of the patient was measured over
a stipulated period of time and was observed to be normal. The urea level is 5.4 while the
sodium level is 141 and the potassium level is 4.2. The glucose level in the urine output was
assessed to be 5.1 which deemed under the normal range. Ventolin is a drug that opens up the
medium and enormous aviation routes in the lungs (Matera et al. 2018). It is utilized to treat
asthma, including asthma assaults. Beginning of activity of the breathed in form is ordinarily
inside 15 minutes and goes on for two to six hours (Anderson et al. 2018). Basic reactions
incorporate insecurity, cerebral pain, quick pulse, unsteadiness, and feeling on edge. Genuine
