The key strategy to gain sustainability is mainly aims to become more person-centred and
make better use of data. It encourages NHS to be innovative and give high contribution for social
care reform. Such strategy also based on evaluating opportunities for use new techniques in
health and social care for next ten years. In addition to this, it also includes policies for
increasing compliance and consistency; improving quality of services; increasing opportunities
for sharing good practices and improvising health condition of sick especially operated-patients.
It reveals actions require for improving accessibility and sharing e-patient record within NHS to
deliver high services. Therefore, to manage and run social enterprise, management of respective
company concerns more maintaining sustainability in business. In this regard, for establishing
healthcare app to monitor medical conditions of post-operative patients on individual basis,
management has formulated following strategies (Bolton, 2016). In this regard, caring for the
population of patients facing complications after operations in regular check-up, requires a new
paradigm. To a large extent, for developing and implementing such a system will hinge on
addressing the following four main challenges:-
Realigned Reimbursement: In general, patient after operations face lot of issues to
come hospitals for regular check-up. Therefore, there must be a mechanism in NHS, whereby
service providers are compensated to make interaction with such people after discharge and take
records of their health through mobile app. This would help in maintaining electronic records of
patients and provide timely services to them before discrepancies.

Team-Based Care: In order to provide effective and efficient services, it is essential for
management of NHS to concern on development of required skills and abilities of team-
members. It includes associated team members of health care industry- nursing, social work,
community health coaching and pharmacy. Therefore, they must be trained and available to
coordinate post-operated patients and their relatives in effective manner.
Patient and Family Engagement: For improving abilities of patients to manage their
health conditions and adhere to treatment plans, in own manner, NHS also concerns on expanded
opportunities for such individuals and family engagement in self-management programs (Baum,
2016). This would help in make people able to care themselves and prevent from adverse affect
which might cause due to carelessness after operations.
Information Sharing: The current techniques which is focused on medical record
system must be redesigned within NHS. The main aim behind this change is to improve abilities
of clinicians to share information related to patients and facilitate use of evidence-based decision
support in their care.
