Identify who is the primary client/patient.
The prime client, in this case, is Rob, who is 85 years of age. He is diagnosed with
diabetes mellitus which he has kept in control by himself. Now, he is showing the signs of the
first stages of dementia as he had started to behave in an agitated and disoriented manner and
showing signs of memory loss.
Describe the main issues or concerns for the client/patient in the scenario.
The main concerns for the client are that he has moved from his family home to a
small unit when his wife aged 60 years died from cancer. He mostly lives alone and remains
sad after his wife died due to cancer. He has withdrawn from all of his social networks. He
takes less care of himself now. He was recently admitted to the hospital when he started
acting differently. He started behaving in an agitated and disoriented manner. He has started
to behave in a way which shows early signs of dementia (Arunachalam & Mohan, 2018). He
also at times fails to recognize her elder daughter Sally and is even not aware of her presence
at times.
Identify all the types of health and human service professionals that would
be involved in the interdisciplinary team/case conference for this scenario.
Health and human services professionals helps in finding out the resources that are
available for the clients to offer them. They design programs that assist their clients. They
provide client services, like supporting the family of the patients, rehabilitation,
psychological help, social work, and health services. The range of health and human service
professionals include nurses, child welfare, psychologists, social workers, counselors,
therapists, and sociologists to name a few. The child welfare workers work with families to
create a safe and secure environment to keep families together. The safety of the child is the
primary focus of such a service. They assist in chronic and crisis situation. They help their
