INTRODUCTION
Social determinants of health are the conditions in which people are born, grow, live,
work and age. They include factors like socio-economic status, education, neighbourhood and
physical environment, employment, and social support networks, as well as access to health car.
This report has been covered the case study of the Hayley and her family which is based
on social determinant and their affect on families health.
PART 1 ANALYSIS of the Case
Discussion on health, well- being and social determinant
There are several teams which affect our personal life for example health of the
person, well- being and social determination. This all the terms are connected with us and most
of the people affect from them.
Health- Good health is important for every persona and it is important for the people to careful
about their health (Artiga and Hinton., 2019). As per Hayley and her family's case, health of
the Hayley can be affected because she is suffering from lot's of problem, and she started
smoking as well which is not good for her health.
Well being- Well-being is the experience of health, happiness, and prosperity. It includes having
good mental health, high life satisfaction, a sense of meaning or purpose, and ability to manage
stress.
Social determinant- Social development refers to the process by which a child learns to interact
with others around them. As they develop and perceive their own individuality within their
community, they also gain skills to communicate with other people and process their actions.
Discussion on the social determinants of health and well-being from a UK
The employment rates in the England was recorded as the highest in 2016 and 17
which was the highest in all time . However the in work poverty has increased. In years 2015
to the year 2016 more than 50 present of the people in the household living with the income
below the 60 percent of the medium were small children or working age adults from the
households with someone with the work, up from the 35 percent in the year 1994 to 1995.
Crime can have a wide-ranging effect on people’s health in the UK . The rate of first time
entrants into the criminal justice system reduced in 2017. However, police recorded violent

crimes and violent sexual offences continued to rise in 2016 to 2017, with a larger increase in the
most deprived local authorities.

social inequalities.
Health of the person can be affect from the varies factors such as bad habits such as the
smoking, drinking etc. other things which can affect are not eating proper food and many people
do not like to exercise which affect their health (Cullen and et.al., 2018). Well-being can be
called to the person who are living good life, their body is fit and healthy. It is necessary for the
people to maintain their health so they can be well-being person and they can live good life.
Discrimination in the society is the big problems. People compare others on the basis of their
cast, religion, race etc. which affect on their mind, and they ca be depressurised and face
problems who are suffering from the discrimination problem. Discrimination and social
inequalities are similar to the each other because inequality creates in the community because of
discrimination.
How one or more social determinants can impact on individual health being
From the case of Hayley and her family case, it can be said that social determinants affect
individual health being to the great extent. In this case, husband of Hayley, Jesse works in a
warehouse and recently got promotional opportunity. After getting promotion, he needs to give
long working hours to job and efforts as well which makes him tired. Sometimes he does not
come home at night also. He and Hayley have 3 children with the age of 13, 4 and 4 months. Due
to job promotion and busy schedule of Jesses, Hayley has to take all responsibility of her
children and sometimes she got frustrated because she cannot spare time for her self. It is
essential for Jesses to earn money and give time to his job in order to foster her family inan
effective manner (Adler, Glymour and Fielding, 2016).
All these factors are affecting health of Hayley and her children as well. Due to money
problem and single earner of the family, Jesses find difficulties to give time to her family and
taking care of 3 children lonely is making frustrated to Hayley. It is stated that Hayley is facing
problems in sleeping. She used to smoke but at the time of delivery she topped but now again she
started smoking and have drink in the evening. All these factors is affecting her health as she
cannot spare time to play with children and sometime snap at Jesses and children and later feel
guilty. So, in this context it can be said that social determinants and situation affect health as

Hayley is facing problem in sleeping and want sleeping pills. It can cause many other health
problems like depression, anxiety, heart attack and others.
So, here it can be said that one of the main social determinants is: poverty or single earner
of the family. Income is one of the main social determinants which is affecting health of Hayley
and her children as well. Hayley become rude sometimes and due to this behaviour, children will
not prefer to share their feelings with their mother and will perform some unethical acts and it
can spoil them as well. It can lead continuous anxiety in Hayley, lower self esteem, lack of
control or pre mature death (Artiga and Hinton, 2019). Poor mental health is one of the main
effects which social determinants or especially income has on people. So, it is important for
Hayley to keep calm, adapt situation and manage all works by sparing out time for herself as
well.
Range of community assets or resources will be beneficial to improve health and well being
There are several ways by which all effects which social determinants have can be reduced
or eliminated. There are some common techniques or ways which can help people and Hayley
such as:
Connect: Making themselves connecting with society and other people can make them
feel refreshed and energetic. It is analysed that due to lack of sources of income, husband of
Hayley find difficulties in giving time to his family. It makes Hayley lonely but by making
themselves connecting with society members and community can help her out in improving
health.
Healthy diet: It is stated that Hayley started smoking and drink as she finds difficulties in
sleeping. It can be said that it is just a psychology that by smoking and drink, people can keep
themselves tension free. Increasing controlling or self management skills and taking healthy diet
can make people mentally and physically healthy (Adler and et.al., 2016).
Exercise: It is one of the best ways to keep them refreshed and active. By exercising,
people can improve their health and keep themselves away from diseases. It can help Hayley in
being healthy and lose weight and look like models which she wanted to be.

PART 2 Action Plan
Social determinant of health and well-being as a healthcare professional role
As a healthcare professional, I would analyse the situation and key factors or causes of
the problem in a detailed manner. After analysing factors I would suggest the best technique to
solve them. There are some common and effective ways by which I can improve health of people
such as: suggesting healthy diet and making them able to focus on exercising. Exercise keeps an
individual mentally and physically healthy and they feel energetic. This will improve their health
and they can live better and healthy life
In my Opinion Most of the people are suffering from the health related problem because
they used to smoke and consume alcohol which affect o their health and most of the people
come up with the dangerous disease related to their lungs and liver (Rowlands and et.al.,
2017.). As a professional health care I world like to suggest those people to do not consume
things which can affect their health and they can create problem for the health in the future.
Their are some medicines which helps to overcome from the problems like smoking and
drinking. I will give this medicines to them so they can leave this habit and start living healthy
and well-being life. I will also suggests them to do regular exercise and yoga to increase their
consecration power and they can easily leave this bad habit.
CONCLUSION
From the above case of Jenny it has been summarized that social determinants such as
education, lack of access to health care, inequalities and income played a vital role in affecting
health of people. This study has shown one of the main social determinants which is lack of
source of income in family which affect mental health of family members to the great extent.
Lastly, it has shown some ways to decrease impacts of social determinants.
