INTRODUCTION
The assessment and management of dementia is termed to be complex and crucial. As it
takes an individual to suffer number of issues associated with it. General health care practitioners
are often the first person to connect and consult for people who suffering from dementia. As
health care and social care practitioners are guided skilled well to provide quality care and
assistance to an individual. The care can termed to be primary and quality care should be assisted
to a n individual. I the following report, there is case of study of a person who is suffering
dementia and suffering from number of issues which are associated with following disease
(Rosenberg, 2017)

Leone is 27 years old and a qualified social worker. She is fixed a meeting Humerah who
