INTRODUCTION
Person centred care and family centred care is the approach through which any specific
individual or a family can get the healthcare services personally. In this, healthcare professional
is available for providing better health care and delivering of effective services that can lead to
provide improvement in health care. Healthcare professional should deliver effective care and
need to focus on every activity which should not be create negative impact on patient health. In
this, healthcare professional need to focus on patient or family for delivering health care services
all the time. They are needed to provide care for performing every activity which need to
perform by patient or family member for better improvement of health. Daily necessary activity
healthcare professional need to provide help to performing every activity and taking medication
on proper time. This lead to provide better care to patient for their health improvement. Values
and belief of Health Care professional and individual like service user need to perform different
activity. Here, every individual have the right to get decision for their health. Healthcare
professional need to provide them every information which are related with their treatment and
can have impact on their life (Fearns, (2017)). In this report, there is a discussion about health
care worker who can provide person or family-centered care while using values and beliefs
which can be different service user. There is also discussion about on value and belief of
healthcare professional while delivery in care services. There is also discussion about case
scenario which has been faced by healthcare professional or service provider during their
practice which include nature of health condition. This focusing on and information which need
to provide to service user for making informed choice for their health improvement. There is also
discussion about holism, autonomy, empowerment, healthy lifestyle and choice which have
impact on patients lifestyle. There is also a discussion about concept and terms which has been
used by healthcare worker for delivering patient centred care for family centred care for patient.

Person centred care- Person centred care refers to treating a person who is receiving
healthcare services with respect and dignity and are getting involved in all decision related with
their health. Such type of care are known by person-centred care. This is an approach which is
linked with persons healthcare rights for providing healthcare services for health improvement.
