In case management, documentation plays a crucial role; it serves as a proof of services
that have been rendered. Assessments, Treatment Plans, Progress Notes, Quarterly Reports and
Incident Reports are types of Documentation that are utilized in the field of Case management.
Assessments help during the process of case assignment; it helps in the provision of information
that determines the client’s needs (Frankel, Gelman & Pastor, 2018). Treatment plans are a type
of documentation that guides the clients on the direction of services and this documentation helps
to determine the needs of the client and what they might like to achieve. Progress Notes which
are also known as the case notes help in the provision of information that shows up to date
progress that have been made. This documentation is usually needed to be completed from 24
hours to 5 days of time depending upon the policy of the company (Zhi et al., 2015). Quarterly
reports are also a kind of progress report but it provides the progress that has been made over a
course of a three month period. This documentation demonstrates progress towards the goals of
treatment. Incident Reports are the process of documentation that gives critical information at
times of serious events like any medical emergency, death or any injury. Incident Report starts a
chain of documentation which covers from the time of the occurrence of that incident to the
actions that were undertaken as a consequence. For any profession of human service,
documentation is thus a critical aspect since it determines the eligibility and the planning of the
services and provides evidence of such services.
