1INVESTIGATION PRINCIPLES AND ANALYSISExecutive summaryThe report is prepared to describe the two accident analysis tools Acci-Map and STAMP. Thisincludes its key characteristics and failures that are leading to the accidents. Then an Event Tree,PEEPO and a Timeline are prepared for the Industrial Domain at Longford.

3INVESTIGATION PRINCIPLES AND ANALYSIS1. Introduction: The accident analysis has been carried out for determining the reason for the accidents or thesequence of accidents. This is done to prevent the future events of the same kind (Underwood andWaterson 2014). The report has chosen the two accident analysis tools Acci-Map and STAMP describing itskey characteristics and failures, leading to the accidents. Lastly, an Event Tree, PEEPO, and aTimeline are prepared for the Industrial Domain at Longford. 2. Part A: The analysis tools: 2.1. Acci-Map: The approach of AcciMap has been a system based technique. This is done for the accidentanalysis particularly to analyze the reasons of incidents and accidents occurring in the complicatedsocio-technical systems. 2.1.1. The reason behind choosing the Acci-Map:Accimap has been attempting to model the dynamic communication taking place between thevarious socio-technical levels and the roles it has been playing in any accident over due time. It hasbeen providing the insights into the human error and accidents that have arisen quickly in theprevious years (Underwood and Waterson 2013). This could be applied to various domains from theindustries like public health to the aerospace. They have been easy in usage and delivering thegraphical results making that properly suited while interacting with the broader audience. Moreover,it has been reading to be used in conjunction with the other techniques of accident analysis. This hasbeen including the Swiss Cheese Modeling, Root Cause Analysis and so on. 2.1.2. The key characteristics of the Acci-Map:
