Leading and Learning
No Patience for Poison is a tale by Brenda Carle that details her experience with a
physician who overlooks her opinion. The patient has not woken up, and Brenda feels that it is
time to question everything. The cardiologist feels that they should give the patient more time,
but a voice in Brenda tells her that she should proceed with her questioning. She proceeds to
phone Tucson Poison Control who agree that the thiocyanate level should be assessed (Carle,
2010, p14). It is revealed that the patient had not woken up because of thiocyanate poisoning.
The surgeon, though goes silent for a while, thanks Brenda for her efforts and the patient wakes
up following the remediation (Carle, 2010, p15). Brenda feels happy to have questioned the
status quo and challenge a physicians that the patients get the best care. She is happy to make the
patient’s family happy.
It is a tale of courage, but one that illustrates that sometimes those daring steps may be
rewarding. The experience opens up the environment, which is very important for practice.
Brenda decides that she won’t just sit there and watch the patient, not while she is strongly
convinced that there is need consider the thiocyanate level. The quality of care is heavily
dependent on attributes of the care environment (Siedlecki & Hixson, 2015). Effective
communication has an influential role on the success of any healthcare system, and efforts
towards streamlining it are welcome (Amudha, et al. 2018). Training is one of the mechanisms
that can help improve communication between RNs and physicians (Wang, et al. 2018).
The leadership challenges that are projected in this story include the struggles that RNs
experience when trying to communicate with physicians. Brenda tries to express her concerns,
but the physician sweeps them aside as he feels that his status does not allow him to consider the
