Workgroup Profile:
In 2003, Emerald Aged Care, the premium and modernised agent community,
was formed in the Northern suburbs. A resident of this community has few options. They can
choose a one or two-bedroom apartment. Alternatively, they can prefer to remain self-reliant
or have foods or housework services from the Emerald Aged Care. A resident of this
community can access the swimming pool, beauty salon, gymnasium, cinema, and games
room. Emerald Aged Care provides cooking classes, shuttle services for the local place which
are popular, and woodworking workshop to the residents of this community. 24-hour
emergency monitoring and reception facilities are available in this community.
Emerald Aged Care employs 150 individuals. Human resource general manager of
Emerald Aged Care leads the small human resource team of this company. The present CEO,
who is the founder of Emerald Aged Care, has experience of forty years in healthcare
administration. The CEO of this group had the experience as executive roles in metropolitan
health services and regional health services.
To fulfil the growing interest among the people of this community for medical and
allied health support services, Emerald Aged Care started providing this service. At first, this
organisation contract with external providers to visit the community to provide health support
service to the residents of the community every week. To expand the business and achieve
the organisational goal, this workgroup expands its business. This organisation has developed
a small-sized on-site pharmaceutical centre. In the medical centre, there is a general doctor,
three part-time nurses, a psychologist, a physiotherapist and an office manager. For this
centre, this workgroup employs every relevant employee and conducts induction for the
employees.

Problem statement:
There are several performance issues which create an impact on the client. In
the community, three nurses are employed as part-time in partially overlapping shifts. These
three nurses have limited nursing work experience. These three nurses have limited direct
nursing work experience with elderly patients. In this medical centre off Emerald Aged Care,
there is limited interaction between the medical centre staff and several other staff of
Emerald. Poor understanding of the performance review procedure is followed in this
medical centre of Emerald. The Emerald Aged Care need to be flexible for the career
improvement course. To conduct the formal training outside working hours, there is a
minimal time. However, the present learning and development programme is not sufficient
for nurses to treat aged people from the community. HR officer of this business group has to
improve the learning and improvement program of Emerald Aged Care. The learning and
improvement program will help the nurses to build professional development, career
improvement and leadership skills. For the learning and improvement program, most of the
resources, which will be used, are available through the organisational LMS (Yoo and Park
2014). The learning and improvement program will aid those three nurses to acquire few
necessary skills, which are fall assessment and care plan, dementia assessment and care plan,
delirium assessment plan and care plan, and pain assessment and care plan to handle aged
people.

The training and improvement program will aid these three nurses of Emerald Aged
Care to learn fall assessment and care planning, dementia assessment and care plan and
delirium assessment and care plan.

Fall assessment and care plan:
In this learning and development program, which will continue for one year, nurses
will acquire knowledge about falls assessment and care plan. Nurses will evaluate frequent
falls risk factors, which include weakness of leg muscle, postural hypotension, balance
problems, and acute confusion. Falls are frequent at all ages. However, mainly the aged
people faced unintentional injury and death because of falls. Nurses usually communicate
with the patient at first. Improving the quality of nurses through false assessment and care
plan can be beneficial for the patient (Khanuja et al. 2018). Nurses will be encouraged to
record the risk factors regarding the incident of the fall and develop a care plan for the
patient. Evaluation procedure for fall incident, and the procedure to follow evidence-based
practice will be learned by the nurses through this program.
Dementia assessment and care plan:
The disorder of various domains of brain action creates dementia, which is a disease
of continuous and gradual nature. As a result of dementia, aged people faced the problem of
memory loss, learning capacity, language problem, mood changes, calculation, the problem
with communicating, and judgement problem. Dementia is common for hospitalised patients
of those, whose age is over 70 years. The diagnosis of dementia depends on an accurate
history, taking in conjunction with the relative. Nurses will be encouraged to check all
emergency admission for cognitive deficits with the abbreviated mental test (Robinson et al.
2014). Nurses will learn to minimise the problem, develop the emotional wellbeing of the
patient through this learning program, which will continue for one year.
Delirium assessment and care plan:
Delirium is a state of confusion for the patient. Delirium is a common problem of the
person who has demand in early age. If the problem is not recognised early, it can create an

adverse outcome, including death. The confusion assessment method is the instrument to
screen delirium. This assessment focuses on the patient's history of confusion and the level of
consciousness. Family member’s collateral history or carers will be beneficial to identify the
recent change in the reorganisation (Burhenn et al. 2016). The ability to apply Confusion
Assessment Method and Intensive Care Delirium Screening Checklist for every aged patient
with delirium problem.
The design of the learning and improvement programme of nurses will focus on
several issues. Learner profile means the profile of the nurses. These three nurses of aged
care are employed as part-time. They work in partially overlapping shifts. Emerald Aged
Care fixed a budget of $5,000 for the learning and development program (Kong et al. 2014).
The authority assured that father funds might be available if there is a compelling case and
return on investment. Supporting the nurse's gradual professional development and career and
leadership development are the learning outcome of this program. Through this program, not
sources will acquire knowledge to handle the aged patient. These nurses can provide better
patient care in the community.
For this learning and improvement program, needed resources will be available
through the existing resources of Emerald Aged Care. Via organisational LMS, most of the
needed resources will be available. The existing learning and improvement program of
Emerald Aged Care, cover general workplace training such as diversity, OHS. This program
will be completed in 12 months. Outside working hours will be used to complete the formal
training.
In this learning and development programme, nurses will acquire knowledge to
manage aged patients through constructivism. According to constructivism theory, learners
create knowledge for themselves from communication between their individual experience

and their ideas (Bada and Olusegun 2015). Constructivism theory will be applied in this
learning and development programme. In this learning and development program, the nurse’s
activity creates new ideas based on present and earlier experience (Thomas et al. 2014).
Nurses will be prepared by this learning and improvement program to solve the new problem
in an ambiguous situation. The practical experience of the nurses will be utilised in this
program (Kiraly 2014). In twelve months, nurses will learn every detail of the program.
There are few obstacles and challenges for learning and improvement programme of
Emerald Aged Care. The nurses of Emerald Aged Care have limited nursing experience.
Businesses do not have sufficient work experience with aged patients. The weak interaction
between job staff of the medical centre and other Emerald employees is a problem for the
program. Poor understanding of the performance review process is followed at Emerald.
Conclusions:
Therefore, in conclusion, it can be said that the learning and improvement program
will support the nurses to acquire the necessary knowledge to manage the aged patients of
Emerald Aged Care community. This new program will aid the nurses to learn about fall
assessment and care plan, dementia assessment and care plan, and delirium assessment and
care plan through constructivism theory.
