Introduction
The performance outcomes are measured through the effective performance measurement
tool. The tool is basically used by the Healthcare Effectiveness Data and Information Set and
there were many domains that take care of patient care (Ho and Antonucci, 2015). The
productivity of the patient can be maintained through the fee for practice services. In this report,
three different patient interventions will be mentioned and the measurement tool will be
evaluated to measure the clinical practice.
Three patient interventions
The first intervention relates to the vaccines which are targeted to the patient when any
infectious things happen to the patients. The protection against the agent is acquired through
these interventions as the single dose of vaccines is effective. This intervention is cost-effective
and also induces the immune system. Second patient intervention relates to the neonatal and
maternal interventions (Asano and Finlayson, 2015). The proper care is required during the time
of the pregnancy so this intervention helps in neonatal and maternal mortality and also provides
effective treatment to the pregnant woman.
The neonate is directed to care about the practices of breastfeeding so the health system
of the women can be maintained. The third intervention relates to environmental alterations as
there are many infections are caused due to changes in the environment (Nallamothu, et al.,
2014). There are many transmitted diseases such as the fecal-oral, cholera, etc. which is caused
by water infections. Environmental interventions help in supplying clean water, make provisions
related to the sewage system and also protect food storage (Hahn, et al., 2014).
Measurement tool to track patient outcomes
The patient outcomes can be tracked through the patient experience. The outcomes of the
patient can be measured through the Patient-reported outcome measures as this does the
perception of the health care and give effective services to the patient to improve their
performance (Bower, et al., 2014). This tool is used for the balance metric so that improve
patient care can be measured and the effective outcomes can be evaluated. The most realistic

gauge of patient satisfaction can be ascertained and issues can be responded effectively. The
clinicians and the health catalyst of the patient are measured so that their current status can be
ascertained. Effective decisions can be taken through, measuring the patient outcomes like the
current status of the patient is evaluated. The tool helps in predicting the patient outcomes for the
current time and from the particular intervention the patients get many benefited. The healthcare
issues are sorted through the measurement tool and interventions help in preventing the disease
(Peek, et al., 2016).
Conclusion
The major three interventions for the patients are vaccines, maternal interventions, and
environmental alterations. Theses intervention protects the patients from the disease and takes
care of them at the time of infections, delivery or pregnancy and at the time of environmental
disease. Patient experience is an effective measurement tool for measuring the outcomes of the
patient and it helps in evaluating the current status of the patient.
