Physical, mental and social health related key question
There are several questions asked for knowing what are the different ways to
manage own health (Montagnese, S. and Bajaj, J.S., 2019). There is need to
know the person’s information about smoking and drinking habits in order to know
exact what is the problem faced by the person. Screening is done by using
questionnaire in which questions are asked about diet, alcohol consumption and
smoking tobacco. Some of the questions are given in next slide

Have you ever smoked?
. Have you tried to quit in the past, if so how?
7. Do you currently have any smoking/tobacco chewing related
illnesses? If so which (optional):
Do you know people that smoke i.e. friends or family?
In a typical month, how often do you have a drink containing
alcohol?
. How often during the last year have you failed to do what was
normally expected from you because of your drinking?
