Transport option Discount/concession and process of

Self-powered This service provides people with disability
with the right to travel freely on the roads and
make the roads and transport system
convenient for them. Further, it provides them
with the complete details of the routes so that
safety could be reinforced. To assess them, the
person with disability benefit card or travel and
transport card for disabled person so that they
can utilize the benefits provided to them
through concession (TransportNSW 2019).
Car Local government has increased the number of
cars for people with disability so that pooling
of car could be increased. Further, the
government has implemented several schemes
such as taxi users subsidy scheme, ambulance
fee, parking permits, Australian disability
parking scheme and electricity subsidy scheme
so that people traveling through private or pool
cars could be provided with maximum benefits
and security on roads. Further, to avail the
benefit, the person should possess a disability
identification card from the local government
so that these schemes could be used for
transportation related benefits of the person
(TransportNSW 2019).
Taxi Public taxis and cabs are one of the most
important travelling aid available in the state of
New south wales and hence, disability benefit
schemes has been implemented in the state
extensively. The government has provided
schemes such as subsidy for taxi users, subsidy
in fare of such taxis and no refusal schemes so
that discrimination, disparity could be
prevented on the roads for the benefit of the
person with disability. Further, all the taxi
drivers are asked to attend training sessions so
that while serving for such passengers they
could help them with their support to ease the
travel for the person. to avail these facilities,

disability identification card, disability
concession card and so on.
Provider bus This transport is one of the most convenient
and easy way of transport for the people with
disability due to its easier facilities, availability
and provides a timely service to the

the government has provided several sessions
and schemes so that disabled people can use
these transport facilities freely. Disabled
concession cards, disabled identification cards
and are required to avail these services
(TransportNSW 2019).
Public transport Other public transports such as water boats,
water buses, cabs, metro, and others provide
easy and cheaper transport facilities to the
people disability. to access this, the person

Wheelchair facility This facility will help people with disability to
understand the route maps of the state and
travel freely without any risk of security. to
avail such transport concessions, the disabled
person should provide concession cards

Project 2: Community integration plan
Introduction
Strengthening in inclusion process of people with physical or mental disability,
retardation, as well as functional and developmental disability is an important and ongoing
process so that different people with different abilities could be brought together for a
progressive society. It is the duty of the government and the society to include such people so
that their self-esteemed, their confidence and their individuality could be reinforced and they
could be provided with equal treatment within the society. In this section, case study of a young
adult Phillip has been discussed who is suffering from acquired brain injury due to a past
accident. Further, he is affected with moderate disability and has faced the phase of social
isolation while receiving care in the healthcare facility. However, he is keen to take part in the
community and its activities again and hence, a community inclusion plan for Phillip should be
produced so that all the barriers present in the path of his inclusion within the community could
be removed and despite of disability, he could become an important part of the society.
Strategies to include Phillip in the community
The strategies which could be used within the community integration plan to include
Phillip within the community will revolve around the following sections:
Including Phillip and his family in the integration plan so that they can actively
take part in the planning process and provide their feedback to it.

Integration of the person and his family
Family is an important aspect depending on which a person and his personality develops
over the course of years. Phillip was a sport science student, who used to communicate with
everyone within his university. Before accident he used to take active participation in social
activities (Metzelthin et al. 2013). However, after the social exclusion he faced while his
treatment, affected his mental strength. Hence, in this situation, his family should be provided the
charge of social inclusion strategies and planning so that they can implement the activities in the
daily life of Phillip. However, the degree of the family involvement should be discussed so that
they can understand their limits and activities as per their interest of work (Aviram, Ginath and
Roe 2012). In this course the family members and Philip will be provided with educational and
training sessions so that reinforcement of social inclusion and advocacy for policy related to such
inclusion could be strengthened.

Strength based approach
It has been observed through the case study of Phillip that, despite of the physical and,
mental disabilities and difficulties, Phillip has the ability to collect his self-confidence and seek
inclusion in the social activities (Domin and Butterworth 2013). Therefore, the inclusion plan of
Phillip within the society should revolve around the strengths of his personality such as courage,
self-confidence, self-esteemed so that he can use his strengths while dealing with barriers and
difficulties of his situation (World Health Organization 2015). This will be ensured by arranging
a mentoring program for Phillip in which the a volunteer group will provide additional support to
Philip so that he can understand the rights, legislation and policies which strengthens his
existence and inclusion within the society and the best possible ways using which he can take
advantage of the policies to be active and available within the society (Taylor and Morrissey
2012). Further a buddy” will be provided to Phillip who will be able to understand the situations
and actions in which Phillip requires assistance or support and then create a developmental plan
to increase his functionality.
Taking lifespan development into account
It is evident from the case study of Phillip that his disability was acquired after he met an
accident, hence, there is a probability that while facing the society again or while taking active
part, he will become stigmatized or cautious because of his health condition. Hence, in this
aspect, he will be provided with lifespan development strategies so that through the course of his
growth and development, he can identify strengths of his personality and utilize them in his daily
life. in this course, he will be provided with sessions where he will be educated about the
accommodating facilities and travelling equipment which could increase his participation in the

community and integrate his within the society on a community level. This will both encourage
them and provide them with strength for their community inclusion program.
Active support which will be provided to Phillip
Philip will be provided with influential stories where such integration plan was provided
to include a subject in the society and it was achieved successfully. It was an important part of
the community integration plan as it would help Phillip to collect his strengths and apply the
strategies in his life so that he can be provided with his right within the society.
Including social model of disability
Social model of disability determines the different approach of viewing the world with
their disability and their focus of controlling life on their own. This helps to strengthen their
thoughts and then implement the strategies provided to him in his daily life to attain the social
activities for his inclusion (Oliver 2013).

It is important to note here that in order to provide holistic care to Phillip, the care
process would require the collaborative effort of care professionals such as physiotherapist,
occupation therapist as well as physician. The role of each of the care professionals would be
described below along with a supporting rationale that would justify the need to include the care
professionals in the inclusive program. Firstly, the physiotherapist would provide assistance to
Phillip by helping him with mobilisation and also train him with energy saving exercise. The
occupational therapist would train Phillip to perform activities of daily living with convenience.

The physician on the other hand would keep a track on the health outcome of Phillip and
continuously undertake steps to ensure progress of the health outcome. \
Aid, device, stigmas and legislations will be implemented in the process for inclusion:
The aid that would be provided to Phillip would include wheel chair and crutches that
would help Phillip to walk. Further, it would also comprise of providing Phillip with disabled
shoes for men, which are uniquely designed to offer comfort to disabled people who have
difficulty in walking or in mobilizing (Shakespeare 2013).
The device on the other hand would comprise of equipping Phillip with a fitness tracker
which would essentially track the water intake of Phillip. This would help in maintaining a fluid
balance in Phillip.
The most prominent stigma that Phillip would encounter would majorly comprise of his
fear of being judged by people. As a disabled, Phillip might get ridiculed or shown excessive
sympathy which would both make him feel bad. Therefore, equipping Phillip with self-
management technique would help in elevating his confidence level and would make him feel
better.
The Disability Care Act offers a wide range of support services for disabled children like
Phillip. Through the act, Phillip could avail disability services such as speech therapy and also
assistance payment in order to experience a smooth recovery (Raising Children Network 2019).
Review and evaluation:
After the successful implementation of this community integration plan, a review and
evaluation procedure will be conducted. Review will be performed to analyse how well Phillip

has been integrated to the society and evaluation will be conducted to the judge this plans
efficacy and whether any modification is needed. The evaluation procedure will be conducted in
three phase. First phase will be conducted at the beginning of this plan’s implementation, second
phase will be conducted after three month of completion of this plan and final and third phase
will be conducted at six months after the completion of this plan. The evaluation will be
performed in a qualitative manner and will be based on pre- determined open ended
questionnaire. These questionnaire will be asked to Phillip, his immediate family members and
his care giving team of professionals. The response of these questionnaire will be analysed to
determine whether this plan had desired effect on Phillip or any further modification or
amendment is needed in the plan (Sharma et al. 2017).
Assessing risk management:
The target subject of this community integration plan is Phillip and he has a lot of
risk of falling due his moderately impaired brain function as well as cognitive disco- ordination.
Owing to these reason patient has a lot of chance of falling. Furthermore, the subject might be
misinterpreted due to his moderate speech impairment. The subject might be face discriminating
behaviour due the subject’s physical and mental condition. In order to assess the risk for Phillip,
these factors need to consider while assessing risk management. The risk assessment will be
performed by quantitative method. This risk assessment process will be conducted in two steps:
an initial assessment before the implementation of the plan and final assessment after the
completion of the plan. This process will be conducted by quantitatively measuring the falling
and discriminating incidents the subject has faced before the implementation of and after the
completion of the plan. The collected data will analysed to assess the risk management of this
community integration plan (Downes et al. 2016).
