INTRODUCTION
Nursing practices play an important role in service delivery to the patient for their health
improvement. In this, using evidence-based practices are highly effective and allows to deliver of
quality care to the patient for effective health improvement. These practices may include delivery
assessment, development of plan, implementation and evaluation of outcomes. Nursing staff
have the responsibility to take care of the patient's health and should practice evidence-based
nursing practices.

While delivering proper care and services, many functions need to be performed by
nursing staff. In this, there is a risk of human error which may include medication error.
Medication error is identified as one of the patient safety issues while delivering healthcare
services. Nursing staff needs to perform their role to care through performing all the functions.
They should fulfil patients' needs and should provide all the necessary things like helping in
different activities like going to the washroom and others while providing medication at right
time along with the right dose. Nursing staff are busy performing these processes there can be a
risk of human error that can create a risk of life or decline health issues within the patient.
Medication plays a potential role in delivering effective healthcare services for patients health
improvement. Administering wrong medication can be a harmful and life-threatening issue for
any patient and nursing staff. There can be a risk of negative health or sometimes patients may
die. To avoid such types of patient safety issues, it is crucial to focus on its intervention which
can have a positive effect and provide healthcare safety to a patient to occur such type of harm.
Clinical question (PICO)
PICO refers to a concept to provide information that includes patient, problem,
population, intervention, comparison, outcomes. It provides a foreground for the research
question and is used to implement strategies for better implementation of research.
PICO question
 Problem- Medication error in nursing practices.
 Intervention- ensure about right dose, ensure about right medication, neglect.
 Comparison- patient correct information

 Outcomes- Minimise medication error.
Strategy
Medication errors should be avoided and for this, there is a need to use various strategies
that should be effective and have better outcomes. There is a need to follow various strategies
that can have a high positive impact to reduce the risk of medication error while nursing
practices. Some of the strategies which many include are like ensuring patient information,
ensure about adequate communication, medication storage, ensuring about correct drug
according to prescription and others. The above clinical question can provide various evidence
about the medication error that can hurt the patient's health. In this, there is the establishment of a
medication error, error in medication, wrong dose, error in providing medication, wrong
prescribed medication can help to get various searches.
