The clinical scenario I experienced was the case of a geriatric patient who had been
readmitted severally at the hospital due to falls. None of her information was available at the
records department. I was asked to try and retrieve information from the wards. I searched on
the centrally located computer on a nursing unit, and found the information about the patient.
I accessed all the data required to understand the health history of the patient, previous
treatment plans and interventions.
From the data, which I shared with the multidisciplinary healthcare team, I learnt that
the patient has dementia and the health condition was deteriorating. Each time she was
admitted at the hospital, she was treated for injuries related to falls. Using the data, the
multidisciplinary team decided to change the clinical intervention to focus on falls prevention
as a strategy for reducing hospital readmissions.
The foundation of knowledge model articulates that information gathered is only
useful when it has a purpose, use and application, and so long as it is reliable, viable, accurate
and timely. The DIKW model entails transformation of data to information, knowledge and
wisdom in order to conduct suitable actions and make appropriate decisions. Based on the
models, the information gathered from the computer in the nursing unit regarding the patient
was essential for understanding the health status of the patent, the treatment and outcomes.
The data was used to generate new information regarding an alternative treatment plan. The
obtained data provided information that the patient has dementia, and knowledge indicated
that dementia may be a risk factor of falls. Wisdom dictated a need for interventions to
improve cognitive abilities of the patient and prevent falls.
